Colorectal Cancer

Colorectal cancer is the cancer of the colon or rectum located at the lower end of the digestive tract. It is also known as bowel, colon or rectal cancer and is the third most common cancer globally, accounting for nearly 1.4 million new cases and 694,000 deaths per year. Although, colorectal cancers are not talked about widely, they are increasing at a rapid rate and early detection is the key to survival. While the prevalence of this disease was predominantly in the western countries earlier, it has been increasing exponentially in India. Colorectal cancer is one of the most preventable cancers and a healthy lifestyle alongside regular screening can safeguard against the disease significantly.

Causes and symptoms

The risk factors for colorectal cancer usually spikes up over an age, usually after the age of 50. Other prevalent causes are:
The risk for colorectal cancer increases post 50 years of age
Personal history of colorectal polyps (a small clump of cells) or colorectal cancer
Personal history of inflammatory bowel disease (ulcerative colitis or Crohn's disease)
Family history of colorectal cancers
Obesity
Lack of regular physical activity/ exercise

There are also certain risk factors for colorectal cancer, which include:
Change in bowel habits such as diarrhea, constipation, or narrowing of the stool that lasts over a few days
A feeling that you need to have a bowel movement that is not relieved by doing so
Dark stool or blood in the stool
Weakness and fatigue
Weight loss
Cramping or abdominal belly pain

Prevention and myths
According to Dr. Amanjeet Singh, Associate Director, GI Surgery, GI Oncology and Bariatric Surgery, Institute of Digestive and Hepatobiliary Sciences, certain factors can help treat and prevent the risk associated to Colorectal Cancer. These include:

-3 to 4 hours of vigorous physical activity per week can help decrease risk.
-Avoid high intake of red meats including lamb, liver and pork and processed meats such as hot dogs
-Avoid smoking and limit the consumption of alcohol
-Add more vegetables, fruits, and whole grains to your diet
-Get yourself screened regularly post the age of 45 years

The treatment
The treatment for colorectal cancer is determined basis the location and stage of the cancer and overall health of the patient. Colorectal cancer can be treated through surgery, radiotherapy or chemotherapy.

Early-stage colorectal cancer can be treated through:
Colonoscopy- if the cancer is small, localized and within a polyp, it can be removed completely during colonoscopy (test used to detect changes or abnormalities in the colonand rectum)

Endoscopic mucosal resection- this is used in case of larger polyps and involves a colonoscopy. However, a small part of the inner lining of the intestine is also removed along with the polyp using specialized tools during colonoscopy.

Laparoscopic surgery (Minimally Invasive surgery)- this is used in cases where the polyp cannot be removed through a colonoscopy. Laparoscopic surgery involves making small incisions in the abdominal wall for inserting instruments with an attached camera that displays the colon on a monitor for safe and effective removal of the polyp

Advanced colorectal cancer can be treated through:
Partial Colectomy- this procedure involves removal of the part of the colon that contains the cancer along with some normal tissues surrounding the cancer. It is generally conducted through a laparoscopic approach and the surgeon reconnects the healthy portions of the colon or rectum post-surgery.
Surgery for enabling waste to be removed from the body- this is undertaken when in rare cases it is difficult to reconnect the healthy portions of the colon or rectum. The process is called ostomy and involves creating an opening in the abdominal wall (from a portion of the remaining bowel) for eliminating stool into a bag that is fitted over the opening.

Radiotherapy- this involves the use of powerful X-rays and protons for killing cancer cells and is a good option in case surgery cannot be conducted. It is also used to shrink the cancer ahead of a surgery for easy removal

Chemotherapy- this involves the use of drugs for destroying cancer cells and is usually administered post-surgery if the cancer is large or has spread to the lymph nodes. Like radiotherapy, chemotherapy can also be used to shrink the cancer ahead of a surgery.

Targeted drug therapy- this treatment focuses on specific abnormalities within cancer cells and blocks them through targeted drug treatment, thereby killing the cancer cells
Immunotherapy- it is a drug-based treatment that utilizes the patient’s own immune system to fight cancer cells.

Early morning workouts best for weight loss

When trying to lose weight, the timing of exercise plays a key role, recent findings suggest.

In a study of 375 adults who have successfully maintained weight loss and who engage in moderate-to-vigorous intensity physical activity, most reported consistency in the time of day that they exercised, with early morning being the most common time.

The study also found that being consistent in the timing of physical activity was associated with higher physical activity levels, regardless of whether people exercised consistently during the morning, afternoon, or evening.

"Our findings warrant future experimental research to determine whether promoting consistency in the time of day that planned and structured physical activity is performed can help individuals achieve and sustain higher levels of physical activity," said senior author.

Exercise boosts quality of sleep in teens

Exercising for more than usual may lead to improved quality and duration of sleep at night, according to a study.

The study found that getting more exercise than normal- or being more sedentary than usual- for one day, may be enough to affect sleep later that night.

It found that for every extra  hour of moderate to vigorous physical activity, the teens fell asleep 18 minutes earlier, slept 10 minutes longer and had about 1% greater sleep maintenance efficiency that night.

Adolescence is a critical period to obtain adequate sleep , as sleep can affect cognitive and classroom performance, stress and eating behaviours, said a researcher.

The research suggests that encouraging adolescents to spend more time exercising during the day may help their sleep health later that night, he said.

In contrast, the researchers also found that being sedentary more during the day was associated with worse sleep health.

When participants were sedentary for minutes during the day, they fell asleep and woke up later but slept for a shorter amount of time overall.

You can think of these relationships between physical activity and sleep almost like a teeter totter, said a Prof.

When you're getting more steps, essentially your sleep begins earlier, expands in duration and is more efficient.Whereas if you're spending more time sedentary, it's like sitting on your sleep health- sleep length and quality goes down.

While previous research suggests that adolescents need 8-10 hours of sleep at night, recent estimates suggest that as many as 73% of adolescents are getting less than 8 hours of sleep.

Previous research also found that people who are generally more physically active tend to sleep longer and have better quality of sleep.

However, the researchers, said less has been known about whether day-to-day changes in physical activity and sedentary behaviour affected sleep length and quality.

They used data from 417 families from  across 20 states in U.S.

When participants were 15 years old, they wore accelerometers on their wrists and hips to measure sleep and physical activity for 1 week.

One of the strength of this study, was using devices to get precise measurements about sleep and activity instead of asking participants about their own behaviour, which can sometimes be skewed, he said.

The hip device measured activity during the day, and the wrist device measured what time the participants fell asleep and woke up and also how efficiency they slept, which means how often they're sleeping versus tossing and turning , the researcher said.

In addition to finding links between how physical activity affects sleep later that night, the researchers also found connections between sleep and activity the following day.

They found that when the participants slept longer and woke up later, they engaged in less moderate -to- vigorous physical activity and sedentary behaviour the next day.

Walking may help women conceive

If you have a history of pregnancy loss, do not lose hope as a new study suggests that physical activity such as walking may improve the chances of conceiving among women who are overweight or obese.
The study suggested that women reporting more than four hours a week of vigorous activity had significantly higher pregnancy chances compared to no vigorous activity.

"One of our main findings is that there was no overall relationship between most types of physical activity and the likelihood of becoming pregnant for women who had already had one or two pregnancy losses, except for walking...," said co-author,  "... which was associated with higher likelihood of becoming pregnant among women who were overweight or obese," he added.

For the study, researchers recruited 1,214 women between 18 to 40 years old with a history of one or two pregnancy losses.

The researchers found that the association of walking with the ability to become pregnant -- known as fecundability -- varied significantly by body mass index (BMI).

Among overweight or obese women, walking at least 10 minutes at a time was associated with improvement in fecundability, the researcher said.

Moderate activity, sitting and other activity categories were not associated with fecundability overall or in BMI-stratified analyses, they added.

The researchers noted that these findings provide positive evidence for the benefits of physical activity in women attempting pregnancy, especially for walking among those with higher BMI.

Flu risk 'cut by vigorous exercise'

Doing at least two and a half hours of vigorous exercise each week cuts the chance of developing flu, new data suggests.
Around 4,800 people took part in this year's online Flu Survey, run by the London School of Hygiene and Tropical Medicine.
Moderate exercise did not appear to have a protective effect, the researchers said.
Overall, flu rates have been relatively low this winter.
The Flu Survey, now in its fifth year, tries to track as much detail as possible about who does and does not get flu.
One of the questions people must answer when they register, as well as their age, if they are around children and if they have been vaccinated, is how many hours of "vigorous exercise" they do each week, such as running, fast cycling or competitive sports, ranging from none to more than five hours.
They are then asked to log in each week and note how they are feeling, and whether or not they have any flu-like symptoms.
There seem to have been fewer cases of flu this winter
'Dodged the flu bullet'
The researchers say their findings suggest 100 cases of flu per 1,000 people could be prevented just by engaging in vigorous exercise.
They also said this year's survey found some of the lowest reports of flu-like illness in recent years.
Over the winter flu season, only 4.7% of reports were positive for flu-like symptoms compared with 6% last year.
Children also appear to have had lower levels of flu-like illness than last year with just 5% reporting symptoms this flu season compared with 7.9% last year.
Dr Alma Adler, of the London School of Hygiene and Tropical Medicine, said: "We need to treat this result cautiously as these are preliminary findings. However, they are consistent with findings for other conditions and really show the health benefits of exercise.
"Although many people have dodged the flu bullet this winter, flu can occur at any time, so taking advantage of the better weather is a great opportunity to get out and get fit to ward off flu this spring."
Flu Survey provides data to the government, and feeds into Public Health England's national flu surveillance programmes and is being released at the start of National Science & Engineering Week.
