Dare not ignore: Cardiologist reveals the weirdest signs your heart may be in danger long before chest pain strikes

As heart disease continues to be a leading cause of death worldwide, cardiologists are increasingly warning that the heart does not always signal trouble in predictable ways. A recent Instagram post by US based cardiologist Dr Dmitry Yaranov, known online as @heart_transplant_doc, has drawn attention to several unusual symptoms that may indicate serious cardiac distress long before classic chest pain appears.

When the Heart Sends Subtle Signals

In his post, Dr Yaranov cautioned that many dangerous heart conditions manifest through symptoms people often dismiss or misinterpret. “Your heart doesn’t always send obvious warnings,” he wrote, urging people to take unexplained physical changes seriously. The post, widely shared on social media, highlights how early recognition and prevention can save lives.

Jaw Pain That Is Not Dental

One of the most overlooked warning signs, according to Dr Yaranov, is jaw or tooth pain. Quoting his Instagram post, he explained that such pain can be referred discomfort originating from the heart, particularly in women. Because it is often mistaken for a dental problem, it may delay urgent care. In some cases, it can signal angina or even an impending heart attack.

Leg Swelling and the Burden on the Heart

Swelling in the legs or ankles is another symptom that frequently goes ignored. Dr Yaranov noted that fluid buildup in the lower extremities may point to heart failure, a condition in which the heart struggles to pump blood effectively. The swelling tends to worsen by night and reflects the heart’s reduced ability to manage circulation.

Breathing Trouble That Shows Up at Bedtime

Shortness of breath while lying flat, medically known as orthopnea, is a classic but often misunderstood indicator of congestive heart failure. Dr Yaranov warned that needing multiple pillows to sleep comfortably is not just a sleep issue but a red flag that the heart may be under strain.

Nausea, Cold Sweats and Silent Heart Attacks

Heart attacks do not always announce themselves with crushing chest pain. Dr Yaranov pointed out that nausea, cold sweats, dizziness or unexplained anxiety are frequently seen in so called silent heart attacks, especially among women. These symptoms are easy to confuse with digestive or stress related issues, which makes listening to one’s instincts critical.

An Early Vascular Clue

Perhaps the most surprising sign highlighted in the post is erectile dysfunction. Dr Yaranov explained that the arteries supplying blood to the penis are smaller than coronary arteries, meaning blockages can appear there first. Erectile dysfunction can therefore serve as an early warning of underlying vascular disease, often emerging years before more obvious heart symptoms.

Why Early Detection Matters

Dr Yaranov is an advanced heart failure and transplant cardiologist practicing in Memphis, Tennessee, with additional clinics across Mississippi and Arkansas. Board certified in advanced heart failure, cardiovascular disease and internal medicine, he frequently uses social media to translate clinical insights into public health awareness.

In his Instagram post, he emphasised that prevention and early action are crucial. If something feels off, he urged, it should not be ignored. His message reflects a broader shift in cardiology toward recognising subtle warning signs and intervening before heart disease becomes life threatening.

As Dr Yaranov’s post underscores, the heart often whispers before it screams. Recognising these unusual signals may be the difference between timely care and a missed chance to protect one’s life.

Avoid the kidney busters

Chronic kidney disease (CKD) is a “silent killer” which could be as fatal as diabetes and hypertension, but carries a much lower profile, with less awareness about it in society. It is, however, important to recognise the growing epidemic of CKD in young adults in our country. In a study in Chennai, 6 per cent prevalence of urinary protein leakage was noted in a sample population of young adults.
Young people often ignore warning signs about their health such as odd readings of high blood pressure, headaches, frothy urine, leg swelling and fatigue, etc.
The genesis of kidney disease can usually start early in youth and largely with the onset of modifiable risk factors.

Risk factors for CKD in young adults:
1. Low birth weight (28 per cent of children are < 2.5 kg at birth ).
2. Hypo-vitaminosis (condition of abnormally high storage levels of vitamins like A or D, which can lead to toxic symptoms) and under-nutrition of the pregnant mother may lead to smaller kidney volume at birth, hence lower eGFR (filtration efficiency).
3. Co-sanguinity and genetic inbreeding can lead to greater risk of congenital anomalies of the renal tract at birth.
4. Kidney filter diseases may result from the impact of poverty, poor sanitation, pollutants, water contamination, overcrowding, exposure to unknown nephrotoxins (heavy metals, plant toxins, indigenous remedies) and lack of access to affordable health care.
5. Growing burden of hypertension and diabetes in India.
6. Scarcity of nephrologists.
7. Autoimmune diseases such as systemic lupus erythematosus (SLE).
8. Consumption of non-steroidal anti-inflammatory pain medicines, use of steroids and excesses of whey protein / muscle gain products in an attempt to build muscle bulk and enhance body image. It can cause severe kidney filter damage, proteinuria and increased risk of stone formation.

Prevention of kidney disease in young age:
Early screening to identify risk factors, combined with education on ways to keep the kidneys healthy, could provide great opportunity to prevent or abate kidney disease before it becomes a problem. The youth must be made aware of:
1. Avoidance of extreme physical exercise.
2. Avoidance of extremes of heat to prevent heat stroke and dehydration.
3. Drinking good volume of water and fluids to prevent renal stone disease, especially in hot climates.
4. Avoidance of whey protein.
5. Avoidance of smoking & alcohol.
6. Avoidance of creatine tablets.
7. Reduction of weight in obese youth.
8. Monitoring BP and controlling it.
9. Avoidance of non-steroidal anti-inflammatory drugs.

Short education sessions/ videos ought to be shown to the millennial generation to create awareness about kidney disease and how to avoid it. With advancing technology and social networking, this could be spread “virally” not only among younger adults but also possibly later via secondary mechanisms (e.g., verbal) to older family members as well.

Warning Signs of a Blood Clot

Blood clotting is a normal process that the human body carries out in order to stop external bleeding. However, sometimes blood clots happen when they're not meant to, such as when it's still within your veins. When this occurs, blood clotting can become life-threatening. Internal clotting can be a cause of heart attacks, strokes, as well as other serious medical conditions. Fortunately, there are 8 common ways to notice if you're suffering from a blood clot:

1. Swelling in One Limb
One serious type of blood clot is deep vein thrombosis (DVT). When DVT occurs, oxygen is prevented from reaching your vital organs. This results in poor blood circulation, which ends up causing swelling. If one or more of your limbs is inexplicably swollen, it would be best to consult your doctor.
2. Leg Pain or Tenderness
People suffering from DVT also commonly experience leg pain or tenderness. In addition to making the limbs swell, DVT may also cause the legs to ache, be sore to the touch, or to cramp up.
3. Chest Pain and Heavy Breathing
Chest pain should never be ignored, and neither should heavy breathing. If both of these symptoms are experienced simultaneously, experts say that you could be suffering from a blood clot in the lungs. Such a clot can quickly turn fatal, so should you experience these symptoms, then stop whatever you're doing, and seek medical attention right away.
4. Shortness of Breath
Much like with heavy breathing, shortness of breath is never a symptom you should ignore. If you notice that you're finding it hard to breathe, have a racing heart, feel a fluttering sensation in your chest, or experience any dizziness, then you may be suffering from a clot in the lungs. These symptoms, particularly when experienced simultaneously, warrant immediate medical attention.

5. A Random Cough Without a Cold
If you aren't ill but are coughing for seemingly no reason, then look out for other potential signs of a blood clot. These include chest pain, shortness of breath, and a noticeably increased heart rate.
6. Red Streaks Along Your Veins
If you notice red streaks on your skin that run down much of the length of your veins, then you should contact your doctor immediately. While blood clots typically cause bruising, streaking is a sign of a more severe and potentially life-threatening concern. These streaks will also make your skin feel a lot warmer to the touch, which is another way of identifying a clot.
7. Severe, Chronic Headaches
If you are continually experiencing very painful headaches, whilst also having trouble speaking or seeing, then this may indicate a blood clot in your brain. Needless to say, this is extremely serious, and you should seek medical assistance immediately.
8. Vomiting or Diarrhea
Severe diarrhea and/or vomiting is never normal under any circumstances. However, if these symptoms keep persisting, then you should contact a medical professional, as they could indicate a blood clot in the abdominal region.

SOME SIGNS OF HEART DISEASE WHICH ONE NORMALLY IGNORES

1) Sleep Apnea- When snoring gets broken with pauses while sleeping, it means brain isn't getting enough oxygen. Signals are sent to heart and blood vessels to work harder to keep the blood flow going. This may raise,  your blood pressure, abnormal heart rhythm, stroke and heart failure. Luckily sleep apnea is treatable by wearing C-pap while sleeping.

2) Yellow-orange bumpy rashes- Very high triglycerides can make your skin break-up around your knuckles, toes and in your bottom. A lot of these fats in blood will lead to hardening of your arteries, which in turn leads to heart problems and strokes .

3) Poor grip- The strength of your grip tells about the strength of your heart. Research suggests that the ability to squeeze well means lower risk of heart disease. If you find it difficult to grasp objects, either you've heart problems or likely to develop one soon.Sadly improving the grip alone will not help to improve the heart condition.

4) Dark spots in mails- little dots of blood trapped in your nails, shows an infection in the lining of our heart or valves , known as endocarditis. Those with diabetes may also get this spots and are more prone to get heart problems and strokes.

5) Dizziness- is often the result of the heart not able to pump enough blood to the brain. Lightheadness could be a symptom of arrhythmia, abnormal heart rhythm.

6) Changes in skin colour- blue / gray fingers or toes could be due to poor circulation or blocked vessels. A lacy, mottled, purple pattern shows up when bits of built-up cholesterol plaques break off, then get stuck in small blood vessels. The bloody splotches just under the skin on the inside of your hands and the soles of your feet is sign of endocarditis.

7) Dark- velvety patches- these thick spots are normally found in skin folds of neck, armpits, groin, when the body isn't able to use insulin. The patches could have skin tags too. If you're not treated for type-2 diabetic, insulin resistance or metabolic syndrome, take help from your doctor to get treatment to control blood sugar and prevent heart diseases.

8) Breathlessness after work or hard to breathe while lying down, have chest pain, kindly call for emergency, to take care of your heart.

9) Swelling in your lower legs-this normally happens during pregnancy, but when one isn't pregnant and finds fluid collection when one stands or sits for long, it shows poor circulation, may be due to a clot also.Suppose the swelling comes suddenly, go for emergency check-up at once.

10) Fatigue- which can be due to various reasons. But when one has heart problems, it can leave one tired, as the heart isn't able to pump blood efficiently to all the parts of the body. It can also be due to other symptoms- anemia, cancer , depression, so best to rule out all the symptoms at the earliest.
