Daytime sleepiness among the elderly may indicate cancer risk

Older people who experience daytime sleepiness even after having adequate sleep at night may be at the risk of developing new medical conditions, including diabetes, cancer and high blood pressure, says a study. The condition called hypersomnolence is defined as excessive daytime sleepiness even after having seven or more hours of sleep.

It can be debilitating for some people, affecting the way that they perform at work and in other daily activities. "Paying attention to sleepiness in older adults could help doctors predict and prevent future medical conditions," said study author Maurice Ohayon of Stanford University in California.

"Older adults and their family members may want to take a closer look at sleeping habits to understand the potential risk for developing a more serious medical condition," said Ohayon.

The study involved 10,930 people. About 34 per cent of the participants were 65 years or older.

The findings are scheduled to be presented at the American Academy of Neurology's 72nd annual meeting to be held in Toronto from April 25 to May 1.

For the study, the researchers interviewed participants over the phone two times, three years apart. In the first interview, 23 per cent of people over 65 met the criteria for excessive sleepiness.

In the second interview, 24 per cent reported excessive sleepiness. Of those, 41 per cent said the sleepiness was a chronic problem.

The study found that people who reported sleepiness in the first phone interview had a 2.3 times greater risk of developing diabetes or high blood pressure three years later than those who did not experience sleepiness.

They were also twice as likely to develop cancer.

Of the 840 people who reported sleepiness at the first interview, 52 people, or 6.2 per cent, developed diabetes compared to 74 people, or 2.9 per cent of those who were never sleepy during the day.

Also, of the 840 people who reported sleepiness, 20 people, or 2.4 per cent, developed cancer compared to 21 people, or 0.8 per cent of those who were never sleepy during the day.

People who reported daytime sleepiness during both interviews had a 2.5 times greater risk of developing heart disease. Those who reported sleepiness only in the second interview were 50 per cent more likely to also have diseases of the musculoskeletal system and connective tissue, such as arthritis, tendinitis and lupus, than those who did not have daytime sleepiness.

People with diabetes, hypertension are Vitamin D deficit

A pan-India study has linked Vitamin D deficiency and insufficiency to type-2 diabetes and hypertension, making it about more than just bone health.

Vitamin D, also known as the sunshine vitamin that is produced by the skin on exposure to sunlight, is a critical micronutrient that helps regulate the amount of calcium and phosphorus in the body that are required to keep bones, teeth and muscles healthy.

The study shows that 84.2% of type 2 diabetes patients and 82.6% of hypertension patients were Vitamin D deficient, contributing further to the body of research that links Vitamin D deficiency to chronic non-communicable diseases (NCDs).

The overall prevalence of patients with low Vitamin D levels (deficiency and insufficiency) was 83.7%, of which 82% were newly diagnosed cases.

“Our aim was to look at the levels of vitamin D in patients suffering from type-2 diabetes and hypertension to establish a link. We are not saying the deficiency has caused the disease but that those who with the disease may be vitamin D deficient,” said the principal researcher.

“The next step will be to see how improving vitamin D levels in these patients will impact the treatment outcome, since we are always looking for any additional pathways that will further reduce complications in patients with diabetes and hypertension,” he added.

Correcting vitamin D deficiency should not be the top priority for managing diabetes and  hypertension.

“If you are looking from the public health point of view, then vitamin D deficiency will not be on my list of top 5 things to handle diabetes and hypertension. One needs to prioritise. However, if looking at musculoskeletal disorders (pertaining to joints, muscles, nerves etc.) than vitamin D deficiency will be a priority. Having said that, I don’t mean there should be no research around it,” said the head of the department of endocrinology. 

The causes of vitamin D deficiency in India are manifold. Most people do not receive adequate exposure to sunlight, as modernized lifestyles have resulted in less time spent outdoors for work or leisure. High levels of air pollution further hamper the transmission of ultraviolet B (UVB) rays needed to synthesize Vitamin D in the skin.

A social fetish for fair complexion also leads many to avoid exposure to sunlight, especially during noon hours when it is most beneficial.

The study tracked 1,501 adults above 18 years with diagnosed type-2 diabetes, hypertension, or both, across 29 sites between June to September, 2017. 

Vitamin D insufficiency and deficiency was defined as serum 25(OH)D levels 21-29 ng/ml and ≤20 ng/ml, respectively.

“These findings highlight the need for routine screening to ensure early diagnosis and effective management of Vitamin D deficiency to help reduce the burden and risks associated with non-communicable disease,” said the Dr.

Apart from exposure to sunshine for about 45 minutes, the dietary sources of Vitamin D include cod liver oil, salmon fish, mackerel, sardines, tuna, egg yolk and mushrooms exposed to sunlight.

Rest may be best medicine to treat some pains

Getting rest may be as effective as taking medication to treat common musculoskeletal disorders such as carpal tunnel syndrome and low back pain, which caused by repeated moderate-strain activitym, scientists have found.

In a study conducted on rats, researchers showed that a  4 week period of rest was nearly as effective as an experimental drug at reducing discomfort and regaining function.


Rest appears to be good first treatment choice, said a researcher.


Other treatments following rest should include exercises to improve tissues, and pain reducers if needed. If symptoms do not subside and strength does not return following this approach, then pharmacological treatments may be needed to aid recovery, the researcher said.


The researchers trained rats to repeatedly pull a lever in order to model the types of repetitive-strain injuries humans experience.


After 12 weeks, the rats showed a weaker grip strength, discomfort and increased collagen production in the forearm- evidence of strain-induced tissue damage.


The researchers then allowed the rats to rest for 4 weeks. During this rest period one group of rats received an experimental drug that blocks a neurotransmitter. Substance P, which is associated with the sensation of pain, while the other group did not.


We found that the 4 week rest treatment reduces collagen production, improved streg=ngth and decreased discomfort, said the Dr.


The drug blocking Substance P signalling also had similar effects, and was somewhat better at improving grip strength, but at the same time made the animals less sensitive to very hot temperatures, which is concerning. This led us to conclude that rest may be the better treatment, she said.


There appears, however, to be a tipping point at which rest is not sufficient and medications are needed, researchers said.


While the study showed rest was sufficient for recovering from repeated moderate-strain activity, previous studies indicated it was not sufficient when the lever was harder to pull, causing more strain.


In investigating the Substance P-blocking agent and other drugs, the researchers are focused on developing ways to help the tissue actually recover from overuse-induced injury, not jsut relieve pain.


In previous experiments the researchers tried giving the rats ibuprofen and other pain relievers while they were performing the lever pulling task, however, this seemed to only mask the pain while allowing rats to continue to pull hard, worsening tissue damage.

If you are one of those who steps out in heels frequently, you may want to change your habit as a new study shows that injuries related to high heels have doubled in a decade.

The research by University of Alabama at Birmingham showed that between 2002 and 2012, the frequency and severity of those injuries were sufficient to suggest that wearing the appropriate shoes for the appropriate occasion and being aware of one's surroundings are good ideas.

Vice chair and Professor Gerald McGwin, Ph.D., who led the study, said that although heels might be stylish, from a health standpoint, it would be worthwhile for those interested in wearing high-heeled shoes to understand the risks and the potential harm that precarious activities in high-heeled shoes can cause.

In addition to discomfort in the lower leg, ankle and foot, research has indicated that walking in high-heeled shoes has been shown to significantly reduce ankle muscle movement, step length, total range of movement and balance control. Many studies have documented that the long-term use of high heels alters the neuromechanics of walking and places greater strain on the muscles and tendons of the lower legs, which can lead to musculoskeletal disorders later in life.

McGwin's team looked at data of injuries associated with wearing high heels seen in hospital emergency departments between 2002 and 2012. There were 123,355 high-heel-related injuries seen during that period. The peak year for injuries was 2011, with more than 19,000. People between the ages of 20 and 29 were most likely to suffer an injury, followed by the 30-39 age group.

The vast majority of the injuries, over 80 percent, were to the ankle or foot, with just under 20 percent involving the knee, trunk, shoulder, or head and neck. More than half were strains or sprains, with fractures accounting for 19 percent of all injuries. While white females as a group had the largest number of heel-related injuries, the rate of injury for black females was twice that of whites.

The findings are published online in the Journal of Foot and Ankle Injuries.
