How To Recognise A Seizure: The Signs Most People Don’t Know About

When we think of seizures, dramatic images often come to mind, someone collapsing, shaking violently, or losing consciousness. While these symptoms do occur, most people don’t realise that many seizures are far more subtle. These understated signs are easy to miss and are frequently mistaken for stress, tiredness, or simply “zoning out." Unfortunately, this leads to delayed diagnosis and treatment. Recognising the early warning signs can help patients receive timely medical care and prevent complications.

Dr Kunal Bahrani, Chairman and Group Director – Neurology, Yatharth Hospitals shares some lesser-known signs of seizures that everyone should be aware of:

1. Brief Staring Spells That Look Like Daydreaming

Not all seizures cause shaking. A type called absence seizures, more common in children typically lasts just a few seconds. They involve a sudden, brief lapse in awareness: a blank stare, stopping mid-sentence, or pausing an activity without noticing. The person becomes unresponsive for 5 to 15 seconds and resumes normal behaviour immediately afterward, with no memory of what happened. These episodes are often mistaken for poor attention or daydreaming.

2. Sudden, Repetitive Movements You Don’t Notice

Some seizures trigger small, involuntary actions such as lip-smacking, repeated swallowing, finger rubbing, tugging at clothes, or chewing motions. These are often seen in partial seizures, where abnormal electrical activity occurs in only one part of the brain.

3. Unexplained Negative Emotions

A sudden surge of intense fear, anxiety, or a sense of impending doom without any reason may be seizure-related. Some people describe a rising sensation in the stomach, similar to the drop on a roller coaster. These symptoms are often confused with panic or psychological issues but can actually be linked to seizure activity.

4. Strange Sensory Experiences

Seizures can begin with an aura, unusual sensory experiences such as buzzing or humming sounds, strange smells like burning rubber, flashing lights, blurry vision, coloured spots, or tingling/numbness on one side of the body or face.

5. Sudden Confusion or Loss of Awareness

A person may seem awake but behave in a confused or disoriented manner. They may stop an activity abruptly, fail to respond when spoken to, walk aimlessly, or speak incoherently. They often have no memory of the episode afterward. This confusion may last from a few minutes to several hours.

6. Unexplained Injuries or Tongue Bites

Seizures can occur during sleep without the person knowing. Waking up with a bitten tongue (often on the side), severe headache, extreme fatigue, or bruises with no recollection of injury are classic signs of nocturnal seizures.

7. Sudden Jerks or “Electric Shock" Sensations

Sudden, shock-like jerks of the arms or upper body especially shortly after waking may indicate a type of seizure activity. These can be easy to dismiss as muscle spasms.

Seizures aren’t always dramatic. The quiet, easily overlooked ones may be the most dangerous because they delay diagnosis and treatment. If these symptoms occur repeatedly, it is important to consult a neurologist. Diagnosis typically involves an EEG and brain imaging. The good news: most seizures can be effectively managed with medication and lifestyle changes once identified early.

New Nasal Spray May Treat Rapid Heartbeat Condition

A nasal spray could soon provide quick relief for people experiencing sudden, rapid heartbeats. This innovative treatment is being developed by researchers at Weill Cornell Medicine, New York-Presbyterian Hospital, and may eliminate the need for patients to seek urgent medical treatment at the start of their symptoms. 
 
People with Paroxysmal supraventricular tachycardia (PSVT) experience sudden, rapid heart rate spikes due to extra electrical pathways connecting the upper and lower heart chambers. In PSVT, the heart's lower chambers briefly beat faster than 100 times per minute. 
 
Though generally not life-threatening, this condition can significantly impact one's quality of life, manifesting itself in symptoms like fainting, light-headedness, shortness of breath, dizziness, and heart palpitations. It's important to note that this condition can emerge at any stage of life and may be connected to diverse underlying factors, such as anxiety, cardiomyopathy, and pneumonia. 
 
As per the National Library of Medicine, this condition impacts a significant portion of the United States population, and is expected to affect nearly 7 million people by the year 2050.

Developing a safe and user-friendly first-line treatment for PSVT has been a longstanding objective in medical research. Presently, treatment options typically revolve around addressing the underlying causes of PSVT. This may involve prescribing beta blockers for cases linked to hyperthyroidism or utilizing surgical or catheter procedures to correct abnormal heart tissue.

In milder cases, doctors may employ vagal maneuvers, a set of techniques that includes applying abdominal pressure, executing "bearing down" motions, and gently manipulating the carotid artery to soothe the vagus nerve. 
 
New Research Shows Promising Results

In the latest research, published in the Journal of the American Heart Association, the research team built upon a prior randomized, placebo-controlled clinical trial involving etripamil. Etripamil, administered in nasal spray form, functions as a calcium channel blocker, similar to intravenous medications administered in hospitals. This medicine prevents calcium from entering heart and artery cells, causing relaxation and dilatation and, as a result, slowing the heart rate.

The nasal spray, which is suitable for on-the-go use and can be stored at room temperature, can be self-administered by individuals when necessary. Each cartridge has two doses and it is advised to administer one shot into each nostril. If the first dose fails to lower the heart rate, the second dose can be administered, as per the researchers. 
 
A study published in the journal Circulation: Arrhythmia and Electrophysiology last year concluded that self-administered etripamil is a reliable and efficient treatment option for PSVT episodes. Approximately 55 percent of patients successfully reverted to their normal heart rhythms within 30 minutes of taking etripamil, while only 35 percent experienced the same improvement with a placebo.

The most recent research examined how individuals would fare when using the drug in a practical setting, where they used it as needed, obtained refills, and reused it when their subsequent episodes occurred. Among the 169 participants who took part, 105 independently administered at least a single dose of etripamil over the study's duration, accounting for a combined total of 188 occurrences. Forty people were treated in two separate episodes. 
 
Among these, etripamil successfully reinstated regular heart rhythms within 30 minutes in approximately 60% of cases and within an hour in roughly 75% of cases. In the group that administered it twice, 63% regained normal heart rhythms within 30 minutes. However, approximately 25% of these individuals did not respond to the medication during either episode.


Future of Rapid Heartbeat Treatment 
While this study offers promise, it's important to recognize that more work, including subsequent research and eventual approval from the FDA, is necessary before this drug can be considered suitable for public use. 
 
Currently, researchers are evaluating the nasal spray's effectiveness in treating children's heart ailments as well as other heart conditions, such as atrial fibrillation, which occurs when the upper chambers of the heart beat rapidly and irregularly.

What is tachycardia, and when a high heart rate is dangerous

  Tachycardia is when your heart rate is faster than normal. 

    Sinus tachycardia is a normal increase in heart rate, like when you're exercising, and it is not generally considered dangerous. 


    Other types of tachycardia, like atrial fibrillation, cause a prolonged or recurring high resting heart rate — these types are considered dangerous and doctors will recommend treatment.
    

Tachycardia is a type of arrhythmia when your heart rate is faster than normal. A healthy resting heart rate for most adults is between 60 and 90 beats per minute (bpm), but adults with tachycardia usually have a resting heart rate greater than 100 bpm.

Here's what you need to know about what causes the different types of tachycardia and when the condition is considered dangerous. 


What is tachycardia

There are multiple types of tachycardia, which are categorized by where they originate in the heart. Some are not considered a danger to your health while others should be monitored closely. These include:

    Sinus tachycardia
is a normal increase in heart rate, which can happen when you're exercising, scared, or anxious. It originates from the heart's sinoatrial node, commonly known as the heart's natural pacemaker. Sinus tachycardia is not usually considered dangerous because your heart is still beating properly.


    Supraventricular tachycardia occurs in the upper chambers of the heart. It's the most common type of tachycardia in children and also more common in women than in men. Some people experience no symptoms, but in extreme cases, you may lose consciousness and, very rarely, have cardiac arrest.


    Atrial fibrillation is the most common type of tachycardia. It happens when chaotic electrical impulses in the upper chambers of the heart (the atria) lead to rapid, uncoordinated contractions. Researchers estimate that 2.7 million to 6.1 million US adults have AFib. It's associated with 1 in every 7 strokes, tends to cause more severe disability from stroke, and contributes to more than 150,000 deaths annually.


    Ventricular tachycardia occurs in the lower chambers of the heart. It is dangerous: the National Library of Medicine classifies it as a "major cause of sudden cardiac death." Some 300,000 deaths a year in the US are attributed to this type of tachycardia.


    Ventricular fibrillation is a similar process in the lower chambers, where the ventricles quiver instead of pumping necessary blood to the body. The most serious form of tachycardia, v-fib is a medical emergency and leads to sudden cardiac arrest.

Who's at risk of tachycardia

Many types of tachycardia can lead to emergency medical events, like sudden cardiac arrest, a heart attack, heart failure, and stroke. But according to Solomon Sager, MD, a cardiologist and electrophysiologist at the Chicago Cardiology Institute, you might not even know you have it.

While those with tachycardia won't always notice, there are a few common symptoms. The first indicator is a heartbeat that feels too fast, which you can check by taking your pulse. Other symptoms may include heart palpitations, chest pain, shortness of breath, or lightheadedness.

You're more likely to develop some form of tachycardia as you age. That's because structural changes in the heart may cause these abnormal heart rhythms over time, Sager says.

Patients who have a family history of tachycardia or congenital conditions at birth are at a higher risk. Certain health conditions are also associated with an increased risk, such as heart disease, high blood pressure, diabetes, obesity, sleep apnea, and anxiety disorders.

With the help of wearable health trackers like the Apple Watch, more people are discovering they have tachycardia, even when they don't experience symptoms. Sager says you should take this kind of data seriously and follow up with your physician if anything is out of the ordinary.
How to treat tachycardia

Physicians treat tachycardia in a variety of ways, often involving critical lifestyle changes. Sager says people diagnosed with AFib are encouraged to exercise, eat a healthy diet, and treat underlying health issues, such as sleep apnea or diabetes. They should also cut down on alcohol and quit tobacco.

People with ventricular tachycardia more often require medication, surgery, or ablation. Doctors may prescribe antiarrhythmic medications to prevent arrhythmias from coming on, beta blockers to slow heart rate, and blood thinners to reduce the risk of blood clots.

Patients who still have tachycardia despite medication may undergo catheter ablations. During this two- to four-hour procedure, a cardiologist guides a flexible wire into the heart that burns affected heart tissue using radiofrequency energy. This stops abnormal electrical signals with a 90% success rate in some types of tachycardia.

Magnesium can cause sudden cardiac death

A new prospective study based on data from the Women's Health Initiative found a potential inverse association between dietary magnesium and fatal coronary heart disease in postmenopausal women.

The study, which also showed a trend between magnesium and sudden cardiac death in this population, is published in the Journal of Women's Health, a peer-reviewed publication from Mary Ann Liebert, Inc., publishers.

Charles Eaton, MD, Alpert Medical School of Brown University, and a large team of researchers from various institutions coauthored the article entitled "Association of Dietary Magnesium Intake with Fatal Coronary Heart Disease and Sudden Cardiac Death: Findings from the Women's Health Initiative."

The researchers examined magnesium intake at baseline for more than 153,000 postmenopausal women and identified the development of fatal coronary heart disease and sudden cardiac death over the subsequent 10.5 years of follow-up. The data revealed that higher magnesium intake was associated with a statistically significant risk reduction in fatal coronary heart disease and a reduction in the risk of sudden cardiac death.

Kornstein, MD, Editor-in-Chief of Journal of Women's Health and Executive Director of the Virginia Commonwealth University Institute for Women's Health, Richmond, VA, stated: "If the findings of this study are confirmed, future research should test whether high-risk women would benefit from magnesium supplementation to reduce their risk of fatal coronary heart disease."

'Broken Heart' Syndrome could be brain-based

A heartbreak after a sad event could literally "break your heart" -- and the blame may be on your brain, scientists believe.

"Broken Heart Syndrome', also known as Takotsubo syndrome (TTS), is a sudden temporary weakening of the heart muscles, accompanied by symptoms including chest pain, shortness of breath and sometimes heart attack.

TTS usually appears shortly after severe emotional stress like heartbreak after a breakup or death of a loved one, in addition to other types of extreme sadness, anger, or fear.

A recent study published  sought for more clues underpinning a possible heart-brain correlation. The researchers, which comprised of neuroscientists and cardiologists, compared MRI brain scans from 15 TTS patients with those of 39 healthy patients and looked at three areas of the brain -- amygdala, hippocampus, and cingulate gyrus.

These areas are involved in emotional processing, motivation and memory, collectively controlling our response to stress and regularly interact with each other. The amygdala and cingulate gyrus also handle certain unconscious bodily functions such as the heartbeat and breathing.

According to the study, TTS patients have exhibited a breakdown in communication between these regions of their brains controlling both emotional processing and automatic bodily processes.

"(This finding) strongly supports the idea that the brain is involved in the underlying mechanism of TTS," a cardiology professor and the study's lead researcher, said in a news release.

The study suggests that our emotions can take a much greater toll on our physical health than we realize, which is why it's important to pay attention to your emotional health and holistic well-being.
