HIV may disrupt neurodevelopment, affecting how children learn, reason and function

A new study has given a clear understanding about how even with early treatment; HIV still manages to attack young brains. Across sub-Saharan Africa today, a vast majority of children suffer from HIV. While early antiretroviral treatment, or ART, ensured that children with and exposed to HIV have less deadly results, studies have shown that the virus can still affect the brain.

HIV may disrupt neurodevelopment, affecting how children learn, reason and function. This is why Michael Boivin, Professor and Head of Psychiatry Research at the College of Osteopathic Medicine, State University of Michigan, gave a two-year longitudinal study published in Clinical Infectious Diseases to obtain a clear picture of how HIV influences children's neuropsychological growth. The study was supported by the National Institute of Allergy and Infectious Diseases, part of the National Institutes of Health.


The neuropsychological performance in three groups of children 5 to 11 years old has been examined by Boivin and his colleagues: individuals who acquired HIV perinatally and were treated with ART, those exposed but HIV-negative, and those who were never exposed.

The study was carried out at six testing sites in four Sub-Saharan African countries to look robustly at how HIV affects children in this field. To date, it's the first well-validated, multi-site neuropsychological evaluation of African school-aged children affected by HIV.

The researchers found that there are still major neuropsychological challenges for children living with HIV even in the face of early treatment and good clinical care. Boivin said: "These children came into the study with a deficit compared to their counterparts. It stayed about the same throughout the two years, except in one important area: reasoning and planning. On that specific test domain, the children living with HIV failed to progress over time."

In other words, in the field of planning and thinking over the course of the study, the difference between infected and HIV negative children increased. Such skills typically develop in healthy children in the school years.

"This is the most important cognitive function for the future of children living with HIV in terms of their likelihood of taking their medications, making good decisions, abstaining from risky behaviours like early sexual activity, psychosocial issues and school-related achievement," Boivin said.

However, early medical treatment started as early as 6 months of age, is probably not enough to address the neurocognitive deficits associated with HIV, even though it helps keep children alive and healthier than they would be without treatment.

In these children, treatment should be started even earlier to improve long-term neurocognitive outcomes. Boivin and his colleagues are already working on long-term care and support. Earlier this year, Boivin received a 5-year, $3.2 million NIH grant to continue his work with children affected by HIV in Uganda and Malawi.

Through this grant, researchers will investigate how MSU-developed computer cognitive games can serve as tools for neurocognitive evaluation, enrichment and potentially rehabilitation. Boivin hopes that the findings of the two experiments will contribute to making this neuropsychological evaluation paradigm part of the cost-benefit of caring for HIV-affected children.

"Often it's overlooked or seen as an afterthought, but unlike other areas of medical follow up, neuropsychological evaluation really gets at how well the kids are going to adapt and function in school, at home, in the community and in society in general," Boivin said. "It's really what links us most directly to the human burden of disease."

Any Amount of Alcohol During Pregnancy Can Affect Baby

A new review has revealed that consuming alcohol at any point during pregnancy can lead to poorer brain function in newborns.

The study, ‘Evidence of detrimental effects of prenatal alcohol exposure on offspring birth weight and neurodevelopment from a systematic review of quasi experimental studies’ was published in the International Journal of Epidemiology.

Researchers at the University of Bristol compared 23 published studies on drinking during pregnancy and analyzed them in the review.

It was found that drinking alcohol in any trimester can affect the baby, in many cases leading to lower birth weight and poorer cognitive functioning.

According to a report by Daily Mail, the group tried to compare similar groups of people, who were only different in terms of exposure to alcohol during pregnancy. In earlier studies, research was conducted through ‘observational’ studies only, where participants were already exposed to a risk factor and researchers did not alter this.

Hence, this was the first time that results from different study designs were compared in relation to the effects of drinking alcohol during pregnancy.

Speaking to Daily Mail, study lead Dr Luisa Zuccolo, from the University of Bristol, said that their work confirmed the current scientific consensus which says “consuming alcohol during pregnancy can affect one's child's cognitive abilities later in life, including their education”.

Vehicular emission weakens bones, speeds up ageing

Living in polluted cities is harmful for health. Earlier several studies have shown air pollution causes a range of medical issues such as lung cancer, stroke, heart diseases, asthma, dementia, depression et al. Now, a new study has warned that inhalation of toxic airborne particles emitted by diesel and petrol vehicles can weaken bones and speed up the ageing process.

Researchers came to this disturbing conclusion after studying the health of nearly 4,000 residents from 28 villages just outside the city of Hyderabad, in southern India.

The researchers, whose study is published in the JAMA Network Open journal observed that those who inhaled more toxic airborne particles had less bone mass in their spines and hips. Household air pollution did not have a clear association with bone mass.

The bone mass loss due to the oxidative stress and inflammation was caused by the toxic particulate matters, said the study led by Otavio T Ranzani, from Barcelona Institute for Global Health, Barcelona, Spain. Osteoporosis is a health condition that weakens bones, making them fragile and more likely to break.

The researchers took measurements of PM2.5 and black carbon in the atmosphere in each village. Total 28 villages were studied. PM2.5 is the finest type of particulate matter, while black carbon is a larger toxin. Both come mainly from petrol and diesel vehicle exhausts. Analysis revealed average PM2.5 exposure was 33 micrograms per meter cubed (ug/m3) — far above the maximum 10ug/m3 levels recommended by the World Health Organisation (WHO), said the researchers Carles Milà from National Institute of Nutrition, Indian Council of Medical Research, Hyderabad and Bharati Kulkarni from Department of Non-communicable Disease Epidemiology, London School of Hygiene and Tropical Medicine, London.

The researchers had cross-referenced pollution levels with X-rays measuring bone mass in participant’s lower back and hip. Results showed that exposure to air pollution were associated with lower levels of bone mass. For every 3ug/m3 increase in fine particulate matter, there was a decrease of -0.57g of bone mass in the spine and -0.13g in the hip, said the study titled ‘Association of Ambient and Household Air Pollution With Bone Mineral Content Among Adults in Peri-urban South India’.

An increase of 1ug/m3 of carbon saw bone density shrink by -1.13g in the spine and -0.35g in the hip, said Ranzani adding that “this study contributes to the limited and inconclusive literature on air pollution and bone health.”

According to the WHO, air pollution causes an estimated seven million premature deaths a year worldwide — one third of deaths from stroke, lung cancer and heart disease.

The WHO also points out that every day around 93 per cent of the world’s children under the age of 15 years (1.8 billion children) breathe air that is so polluted it puts their health and development at serious risk. Tragically, many of them die: WHO estimates that in 2016, 600,000 children died from acute lower respiratory infections caused by polluted air.

A recent report by the global health agency notes that when pregnant women are exposed to polluted air, they are more likely to give birth prematurely, and have small, low birth-weight children. “Air pollution also impacts neuro-development and cognitive ability and can trigger asthma, and childhood cancer. Children who have been exposed to high levels of air pollution may be at greater risk for chronic diseases such as cardiovascular disease later in life,” said the report.

Consumption of choline during pregnancy may boost babies' metabolism

Researchers has found that consuming more foods with choline during pregnancy may boost metabolism and brain development of babies.

Although the role of choline in neurodevelopment has been studied before in rodents, the new research was conducted on pigs, which has more relevance to humans.

A doctoral student in the Neuroscience Program said,"In pigs from choline-deficient moms, their brains were about 10 percent smaller overall."

And 11 of the 19 regions were significantly smaller in choline-deficient brains, he said. The result was the same in grey and white matter concentration of the piglets.

Conversely, piglets whose mothers consumed sufficient choline during pregnancy had higher concentrations of grey and white matter in the brain's cortical regions.

Grey matter is primarily made up of the neurons themselves, while white matter comprises the material that connects neurons and bridges in different parts of the brain.

For the study, the team gave pregnant sows choline-deficient or choline-sufficient diets through the second half of their pregnancies.

After weaning, piglets were fed choline-deficient or choline-sufficient milk replacer for 30 days. Then the month-old piglets were scanned by magnetic resonance imaging (MRI).

The left and right cortex were found to be larger in the choline-sufficient pigs, as a result of the a greater density of grey matter in the brain.

As part of the study, some of the pigs from choline-deficient mothers were also given adequate amounts of choline after birth.

The results showed that it is not sufficient for the brain.

"Postnatal supplementation cannot correct for prenatal deficiency. It has to occur during development," explained the associate professor at the varsity.

"We know that the structural alteration is there, but it may not manifest in ways we can see until later in life. That's why it's important to think about this during gestation because the changes are occurring then," the Dr. noted.

Smell may shape human memories

Scents and smells can form the basis of some of the most significant memories humans form in their lives, a new study suggests.
The discovery may provide a means to detect early problems with memory formation and memory retrieval in the brain, researchers said.
An international team led by University of Queensland has boosted understanding of the smell-memory connection, showing that olfactory memory in honeybees regulates receptors in their antennae.
Bees' antennae function like a human nose, said Queensland Brain Institute scientists Dr Judith Reinhard and Associate Professor Charles Claudianos.
"French novelist Marcel Proust in his novel Remembrance of Things Past (also known as In Search of Lost Time) described how his childhood memories started to flow when he tasted a madeleine cake dipped in linden tea, as he had been given as a child," Reinhard said.
"Our team found that odour memories trigger recall of associated events, and that long-term odour memory formation in the brain regulates the sense of smell in the 'nose' via regulating the receptor molecules," Reinhard said.
"Preferences for different foods and beverages are linked to our sense of smell, and our research shows that long-term scent memories modify how odours are perceived," said Reinhard.
"In a nutshell: our smell experiences shape our preferences," she added.
"The study demonstrates for the first time that the ability to smell different things is experience-dependent and modulated by scent conditioning," said Reinhard.
Claudianos said the findings could help explain the wide variability of smell perception in humans and the neurological mechanism underlying the common phenomenon of "acquired taste", where repeated sensory experience with a flavour or aroma leads to perceptual changes.
"This knowledge will provide an enormous insight for understanding food and aroma perception," he said.
"The discovery may also provide a means to detect early problems with memory formation and memory retrieval in the brain," said Claudianos.
Individuals with neurodevelopmental disorders such as autism and schizophrenia or neurodegenerative disorders such as Alzheimer's and Parkinson's disease often have an altered sense of smell perception, researchers said.
The study was published in the Journal of Neuroscience.

‘Lack of awareness among parents a major hurdle in treating children with cerebral palsy’

The Indian Academy of Cerebral Palsy had declared October 3 as National Cerebral Palsy 
Day. A Dr. said that lack of awareness among parents was a major hurdle in treating 
children with cerebral palsy. Simple physiotherapy exercises and occupational therapy 
along with spasticity control measures would help these children to improve their walking 
abilities and integrate with the society.
Cerebral palsy was a neurodevelopmental disorder. A majority of children with this 
disorder would suffer from physical and mental disabilities.

Infants' Inattentiveness Might Signal Later Autism


 Attention problems might be seen in 6-month-old infants who 
are later diagnosed with autism according to a new study .
Researchers found that the infants who paid less attention to 
people's overtures and activities than infants who did not 
develop autism.

This information could be used to identify infants at greatest 
risk of developing autism and provide them with early 
treatment, the researchers said.

This work is highly consequential for identifying new treatment 
targets and early intervention strategies.

The study included 67 infants at high risk for developing autism 
and 50 low-risk infants. Their eye movements were tracked 
while they watched a video of a woman doing everyday things, 
such as making a sandwich, looking at toys, or speaking.
Compared to the other infants, those who were later diagnosed 
with autism spent less time watching the social activities 
depicted in the video. When they did pay attention, these 
infants spent less time watching the woman's face.

Autism is a neuro-developmental disorder characterized by 
impaired communication and social interactions. The 
researchers said this is the first study to show that early signs 
of autism are present in the first year of life.

The researchers are now trying to determine the specific 
causes behind this lack of attention to social activities in infants 
later diagnosed with autism.
