Donating Plasma for COVID-19 Patients: Everything You Need to Know

For those of us who aren't on the frontlines of the coronavirus pandemic as essential workers, the best way to help out has been pretty straightforward: practice social distancing by staying home. But now, people who have already had COVID-19 may be able to start helping out in a way many others can't: donating their blood plasma. 

As part of the ongoing fight against the disease—which has infected more than 750,000 people in the US, according to the most recent data by Johns Hopkins University's Coronavirus Resource Center—the American Red Cross has partnered with the Food and Drug Administration (FDA) to acquire convalescent plasma donations from those who have contracted and recovered from COVID-19.

Here's everything you need to know about plasma donation—including why it's so helpful, and how you can help out.

What is blood plasma, and why could it benefit COVID-19 patients?

According to the Red Cross, convalescent plasma is the liquid part of blood that carries cells and proteins throughout the body—about 55% of your blood is made up of plasma; the other 45% is red blood cells, white blood cells, and platelets. Overall, blood plasma has four essential functions: It helps maintain blood pressure and volume, aids with blood clotting and immunity, supplies muscles with electrolytes like sodium and potassium, and helps maintain your body's pH balance to help cells function properly.

Typically, blood plasma is given to trauma, burn, and shock patients, along with those who suffer from severe liver disease, immune deficiencies, or bleeding disorders. According to nonprofit The Blood Center, the primary supplier of plasma derivatives to local hospitals throughout South Louisiana and parts of the Mississippi Gulf Coast, children and adults with leukemia and other types of cancer, and people undergoing bone marrow transplants may also require plasma transfusions.

It's blood plasma's role in immunity that may benefit coronavirus patients—particularly because a vaccine is not yet available. "When a patient becomes infected with COVID-19, the body starts to build its own immune response by creating antibodies to fight the virus, which can take anywhere from seven to 14 days,” Kimberly Sanford, MD, President-Elect of the American Society for Clinical Pathology and director of transfusion medicine at VCU Health, tells Health. “By receiving blood plasma from a survivor, patients acquire the protective antibodies needed to fight the virus, shortening the time it takes for a patient to develop them on their own.” Ultimately, blood plasma from previously infected COVID-19 patients may help current patients recover from the infection more quickly.

As with many new and experimental treatments for COVID-19, blood plasma has yet to be named as a proven, effective treatment for general use, but the FDA has begun encouraging recovered COVID-19 patients to donate plasma for the development of blood-related therapies. On April 3, the agency announced a "national effort to facilitate the development of, and access to, two investigational therapies derived from human blood," one of which being convalescent plasma. (The other, hyperimmune globulin, is also an antibody-rich blood product that may benefit COVID-19 patients.)

The FDA points to "prior experience with respiratory viruses" along with data from China—specifically a study published on MedRxiv, a preprint server of yet-to-be peer-reviewed studies—that shows convalescent plasma has the potential to "lessen the severity or shorten the length of illness caused by COVID-19." Studies carried out during previous epidemics and pandemics, including Ebola, SARS, MERS and 2009 H1N1 (swine flu), also suggest convalescent plasma can be beneficial, hematologist and oncologist Timothy Byun, MD, who directs cancer research at St. Joseph Hospital in Orange County, CA, tells Health. 

In addition to facilitating a nationwide program to allow for the collection and distribution of blood plasma, along with the Mayo Clinic and the Red Cross, the FDA is also "facilitating the conduct of well-controlled clinical trials at academic institutions to rigorously evaluate the safety and efficacy of convalescent plasma," per the agency's news release.

How can you donate blood plasma?

Most important: In order to donate blood plasma specifically for COVID-19 patients, you must have been tested for COVID-19 and with a confirmed diagnosis. If you only experienced symptoms of COVID-19 but weren't tested, you’re not eligible at this point, although you can complete the American Red Cross donor contact form to be contacted when a process for testing unverified COVID-19 cases has been established.

While each hospital and organization can operate under their own guidelines, the current FDA guidelines require a plasma donor to have received a positive COVID-19 test, followed by a negative test, and to have been symptom-free for at least 14 days; or to have received a positive COVID-19 test followed by a symptom-free period of at least 28 days. 

Past that, the Red Cross says that plasma donors must also meet regular blood donation eligibility requirements. That includes being at least 17 years old, weighing a minimum of 110 pounds, and being in generally good health and feeling well, even if you have a chronic condition.

Before donating plasma, the American Red Cross recommends drinking an extra 16 ounces of water or other nonalcoholic drink and eating a healthy meal (avoid fatty/junk foods) to help prevent dehydration, lightheadedness, dizziness and fatigue. 

The act of donating plasma is also a bit different than a general blood donation, according to Dr. Byun, who was part of the team responsible for the first plasma transfer in the Western US. During a plasma donation at an official collection site, blood is drawn from the arm and passed through a machine that collects the plasma, then returns the red cells and platelets back to the donor, along with some saline to replace the withdrawn plasma. The procedure is perfectly safe, takes about an hour or so, and the most common side effects are mild dehydration and fatigue. Some donors may experience bruising at the injection site. 

To donate plasma safely, make sure you visit an accredited center, where you should go through a screening process that includes an initial blood test, a questionnaire and a physical exam. If you complete the donor contact form on the American Red Cross website, you’ll be contacted by a representative who will confirm eligibility and advise you of the next steps. If you’ve survived COVID-19, donating plasma is something that could truly help benefit those who are currently suffering from the disease.“We are heavily relying on those who have survived the virus to provide the immunity that we can’t currently create with a vaccine," says Dr. Sanford.

Only 43 people in the world have this rarest of the rare blood group!

Globally, A, B, AB, and O are the most commonly found blood groups. However, there exists a very special blood group, so rare that only 43 people around the world have it. Wondering what it could be? 

'Rh-Null' blood type is the rarest blood type observed in people. Also called 'golden blood', the blood type discovered in 1961, is extremely valued since there are only 8 available donors who can help with the transfusion. 


What gives it the rare stature?
The blood running through our veins is made up of red blood cells (RBCs), plasma and antigens, which number around 342. The same antigens work to determine your blood type. While 160 out of the 342 antibodies are very commonly found, people who miss out on these antigens tend to have a rare blood type. Blood is considered Rh-null if it lacks all of the 61 possible antigens in the Rh system. The Rh-Null phenotype reportedly happens in one in 6 million people.


Precisely, those who lack 'Rh' antigen in their bloodstream are thought to be the lucky ones carrying golden blood in their veins. Since it's still very rare, scientists are still studying factors and conducting several tests, considering that Rh-null blood type people can act as universal blood donors for people who have rare blood groups.

Just like us, people who carry Rh blood type are capable of leading normal, healthy lives without major danger to life. However, since the RBC count is a little low, they tend to be at the risk of borderline anemia.

Are there any risks associated with it?
Since the population of people with golden blood is fairly low, they are taken care of everywhere and practice extra precaution, considering that theirs is the most sought after blood group.

Rh-null might be the most esteemed blood type but it can be a little difficult to find a donor who can help them out in case of transfusion and medical care as their numbers are low. Transportation of blood across borders in such rare cases becomes more difficult because of bureaucracy. Hence, transfusion is not that easy. Some of the people carrying the blood type are even asked to insure their blood or donate it in a bank.

What women eat during pregnancy can modulate risk of ADHD in children

According to a new study, the risk of children  developing attention deficit hyperactivity disorder (ADHD) symptoms can be modulated by  mother’s diet during  while they are pregnant. The study,  had examined samples of umbilical cord plasma to calculate the levels of omega-6 and omega-3 which tend to reach the foetus. According to statistics, a higher omega-6: omega-3 ratio can be linked  to a higher risk of the ADHD symptoms at the age of 7.

Omega-6 and omega-3 can be termed as the  long-chain polyunsaturated fatty acids which have  a crucial role in the function and architecture of the central nervous system, mainly in the later stages of gestation.

These two fatty acids compete for incorporation into cell membranes and are primarily obtained through diet. Since omega-6 and omega-3 have opposing physiological functions, the former promotes systemic pro-inflammatory states, while the latter promotes anti-inflammatory states, a balanced intake of these two fatty acids is important. Previous research had shown that children with ADHD symptoms have a higher omega-6: omega-3 ratio.

The authors studied data from 600 children living in four Spanish regions (Asturias, Basque Country, Catalonia, and Valencia) who are participating in the INMA Project. They analysed umbilical cord plasma samples and data from questionnaires completed by the children`s mothers. The ADHD symptoms were assessed using two standard questionnaires: the first completed by the children`s teachers at the age of four years, and the second by their parents at the age of seven years.

The results showed that, at the age of seven years, the number of ADHD symptoms increased by 13 per cent per each unit increase in the omega-6: omega-3 ratio in umbilical cord plasma. The study analysed the number of symptoms in the children who met the diagnostic criteria for the ADHD (minimum six symptoms) and also in the children with a smaller number of ADHD symptoms. The ratio of the two fatty acids was associated with the number of the ADHD symptoms present but not with the diagnosis of the disorder, and only in the assessment carried out at seven years of age.

The authors suggest that the assessment carried out at four years of age may have been affected by a measurement error because the ADHD symptoms reported at early ages may be caused by a neurodevelopmental delay falling within the normal range.”Our findings are in line with previous studies that established a relationship between the omega-6: omega-3 ratio in mothers and various early neurodevelopmental outcomes,” commented a researcher and lead author of the study.

“Although the association was not clinically significant, our findings are important at the level of the population as a whole,” noted the author.”If a large proportion of the population is exposed to a high omega-6: omega-3 ratio, the distribution for the ADHD symptom scores would likely move to the right and the prevalence of extreme values would increase, leading to a negative impact on the community`s health costs and productivity,” he added.

“This study adds more evidence to the growing body of research on the importance of maternal diet during pregnancy,” commented the co-author of the study.”The nutrient supply during the earliest stages of life is essential in that it programs the structure and function of the organs, and this programming, in turn, has an impact on health at every stage of life. As the brain takes a long time to develop, it is particularly vulnerable to misprogramming. Alterations of this sort could, therefore, lead to neurodevelopmental disorders.”

Aerobic Exercise Can Prevent Chronic Kidney Disease in Diabetes

It is widely known that patients suffering from type 2 diabetes are at higher risks of obesity, metabolic syndrome, renal injury and kidney disease. Physical activity can reduce these risks by preventing complications, but the extent of this link is unknown.

A recent study, has found a strong association between lifestyle interventions like physical activity and prevention of chronic kidney diseases or impaired mineral metabolism.
Researchers used 40 male Zucker rats that were randomly allocated into four experimental groups, two of them (an obese and a lean one) performed an aerobic interval training protocol, and the other two groups were sedentary.

After eight weeks, urine, plasma and femur were collected from these rats and their kidneys were processed for histological studies.

As reported, the researchers found an improvement in blood vessel health and overall kidney function in obese rats, who had a hardening or scarring of the renal arteries, increased protein in the urine, and fat deposits within the filtering structures of the kidneys.

In a comparison between the obese rats in the exercise group and those in the sedentary group, the former showed a reduction in the above mentioned factors, indicating positive progress in kidney function in obesity, and changes in bone composition. They also had higher levels of calcium and copper, but lower concentrations of iron. 

Therefore, the researchers concluded that the specific training protocol used in this study was able to prevent the development of diabetic nephropathy, and affected the metabolism of certain minerals.

Ultrasound, blood test together may boost liver cancer detection

Earlier detection is important to improving survival of patients with liver cancer, and combining an ultrasound imaging with a blood test can help achieve that, say researchers.

Using ultrasound and a blood test for high alpha fetoprotein (AFP) levels together improves detection of early-stage liver cancer significantly, said the study published in a journal .


AFP is a plasma protein that is produced in abundance by the liver cells in the foetus. In adults, AFP levels are normally low, but liver cancer can cause AFP levels to rise.


"Liver cancer screening in patients with chronic liver disease has traditionally been performed using an abdominal ultrasound. While ultrasound is readily available and noninvasive, it misses many cancers when they are small," said an associate professor.


Risk factors for liver cancer -- also known as hepatocellular carcinoma (HCC) -- include hepatitis C infection, chronic heavy alcohol consumption, and nonalcoholic fatty liver disease related to diabetes and obesity.


Symptoms can include upper abdominal pain or swelling, loss of weight or appetite, white chalky stools, and general fatigue, the researchers said.


"Our study found that adding the blood biomarker alpha fetoprotein increased detection of early-stage hepatocellular carcinoma from 45 per cent with ultrasound alone to 63 per cent using the two tests in combination," a researcher. said. 


Liver cancer screening guidelines for patients with cirrhosis vary, with some guidelines calling for just imaging and other guidelines calling for both imaging and AFP measurement.


"Our results support a change in clinical practice and the routine use of ultrasound and biomarkers together for liver cancer screening," a researcher said. 


The results were based on a meta-analysis of 32 previous studies. Meta-analysis refers to a technique of combining the results of multiple scientific studies.
