Omega-3 fatty acids may prevent scarring around the breast implants

Scarring around the breast implants called capsular contracture during reconstructive or cosmetic breast surgery is a common, costly, and painful complication in women receiving them.

The anti-inflammatory effects of omega-3 fatty acids, found in fish oil, might help to avoid abnormal capsule formation suggests an experimental study in the March issue of Plastic and Reconstructive Surgery®, the official medical journal of the American Society of Plastic Surgeons (ASPS).

Mice fed omega-3 fatty acids had reduced capsule formation occurrence around implants - likely due to reduced collagen deposits - according to the research by Giuseppe A.G. Lombardo, M.D., Ph.D., of Cannizzaro Hospital in Catania, Italy and Serena Tamburino, MD, of Chi.Pla Plastic Surgery in Catania, Italy, and colleagues. "We believe that omega-3 supplement is a simple and promising method that could be used to prevent or at least reduce capsular contracture after silicone implant surgery," the researchers write.

When any foreign tissue is implanted, the body naturally develops connective tissue around it. But in capsular contracture, the tissue hardens into a shell that becomes thicker and firmer over time. This results in inflammation, pain, and sometimes dislocation or deformation of the implant. Repeat surgery is sometimes needed to remove the capsule and replace the implant.

"Past research on preventing capsular contracture has focused on reducing bacterial contamination using antibiotics," Dr Tamburino comments. "Fewer studies have focused on controlling the inflammatory process, particularly the arachidonic acid cascade, the main pathway leading to inflammation."

Omega-3 fatty acids - found in oily fish, as well as fish oil supplements - play a major role in the arachidonic acid cascade. The researchers designed a study to see if omega-3 fatty acid supplementation affects capsule formation around implants in animals. Tiny silicone implants were surgically placed under the skin of anaesthetized mice. One group of animals was regularly fed an omega-3 fatty acid supplement, while a control group received water.

After several weeks, capsule formation around the implants was compared between groups. The results showed reduced capsule formation in the group receiving omega-3 supplementation - the implants were thinner and more transparent than in the comparison group.

In addition, capsules from the omega-3-supplemented animals showed reduced expression of transforming growth factor (TGF) beta-2: a cell-activating protein (cytokine) that promotes inflammation. The reduction in TGF beta-2 expression was associated with decreased collagen deposits in the implant capsule.

Could fish oil help to prevent capsular contracture in women undergoing breast surgery using implants? It's too early to say - for one thing, for humans to take a level of omega-3 supplementation similar to that given to rats in the study "would be quite a dose," the researchers write. "

Moreover, we still don't know if this treatment would provide long-term benefit," Dr. Tamburino and coauthors add. "Further clinical studies are warranted to examine their therapeutic applicability and additional studies should be conducted to support our findings concerning the decrease in capsular contracture occurrence."

Anxiety may be early indicator of Alzheimer’s disease

Heightened anxiety in older adults may be an early indicator of Alzheimer’s disease, a study has warned.

Alzheimer’s disease is a neurodegenerative condition that causes the decline of cognitive function and the inability to carry out daily life activities.

Past studies have suggested depression and other neuropsychiatric symptoms may be predictors of AD’s progression during its “preclinical” phase, during which time brain deposits of fibrillar amyloid and pathological tau accumulate in a patient’s brain.

This phase can occur more than a decade before a patient’s onset of mild cognitive impairment.

Researchers examined the association of brain amyloid beta and longitudinal measures of depression and depressive symptoms in cognitively normal, older adults.

The findings, suggest that higher levels of amyloid beta may be associated with increasing symptoms of anxiety in these individuals.

These results support the theory that neuropsychiatric symptoms could be an early indicator of AD.

“Rather than just looking at depression as a total score, we looked at specific symptoms such as anxiety. When compared to other symptoms of depression such as sadness or loss of interest, anxiety symptoms increased over time in those with higher amyloid beta levels in the brain,” said a Dr.

“This suggests that anxiety symptoms could be a manifestation of Alzheimer’s disease prior to the onset of cognitive impairment,” said the Dr.

“If further research substantiates anxiety as an early indicator, it would be important for not only identifying people early on with the disease, but also, treating it and potentially slowing or preventing the disease process early on,” she said.

As anxiety is common in older people, rising anxiety symptoms may prove to be most useful as a risk marker in older adults with other genetic, biological or clinical indicators of high Alzheimer’s disease risk.

The researchers derived data from an observational study of older adult volunteers aimed at defining neurobiological and clinical changes in early Alzheimer’s disease.

The participants included 270 community dwelling, cognitively normal men and women, between 62 and 90 years old, with no active psychiatric disorders.

Individuals also underwent baseline imaging scans commonly used in studies of Alzheimer’s disease, and annual assessments with the 30-item Geriatric Depression Scale (GDS), an assessment used to detect depression in older adults.

The researchers calculated total GDS scores as well as scores for three clusters symptoms of depression: apathy- anhedonia, dysphoria, and anxiety. These scores were looked at over a span of five years.

Researchers found that higher brain amyloid beta burden was associated with increasing anxiety symptoms over time in cognitively normal older adults.

The results suggest that worsening anxious-depressive symptoms may be an early predictor of elevated amyloid beta levels - and, in turn AD - and provide support for the hypothesis that emerging neuropsychiatric symptoms represent an early manifestation of pre-clinical Alzheimer’s disease.

One of the hypothesized causes of Alzheimer's is the over-accumulation of the protein amyloid beta (Abeta) in different areas of the brain. This results in the deposition of aggregated protein plaques, which are toxic to neurons. One of the most promising ways to fight the plaques is to "tag" the Abeta proteins with antibodies that signal the patient's own immune system to attack and clear them.
To be most effective, this treatment has to be given as early as possible, before the first signs of cognitive decline. But this requires repeated vaccine injections, which can cause side effects. Scientists at Acole Polytechnique Federale de Lausanne in Switzerland have now solved the problem with an implant that can deliver a steady and safe flow of antibodies to the patient's brain to clear Abeta proteins.

The implant, which works by activating the body's immune system and turning it against the disease, is designed to sit underneath a person's skin, and over time, releases a steady flow of antibodies into the bloodstream.

Those antibodies then cross into the brain, where they target the beta amyloid plaques that are a hallmark of Alzheimer's disease.

Scientists tested the device on genetically engineered mice and discovered that the capsule prevented the plaques from forming.

The discovery could transform the way doctors approach Alzheimer's treatment and prevention, experts hope.

The capsule itself is based on a design from Aebischer's lab published in 2014. It is referred to as a "macroencapsulation device" and it is made of two permeable membranes sealed together with a polypropylene frame. The completed device is 27-mm long, 12-mm wide and 1.2-mm thick, and contains a hydrogel that facilitates cell growth. All the materials used are biocompatible, and the lab specifically used a method that is easily reproducible for large-scale manufacturing.

The study is published in the journal Brain.

Cure kidney stones with lemonade.

Kidney stones have become a more common health complaint than heart disease, stroke, and diabetes, according to figures released this year . The number of people suffering from them has doubled in the past 13 years. If you fall into that crowd, start downing lemonade. Lemon juice has the highest levels of citrate of any citrus juice, and that citrate helps dissolve any calcium deposits that will eventually turn into kidney stones. Squeeze your own fresh lemons to make lemonade, or buy a commercial mix. Doctors say that you can get as much citrate as you need from regular old lemonade, without having to make your teeth curl by sucking on a raw lemon.

Embolic stroke

This type of stroke may occur when a blood clot or a piece of atherosclerotic plaque

(cholesterol and calcium deposits on the wall of the inside of the heart or artery) breaks

loose, travels through the bloodstream, and lodges in an artery in the brain. When blood flow

stops, brain cells do not receive the oxygen and glucose they require to function and a stroke

occurs. This type of stroke is referred to as an embolic stroke. For example, a blood clot

might originally form in the heart chamber as a result of an irregular heart rhythm, like atrial

fibrillation. Usually, these clots remain attached to the inner lining of the heart, but

occasionally they can break off, travel through the bloodstream (embolize), form a plug in a

brain artery, and cause a stroke. An embolism can also originate in a large artery (for

example, the carotid artery, a major artery in the neck that supplies blood to the brain) and

then travel downstream to clog a small artery within the brain.
