Israeli scientists find way to treat pancreatic cancer in 14 days

A new treatment developed by Tel Aviv University could induce the destruction of pancreatic cancer cells, eradicating the number of cancerous cells by up to 90% after two weeks of daily injections of a small molecule known as PJ34.

Pancreatic cancer is one of the hardest cancers to treat. Most people who are diagnosed with the disease do not even live five years after being diagnosed.

The study, led by Prof. Malka Cohen-Armon and her team at TAU’s Sackler Faculty of Medicine, in collaboration with Dr. Talia Golan’s team at the Cancer Research Center at Sheba Medical Center, was recently published in the journal Oncotarget.

Specifically, the study found that PJ34, when injected intravenously, causes the self-destruction of human cancer cells during mitosis, the scientific term for cell division.

The research was conducted with xenografts, transplantation of human pancreatic cancer into immunocompromised mice. A month after being injected with the molecule daily for 14 days, “there was a reduction of 90% of pancreatic cells in the tumor,” Cohen-Armon told The Jerusalem Post. “In one mouse, the tumor completely disappeared.”

“This molecule causes an anomaly during mitosis of human cancer cells, provoking rapid cell death,” she said. “Thus, cell multiplication itself resulted in cell death in the treated cancer cells.”

Moreover, she said, PJ34 appears to have no impact on healthy cells, thus “no adverse effects were observed.” The mice, she said, continued to grow and gain weight as usual.

She added that she first published about the mechanism in 2017 when it was used to effectively treat triple-negative breast cancer implanted in xenografts. This type of breast cancer – which tests negative for estrogen receptors, progesterone receptors and excess HER2 protein – like pancreatic cancer, is very hard to treat and many women don’t live more than five years after being diagnosed.

Though Cohen-Armon said the team did not specifically study whether or not the treatment could prolong the lifespan of a patient, one can assume such an effect could result if the cancerous cells are eliminated.

How long will it take to move from mice trials to human trials?

She estimates that would take “at least two years on the condition that we get enough funding.”
First, she said, the group will test the treatment on pigs and then apply for permission from the FDA to administer humans with this molecule.

“I am optimistic,” Cohen-Armon concluded.

Consuming starch enriched food can replace insulin injections for diabetics

Consuming a starch-rich breakfast early in the morning followed by a light dinner could replace insulin injections and other medications used by many diabetic patients, says a recent study. People diagnosed by Type 2 diabetics usually inject themselves with insulin, a hormone that regulates the movement of sugar into liver, muscle and fat cells, up to four times a day.

But insulin injections are linked to weight gain and the loss of control of blood sugar levels. This triggers a vicious cycle of higher insulin doses, continuous weight gain, a higher incidence of cardiovascular disease and other complications.

According to the research, our metabolism and biological clock are optimised for eating in the morning and for fasting during the evening and night, when we are supposed to be asleep.  A Prof  said: "But the usual diet recommended for Type 2 diabetes consists of several small meals evenly distributed throughout the day. For example, three meals and three snacks daily are including a snack before going to sleep to prevent a drop in sugar levels during the night."

The research was published in the journal of Diabetes Care.

According to Prof., the traditional diabetic diet specifies six small meals spread throughout the day. "But our research proposes shifting the starch-rich calories to the early hours of the day. This produces a glucose balance and improved glycemic control among Type 2 diabetics."

Thus, explaining the reason behind this theory of diet, he said: "We believe that through this regimen, it will be possible for diabetics to significantly reduce or even stop the injections of insulin, and most of the anti-diabetic medications, to achieve excellent control of glucose levels."

The researchers studied 29 Type 2 diabetes participants and compared a new '3M-diet,' more in alignment with our biological clock, with a control group on the traditional 6M-diet. The experimental 3M-diet comprises a meal of bread, fruits, and sweets in the early hours of the morning; a substantial lunch; and a small dinner specifically lacking starches, sweets, and fruits.

The group on the traditional 6M-diet did not lose weight and did not experience any improvement of sugar levels, requiring an increase in medication and insulin doses. But the group on the 3M-diet not only lost weight but also experienced substantially improved sugar levels. Prof Jakubowicz is of the view: "In addition, the 3M-diet improved the expression of biological clock genes. This suggests that 3M-diet is not only more effective in controlling diabetes."

The upregulation of the biological clock gene expression in the 3M-diet might be the mechanism behind its success, as it enhances insulin secretion and improves sugar delivery into the muscles, creating a balanced daytime and nocturnal glucose metabolism.

Steroid injections for hip and knee may be harmful

Commonly given hip and knee steroid intra-articular injections may be harmful to some patients with at-risk conditions, according to a new study.

Corticosteroid injections are often given to reduce pain and inflammation  from osteoarthritis.

Researchers  have found accelerated arthritis and joint destruction  can be the unintended result of intra-articular corticosteroid injections.

“We are now seeing that these injections can be very harmful to the joints with serious complications such as osteonecrosis, subchondral insufficiency fracture and rapid progressive osteoarthritis,” said study author .

“Intra-articular corticosteroid injection should be seriously discussed for pros and cons. Critical considerations about the complications should be part of the patient consent which is currently not the case right now,” he added.

Osteoarthritis of the hip and knee is among the most common joint disorders.

A frequently performed treatment for osteoarthritis and other joint related pain syndromes are intra-articular corticosteroid injections, yet there is conflicting evidence on their potential benefit.

For the study, the researchers conducted a search on patients they had injected in the hips and knees during 2018.

They found that eight per cent had complications, with 10 per cent in the hips and four per cent in the knees.

The researchers also suggest that the radiologic community should actively engage in high-quality research on this topic to better understand potential at-risk conditions prior to intervention, and to better understand potential adverse joint events following these procedures to avoid possible complications.

Back pain for hubby- new treatment

For more than 5 months hubby has been having back pain. Earlier, it was mild, once he used to lie down the pain used to disappear. Then I had a suspicion if due to his falls, if he had a hairline fracture somewhere. Luckily no fracture, this was a young orthopedic than my surgeon who was very experienced and sadly he is no more, as when he did my surgery itself, he was in his 70s.

This young surgeon wanted to rule out any knee problem as hubby told him his legs buckle, hence he falls down. Luckily nothing wrong with the knees. His spine is slightly curving. He wanted him to have an MRI done, due to phobia for closed place first he refused, then when his pain was worsening, he agreed. It showed that he has problems in L4-L5 and in his tail bone. So, we\re worried if he was going to suggest any surgery, as he had double bypass surgery for his heart.

Luckily, the treatment was such that he had to lie on his stomach, with a monitor / new type of scanner to show them the various points in spine, surgeon with an interventionist, gave injections in different places to numb the sensory nerves ! Hubby was fully conscious, the whole procedure lasted for less than 45 minutes. It was done by giving local anesthesia. When the surgeon asked him if he can see the scanner, hubby said, it would have clearer if it was in colour !

Yesterday, soon after the procedure, as his back was still numb, he said no pain, slowly as hours progressed, he started complaining of pain, but didn't want pain killer nor hot massage. But then this morning, he said the pain was comparatively less on getting up, as earlier, soon after getting up from bed itself, the pain was awful. The surgeon said, it varies from person to person, some feel total relief soon after the procedure, some make take days, so till then walking is limited to the bar essentials, no exertion till we see the surgeon after 3 weeks.

Both of us need lumbar belt, at least in one thing, we agree, wearing belt !

So, I was with hubby in the hospital from yesterday morning  till this afternoon when he got discharged. As, I'm the one, who had 13 surgeries, when hubby used to run around to pay, now, it is my turn to repay back all his services !

I used my time in the hospital to finish 2 pairs of breast prosthetic and almost finishing 1 more now. Hence, I'ld not post any post yesterday.

Wishing one and all good health, as one of the biggest festivals which is celebrated all over India with a lot of fervour starts from this Sunday and lasts for 10 days. The 10th day is celebrated as victory of good over evil !

Contraceptive injections do not increase risk of contracting HIV

A landmark study has ended 30 years of anxiety that hormonal contraceptive injections may increase women’s chances of infection from HIV.

But the study found a dramatically higher rate of HIV infection among women in southern Africa than was expected, which one leading campaigning organisation said signified a public health crisis”.
In many African countries with high HIV rates, women predominantly use DMPA-IM, known as Depo-provera – three-monthly injections that allow them to keep their use of contraception secret from their partner.

For three decades there have been concerns and in 2011 the Lancet published evidence that women using the progestogen-only injections to avoid pregnancy were twice as likely to become HIV-positive.

In 2012 the World Health Organization advised couples to use condoms as well, which is not always negotiable in some communities. In sub-Saharan Africa, nearly 60% of all new infections occur in women. There were calls for new methods of contraception that would also prevent HIV and be controlled by women themselves.

Further research published in 2015 found that women using the injections for contraception were 40% more likely to get HIV. However, the authors said the dangers had to be balanced against a woman’s risk of an unwanted pregnancy, which could also jeopardise her life. The WHO said in 2017 that women should be advised of the risk.

The new study, called Echo (Evidence for Contraceptive Options and HIV Outcomes),  was designed to put an end to the controversy. It compared the HIV infection rates among women using hormonal injections with those using an IUD (intrauterine device or coil) or an implant. It involved more than 7,800 women in Eswatini, Kenya, South Africa and Zambia.

“Our randomised trial did not find a substantial difference in HIV risk among the contraceptive methods evaluated, and all methods were safe and highly effective at preventing pregnancy,” said one of the study’s co-authors.

The WHO will now review its guidance and is likely to say that women should no longer be concerned about the injectables or have to choose between the possibility of pregnancy and the fear of HIV infection.

But there is alarm at the high rate of HIV infection among the women in the trial – which was 3.8%, and worse in women under 25. The infection rate was higher than the “substantial risk” rate of 3% that the WHO says should trigger the use of PrEP (pre-exposure prophylaxis), a daily antiviral drug which protects against infection.

“The study highlights the need to step up HIV prevention efforts in these high-burden countries, particularly for young women,” said a Dr. , of the WHO’s HIV and Hepatitis department. “These should include providing HIV testing and a range of HIV prevention choices within contraceptive service programmes.”

A member of the study’s management committee, said: “The results on this question are reassuring but our findings are also sobering because they confirm unacceptably high HIV incidence among young African women.”

AVAC, a global HIV advocacy organisation, said there was good news and bad news. The trial had shown that the injectables were safe. On the other hand, it “delivered a sobering reminder that women and girls in east and southern Africa are still at very high risk of HIV infection. An overall HIV infection rate of almost 4% in the study points to a public health crisis for women in the region,” said  AVAC’s executive director.

The trial participants were not considered to be at high risk. “The women in this trial are our sisters and daughters and mothers who were simply seeking contraception,” said executive director of the International Community of Women Living with HIV/AIDS Eastern Africa. “It is a wake-up call to put HIV prevention onsite at every family planning clinic, including PrEP and female condoms with peer support and trained providers.”

A member of the global community advisory group for the Echo study, said women needed more options for contraception combined with HIV protection. “Regardless of the data from the Echo trial, the limited choice of contraceptives that women have is not OK. We hope that this result will prompt action and put women first. Women want more options,” she said.

Roche launches Hemophilia A treatment drug in India

Drug firm Roche Wednesday said it has launched Emicizumab under the brand name Hemlibra in India for preventive treatment of Hemophilia A, an inherited disorder in which a person's blood does not clot properly.

Hemlibra is the first weekly subcutaneous prophylaxis injection shown to prevent or reduce the frequency of bleeding episodes and improve the quality of life, Roche said in a statement.

All current prophylactic treatment options for people with Hemophilia A with factor VIII inhibitors require intravenous infusions several times a week, it added.

"The introduction of Emicizumab (Hemlibra) is a significant milestone in the treatment of Hemophilia A in India and reaffirms our commitment to bring Roche's ground breaking medicines to patients in India as early as possible," said a spokesperson of the company.

This breakthrough medicine represents a completely new way to manage Hemophilia A and redefines the standard of care, she added.

The company, however, did not disclose the price at which it would be selling the drug in the country.
Hemlibra is approved by multiple regulatory authorities across the world and is now also approved and available in India, the statement said.

Hemophilia A is an inherited, serious disorder in which a person's blood does not clot properly, leading to an uncontrolled and often spontaneous bleeding.

Parkinson's disease: Popular western medicine Apomorphine now launched in Bengaluru

In a boon to patients in the middle stages of Parkinson’s Disease, a private hospital in Bengaluru, in association with UK-based specialty pharmaceutical company, has launched Apomorphine for the first time in India. Apomorphine, available both as injections and infusion pumps, has been popular in the West for over 15 years in the management of Parkinson’s, but Indian patients could not benefit from it until now because of a lack of regulatory approvals. Now, it will be given on compassionate grounds to a select number of patients.

This powerful new drug stimulates the production of dopamine by nerve cells in the brain, providing quick and effective relief to patients and improving their quality of life. A Parkinson’s Disease and Movement Disorders Specialist,  where the drug will be available, said, “Currently in India, patients of Parkinson’s have only two options: either oral treatment for the early stages of the disease, or very expensive Deep Brain Stimulation (DBS) surgery for advanced stages. There was no treatment for the middle stages of the disease. Most patients in India cannot afford DBS. This procedure is also not a choice for all ages or stages of Parkinson’s Disease.”
“Introduction of apomorphine in India has given an important medical option to doctors in the middle stages of the management of Parkinson’s Disease,” Dr.  said. Dr.  said once the Drug Controller General of India approves the drug it will be available at other hospitals. The prevalence of Parkinson’s Disease in India is 300-400 out of 100,000, which is expected to double by 2030.

Ways To Manage Hand Pain

The human hand is made up of 27 bones and a number of tendons and ligaments. An injury to any one of these bones or a disease that affects the bones and ligaments can make you experience excruciating pain in your hands. This pain can affect your ability to perform simple day to day tasks like holding a spoon or writing. Some common reasons your hand may pain are muscle strains, fractures, arthritis and carpal tunnel syndrome. Apart from pain relieving medication, there are a number of ways you can deal with this pain.
  1. Hot and cold compresses: Hot and cold therapy is ideal for any type of muscle or ligament injury. A cold compress helps in reducing the pain and inflammation by constricting blood vessels and numbing the area. On the other hand, a hot compress dilates blood vessels and increases the flow of blood and oxygen to the affected area. Ideally, the two should be used alternatively for 10 to 20 minutes each. A bag of frozen vegetables or ice wrapped in a towel makes an easy cold compress. To treat your hand with heat, use a moist heating pad or a hot water bottle. You could also dip your hands in warm water.
  2. Massage: A massage is a great way to treat hand pain that results from carpal tunnel syndrome. If the exact cause of pain is not known, massage the entire hand, wrist and forearm. A massage enhances blood flow to the area and hence improves flexibility and opens locked tendons. Using a few drops of essential oils such as lavender or  rosemary can also aid in relieving hand pain.
  3. Hand exercises: If your work involves repetitive actions like typing, knitting or embroidery, the muscles in your hand can get locked into position thereby, causing pain and discomfort. To relieve this it is important to take a break from this repetitive exercise every hour and flex your hand. Try bending your fingers forwards and backwards and rotating your wrists in both directions to ensure that your joints remain smooth.
  4. Epsom salt water bath: For instant relief from intense hand pain, soak your hands in a bath made of hot water and Epsom salts for 10 to 20 minutes. Follow this by wrapping your hands in a warm, wet washcloth and applying gentle pressure over the hand for 5-10 minutes. You should instantly feel relief from the pain and experience increased the flexibility of hand.
  5. Injections :An injection of a corticosteroid into a joint can reduce inflammation. "The relief it provides can last up to a year," says Dr. For some people the amount of relief diminishes with subsequent injections.

    6. NSAIDs - A nonsteroidal anti-inflammatory drug (NSAID) helps relieve hand pain by blocking enzymes that produce pain and swelling, but Dr.  says it's not effective for carpal tunnel syndrome. Long-term use of oral NSAIDs such as ibuprofen (Advil, Motrin) are linked to ulcers, stomach bleeding, liver damage, and increased risk of heart attacks. Topical NSAIDs, such as diclofenac (Voltaren), may pose less risk.

    7. Splinting :A splint stabilizes the position of your fingers, thumb, or wrist. "Wear a splint for a few weeks if arthritis flares, so the inflammation can settle down," says Dr.
