Internal bleeding post-heart attack might be sign of cancer

Internal bleeding during the first 6 months after being discharged from hospital for a heart attack treatment might indicate the development of cancer in the body, a new study has found.

The findings of this study were presented at ESC Congress 2019.

“Patients should seek medical advice if they experience bleeding after discharge for a heart attack,” said study author.
 
“Particularly if the bleeding is of gastrointestinal, pulmonary or genitourinary origin, without any obvious reason, and occurs in the first six months. If the cause is cancer, early detection can improve prognosis,” suggested the researcher.
 
Following discharge for acute coronary syndrome (heart attack or unstable angina), patients are typically treated with dual antiplatelet therapy for around one year.

This treatment inhibits the formation of blood clots but raises the risk of bleeding.

Researchers for the study retrospectively reviewed the hospital records of 3,644 acute coronary syndrome patients discharged with dual antiplatelet therapy from Alvaro Cunqueiro Hospital.

Patients were followed-up for a median of 56.2 months for bleeding events and cancer. The researchers analysed associations between bleeding and the absolute risk of a new cancer diagnosis.
Bleeding occurred in 1,215 patients (33 per cent) during follow-up and 227 patients (6 per cent) had a new diagnosis of cancer.

After adjustment for factors known to influence bleeding or cancer, post-discharge bleeding was associated with a threefold higher risk of a new cancer diagnosis.

The median time from bleeding to cancer was 4.6 months. The link with cancer increased as the severity of bleeding worsened.

Spontaneous bleeding with no apparent cause was linked with four times higher risk of cancer diagnosis while there was no relation with bleeding due to trauma such as injury or bladder catheterisation.

Regarding the location, blood in the faeces was associated with a nearly fourfold risk of cancer diagnosis, while coughing up blood or blood in the urine were linked with four and eight-times greater risks, respectively.

There was a relationship between bleeding and cancer regardless of whether patients were still on dual antiplatelet therapy or not.

The Dr. said, “Most of the bleeding episodes in the study were mild. The bleeding events more strongly related with a new cancer diagnosis were severe haemorrhages of unknown cause requiring surgery – for example, digestive bleeding needing endoscopic treatment.”

“We found a higher incidence of cancer in the first six months after discharge regardless of whether patients were taking dual antiplatelet therapy or not,” the doctor added.

New technique to measure blood clot developed

Scientists have developed a new technique that allows them to measure blood clotting as well as the formation of free radicals, that leads to the build-up of blood clots, which in turn cause heart disease, stroke and dementia. The technique, investigates a part of the blood clotting process which focuses on the ways in which platelets from blood samples clump together.

"This method may be useful for future studies looking into new anti-platelet treatments for diseases such as diabetes, where clotting is disturbed and increases the risk of heart attack or stroke," according to the study.

The researchers discovered that the enzyme NADPH Oxidase is critically important for the generation of free radicals, the stimulation of blood clotting and the promotion of blood vessel damage in patients.

They successfully used the technique in mice and human cells. Their aim is to better understand how blood cells function, which will help to develop new drugs against blood clotting diseases or to test the risk of clotting diseases in patients.

"We're really excited to discover this new technique and its potential to understand how blood vessel diseases develop. For the first time, we can now simultaneously measure blood clotting and the formation of free radicals," said lead author. 

"We know they play a key role in blood vessel damage caused by ageing, diabetes, obesity and chronic inflammation. We're currently using this technique in our efforts to develop a new treatment to protect the blood vessels in diseases such as heart diseases, stroke, obesity, and vascular dementia," he added.

Sperm Could Be a Bigger Factor in Miscarriages Than We've Been Led to Believe

We all know it takes two to tango, but even though males contribute half the genetics of a pregnancy, they are too often left out of the picture. This is especially true when it comes to studies on miscarriage and recurrent pregnancy loss (RPL).

A growing body of research is slowly revealing what an oversight that truly is. Because even though carrying a baby to term is commonly thought of as a female issue, that is almost certainly a misconception.

In reality, 60 percent of miscarriages are caused by genetic problem, and that suggests male sperm is also partly responsible.

Investigating the sperm quality of 50 males whose partners had experienced three or more consecutive miscarriages, a new study offers even more evidence that poor quality sperm may be an important risk factor for RPL.

Compared to 60 males whose partners had not experienced any miscarriages at all, the males involved in this study showed twice as much DNA damage in their sperm.

"Traditionally doctors have focused attention on women when looking for the causes of recurrent miscarriage. The men's health - and the health of their sperm, wasn't analysed," says lead author , an expert in reproductive endocrinology and andrology.

"However, this research adds to a growing body of evidence that suggests sperm health dictates the health of a pregnancy. For instance, previous research suggests sperm has an important role in the formation of the placenta, which is crucial for oxygen and nutrient supply to the fetus." 

Believe it or not, when it comes to RPL, this study is the first research to evaluate the hormonal and metabolic health of sperm. And while the sample size investigated may be small, the findings have given researchers several important clues to follow.

The authors are now proposing that sperm damage may be triggered by a reactive oxygen species (ROS), found in the fluid that bathes sperm.

These ROS molecules are typically beneficial, protecting sperm from bacteria and other infections. But if their concentration gets too high - say, after an infection - it can also cause significant damage.
In the sperm of males whose partners have suffered miscarriage, the authors found four times more of these molecules. And while they still aren't sure why this is happening, this is one of the most important hints to date as to what could be behind RPL.

As research in this area slowly begins to mature, there is increasing evidence that obesity and old age can lower sperm health. As a result, the team is looking at whether either of these factors can somehow trigger a proliferation of these damaging molecules.

It's not a bad hunch given that, on average, the male participants in this study whose partners had experienced miscarriages were slightly older and more overweight than the controls. If it's true, however, it also means we have been consistently overlooking a potentially major factor behind miscarriage.

Fewer than half of all miscarriages in the US have an identifiable cause , but while women with RPL are routinely screened for risk factors, current guidelines do not recommend the same for male partners.

"It has taken medicine a long time to realise sperm health has a role to play in miscarriage – and that the cause doesn't lie solely with women," says the author.

"Now we realise both partners contribute to recurrent miscarriage, we can hopefully get a clearer picture of the problem and start to look for ways of ensuring more pregnancies result in a healthy baby." 

This study has been published in  a medical journal.

Study reveals how gene mutations cause immune disease

According to a recent research, a group of scientists have found how a gene mutation affects T cell function to promote immune disorders.

The team then tested a treatment based on the discovery -- successfully fixing donated immune cells from a 16-year-old boy with an abnormally low level of white blood cells called lymphopenia.

The discovery centers on mutation of the gene Gimap5, which is important to the healthy formation and function of CD4+ T cells, one of the immune system's super soldiers against infection and disease.

Scientists noted that the protein associated with the Gimap5 gene (also Gimap5), is important because it regulates a protein that inactivates an enzyme called GSK3.

If GSK3 isn't inactivated it causes DNA damage in T cells that are expanding, causing the cells to not survive or function correctly.

In mice and human blood cells, the researchers tested drugs that inhibit GSK3, improving immune system function in mice and restoring normal T cell function in the human cells.

Study's lead author  said, "Our data suggest GSK3 inhibitors will improve T cell survival and function and may prevent or correct immune-related disorders in people with Gimap5 loss-of-function mutations. Therapeutically targeting this pathway may be relevant for treating people with Gimap5mutations linked to autoimmunity in Type 1 diabetes, systemic lupus erythematosus or asthma." 

The scientists associated with the current study said additional research is needed before the data have clinical relevance for patients.

Meanwhile, new experiments are underway to translate the findings into the clinic, the researcher said. The scientists are investigating if and how genetic variants in Gimap5 affect GSK3 regulation causing malfunctioning T cells in patients with immune disorders.

They also are exploring the therapeutic potential of GSK3 inhibitors in pre-clinical mouse models of allergic lung disease and lupus to see if they can improve patient outcomes.

Butter, yogurt increases risk of colon cancer

Beware! Eating a high-cholesterol diet like butter, ghee, sausages, full fat cream and yogurt may increase risk of colon cancer, warns a recent study.

In a study of mice, scientists  discovered that boosting the animals' cholesterol levels spurred intestinal stem cells to divide more quickly, enabling tumours to form 100 times faster. The study has identified a molecular pathway that could serve as a new drug target for colon cancer treatment.

Study author said, "We were excited to find that cholesterol influences the growth of stem cells in the intestines, which in turn accelerates the rate of tumor formation by more than 100-fold".

"While the connection between dietary cholesterol and colon cancer is well established, no one has previously explained the mechanism behind it", he added. The team increased cholesterol in the intestinal stem cells in some of the mice by introducing more of the substance into their diets.

While in others, they altered a gene that regulates phospholipids, the primary type of fat in cell membranes, which spurred the cells into producing more cholesterol on their own. The stem cells' ability to multiply increased in both groups.

The results indicated that as the animals' cholesterol levels rose, their cells divided more rapidly, causing the tissue lining their guts to expand and their intestines to lengthen. These changes significantly sped up the rate of tumor formation in their colons.
