Neurologist explains difference between normal forgetfulness and dementia memory loss: ‘If it gets worse over time…’

Forgetfulness is common, especially as one gets older. But sometimes, what seems like simple memory lapses can hint at something even more serious. Memory loss is serious in dementia, substantially impacting day-to-day activities. Activities which were once second nature become difficult or appear confusing. The neurodegenerative disease, like dementia's forgetfulness, is persistent, which may worsen over time.

Dr Elanthirayan GC, consultant, neurology, SRM Global Hospitals, Chennai, shared in an interview with HT Lifestyle key insights on whether forgetfulness in old age is simply a natural part of ageing, or an early sign of dementia.

Elaborating on the nature of dementia, Dr Elanthirayan explained that it goes far beyond occasional forgetfulness. He said, “Dementia is not a compact condition; it is a vast word for decreasing memory power and thinking capacity that is bad enough to make everyday life difficult. The most common type of dementia is Alzheimer's disease.”

How is regular forgetfulness different from dementia?

Even regularly, there are many common memmory slips, like forgetting wallet at home or misplacing keys. But when this happens persistently and start to affect daily life, then they may hint at something more serious.

Dr Elanthirayan broke down the key differences between age-related forgetfulness and dementia.

He explained, “Forgetting names, things where you left them or having trouble in remembering words while speaking for a short period of time are all normal signs of becoming older. Mostly, these mistakes are not serious and can be cured. On the other hand, memory loss caused by dementia is long-term and disruptive. A person could behave the same way by asking the same questions repeatedly. Getting lost or not recognising places you already know. Having trouble with everyday activities like cooking or making payments, not remembering the time or place. Changes in personality or behaviour.”

What is the reason behind forgetting things?

Memory loss can also happen due to a wide range of other health conditions, not just as a sign of dementia.

The neurologist shared, “Dementia is a common cause of memory loss, but there are many other disorders, and most of them can be treated. Lack of vitamins (like B12), having problems with the thyroid, anxiety or depression, and drugs that can change how your brain works- difficulties to fall asleep, injuries in the head, and consumption of drugs and alcohol. Finding out the reason for memory loss through a medical exam is important because many factors that are not related to dementia can be treated and cured.”

Visiting a doctor on time when memory issues surface can help identify the cause early, allowing for timely treatment and better management of the condition.

10 Tell-Tale Signs That You Might Have Hypothyroidism

If you’re constantly sleepy and can’t lose weight, then it’s possible you have hypothyroidism, a condition that affects millions of people around the world. In the human body, the thyroid gland is a butterfly-shaped gland at the base of the neck that secretes two very important hormones – thyroxine and thyroid stimulating hormone. When hypothyroidism is present in an individual, levels of thyroxine are normal to low, whereas levels of thyroid stimulating hormone are elevated.
In turn, these hormone imbalances cause the pituitary gland to work harder than it normally should, as it tries to bring hormone levels back into balance. If you’ve gained quite a bit of weight recently, but you’re at a loss as to why, then the cause might be hypothyroidism. The condition causes metabolic function to slow down. Even if you happen to be dieting and exercising regularly, you will not be able to lose any extra pounds, because your body accumulates water and salt when your thyroid is sluggish. 
 
3. Forgetfulness 
Cognitive function takes a hit when your thyroid isn’t working properly. Hormone imbalances as a result of this malfunctioning, can cause forgetfulness and poor memory. Seeing as this hormone deficiency or imbalance slows everything down, your neurological function could be affected after some time. 
 
4. High Cholesterol Levels 
Although it isn’t typical for a doctor to look for thyroid problems during a check-up, they’re likely to measure cholesterol levels. If they are elevated, they could also point to a thyroid problem. This is because hypothyroidism can reduce clearance of cholesterol particles in the body. If you’re looking to lower these levels, a good food to try is overnight oats. 
 
5. Little Interest in Love Making 
Having little to no interest in love making can also point to a thyroid disorder. This is because of the relationship between thyroid function and adrenal glands, which are the organs that control the responsible hormones, like testosterone. In a nutshell, low levels of thyroid hormones can also result in low levels of needed hormones. 
 
6. Feeling Gloomy or Depressed 
One of the more debilitating side-effects of hypothyroidism is feeling down in the dumps or depressed. While depression can, of course, be related to other things, it can be a sign of hypothyroidism, especially when it is experienced in conjunction with one or more of the other symptoms that are mentioned in this list. 
 
 
7. Dry Skin 
If you’ve desperately been trying to add moisture to your skin seemingly to no avail, it could also be a sign of a thyroid problem. Changes in texture and appearance could be as a result of low thyroid function. You should especially be concerned if your dry skin is compounded with brittle nails, poor wound healing and hair loss. Make it a priority to see a doctor if you’re experiencing these things in combination.


8. Constipation

Being unable to go can be caused by an under-active thyroid. Seeing as the thyroid gland regulates metabolism, under activity can slow down digestive tract action. This can also weaken the contraction of the muscles that line the digestive tract. This causes your stools to move through your intestine far too slowly. Eating fiber-rich foods can help with constipation, but if symptoms persist, then it’s time to head to your doctor. 
 
9. Sore Muscles 
If you exercise regularly, then it’s normal to experience muscle fatigue, however if the fatigue feels constant, it could be linked to a thyroid problem. The hormones that regulate metabolism can affect how your muscles feel if they are at low levels. To counter these symptoms, do low-impact exercises like elliptical stepping or swimming. Also, consider increasing the amount of fish in your diet. 
 
10. Feeling Cold 
Wearing many different layers in the middle of summer obviously isn’t normal, and if you’re doing so, you might be suffering from hypothyroidism. The thyroid gland regulates body temperature, and its function is affected if it is producing too little thyroid hormone. Body temperature tends to decrease as a result of this, because less energy is being burned by downstream cell targets. Less energy together with a slow metabolism means less heat.

Not to forget


Diagnosed back in 1906 for the first time, Alzheimer's disease is one of the diseases affecting the elderly, marked by a tremendous psychological impact on not only the patient but on their families and the primary care givers to great extents. September being world Alzheimer's month dedicated to bringing awareness about Alzheimer's and other dementia conditions which more than 50 million people worldwide suffer from today, September 21 is globally observed as World Alzheimer's Day. 

Alzheimer's Disease International informs that a new dementia case is registered every 3.2 seconds totaling up to 9.9 million cases worldwide per year. What is commonly perceived as the disease of chronic forgetfulness, dementia is a range of symptoms that are associated with deteriorating memory or other thinking skills, the most severe of which can affect regular mundane functioning of the affected person. Dementia, however, is not a specific disease but an umbrella under which Alzheimer's disease accounts for over two-thirds of all diagnosed cases. There happens to be over a hundred different forms of dementia besides Alzheimer's. The average age limit for the diagnosis of dementia is just 65, and this is young given latest medical care facilities and resultant life expectancy. 

On the flipside, this increased life expectancy also means more people being prone to be diagnosed with dementia because of the rising overall numbers of elderly people in the world. India has an imposing demographic dividend and this also implies a massive strength of elderly in India in the foreseeable future. As per census estimates, India's elderly population increased from 70 million to 104 million between 2001 and 2011. In 2011, the population over 60 years of age comprised 8.6 per cent of the total population. As the population ages, the burden of geriatric diseases will get heavier and preparedness for that is necessary, especially for degenerative diseases like dementia. A lack of awareness compounded by a dearth of specialists in geriatric diseases paints an unfortunate picture of the ailing elderly. Also considering a right to life with dignity, awareness of the disease is crucial to preparedness to deal with it.

Menopause: How women can have a smooth transition

 Menopause is a transitional phase in a woman’s life. Starting with perimenopause, when a woman’s body slows down production of estrogen and progesterone with significant changes in the menstrual pattern, menopause technically begins when a woman has not had her period for 12 months. It is a part of the normal aging process that every woman has to undergo at some point in her life. 

A pan India survey conducted by IMS identified 46.2 years as the average age of natural menopause in India, which is about 5 years earlier than in the Western world. One estimate suggests that 130 million Indian women were expected to live beyond menopause by 2015. Based on current life expectancy and average age of menopause, Indian women are expected to live approximately 24 years beyond menopause.


Menopause triggers drastic changes in a woman's body such as hot flushes and sweating (65%), sleep disturbance (45%), vaginal dryness & atrophy (45%), painful intercourse (20%), urinary incontinence (30-40%), joint and back pain (45-60%), mood swings (60%), forgetfulness/poor concentration (40%), depression and anxiety (20-60%). These in turn lead to reduced Quality of Life (QoL). In some women, natural menopause occurs earlier. Early menopause extends the years lived with lowered quality of life, while also posing a relatively higher risk of heart disease. Lifestyle choices such as smoking and drinking, auto-immune diseases such as thyroid and exposure to radiation or chemotherapy can often cause early menopause.
In addition to causing physical distress, menopause can adversely impact all aspects of life. Not only does it have an impact on emotional well-being, but it is also observed that 45% of working women struggle at work due to reduced productivity, 33% of menopausal women feel that their social life takes a back seat and 51% feel they are unable to enjoy their conjugal life.



Women with early menopause are twice as likely to suffer from coronary heart disease and are also at higher risk of developing type 2 diabetes, than those with later onset of menopause. According to Dr. Meeta Singh, Consultant Gynecologist & Ex-IMS President, Tanvir Hospital, Hyderabad “Early menopausal women need comprehensive care for early diagnosis of cardiovascular diseases, diabetes and osteoporosis. And regardless of age of onset, about 81% of post-menopausal women have low bone mineral density (BMD) due to falling estrogen hormones leading to increased osteoporosis and fracture risk,.”

During menopausal phase, it is important for a woman to be candid with her doctor about symptoms and discuss the best treatment options to improve quality of life. Dr. Singh elaborates, “Though women are opening up in today’s India, they may still hesitate to discuss private topics and symptoms such as depression, vaginal dryness or difficulty having intercourse. Moreover, many women still do not prioritize their own health, often putting their distress on the backburner and suffering silently. Open and honest conversations with healthcare providers on menopause, its symptoms and the best treatment options are essential.” In consultation with their doctor, women can then choose dietary changes, exercise regimens and therapy that is best suited to their menopausal symptoms, medical history, and personal preferences, thus ensuring a smooth transition into the post-menopause phase.

Currently, treatment options include both non-hormonal therapy and menopause hormonal therapy (MHT). Though the former is more commonly prescribed throughout India as it has higher patient acceptance, science shows that MHT has multiple benefits, including alleviation of symptoms such as hot flashes, sleep disturbance, mood swings, and painful intercourse. MHT also helps reduce the risk of osteoporosis and CVDs, especially in women with early menopause. According to Dr. Singh, “Women can keep the symptoms of menopause at bay by supplementing their estrogen and progesterone levels. Menopause Hormone Therapy (MHT) started in consultation with a qualified doctor at the right time, with the right dosage and for the right duration can help menopausal women enjoy better quality of life and live healthy.”

Though menopause is a personal experience, a woman does not have to cope with it on her own. Healthcare professionals can help in management to enhance quality of life. The need of the hour is to educate both women and their families on the importance of holistic management of menopause, so they can stay healthy and feel good at any stage of life.
