An In-Depth Guide on Treating Hidradenitis Suppurativa

Hidradenitis Suppurativa (HS) also more commonly known as Acne Inversa, is a chronic skin condition that can manifest itself in both men and women all across the world. HS presents itself in a variety of symptoms, the most common being the formation of painful lumps under the skin that greatly resemble boils, and these lumps may often secrete pus or blood. While HS is not curable, it is very much treatable and relatively rarely fatal if dealt with properly.

This disease is believed to be rare, but due to its resemblance to acne, as well as the varying nature of symptoms, this condition is often misdiagnosed, resulting in the reported number of persons suffering from this condition being far less than the actual number. A 2017 study showed that Acne Inversa is far more common than previously believed to be.

This condition is also often associated with severe anxiety, self-consciousness, depression and social isolation noted in the people suffering from it. For these reasons and many others, it’s important to be aware of HS, the various symptoms, and most importantly, the treatment options.

What is Hidradenitis Suppurativa?
As stated above, Hidradenitis Suppurativa is a skin condition that results in chronic pain and deep abscesses that are similar to boils on various parts of the body, particularly those high in sweat glands and hair follicles. Approximately 200,000 people have been diagnosed with this ailment, though it is believed that a whopping 1-4% of the population may be suffering from this condition, despite the condition remaining undiagnosed in many cases. 


Including milder cases of this ailment, as many as 1 in 100 people may possibly suffer from this disease. However, significantly more epidemiological data is needed to support the above-said claim. Regardless, this condition presents itself with numerous symptoms, and if you notice any of these symptoms, it is important to consult a doctor or dermatologist as soon as possible to seek treatment. 


The Symptoms of HS
Acne Inversa largely affects the sweat glands of the body, more specifically, areas of the body that tend to rub together, like under the breasts, armpits, buttocks, and groin. The condition may start with singular abscesses or pits in the skin, near hair follicles on the skin. These pits may also contain blackheads and thus may be hard to identify.

As the condition progresses, these bumps and lesions begin to secrete pus and blood, which may emit an unpleasant odor. One of the biggest difficulties faced by people suffering from HS is the slow pace at which these wounds heal, during the process of which they often enlarge and reopen, or flare up and secrete pus and blood. When they heal, they often result in scarring and tracts in the skin, also known as sinus tracts. 


There are only 3 known fatal cases for patients with HS, and in all said cases, the patients also suffered from squamous cell carcinoma - a type of cancer, and the combination of the two being the likely cause of death. Most cases of death relating to HS patients are due to suicide or substance abuse, often caused by the resulting depression and anxiety.  


How does HS develop?
HS begins to develop mostly only after puberty has struck, and is therefore often confused with acne (hence the name Acne Inversa). Despite there being no known cure, this ailment can last for anywhere between a few years to a lifetime and may manifest itself in the form of a single or few lesions in certain specified areas of the skin.

There are two systems that have been used to track the development of HS over the years. The Hurley staging system notes the first stage as the development of lesions that heal without scarring, following by more severe lesions, as well as the presence of sinus tracts, and finally, a broader presence of lesions along with scarring and subcutaneous tracts in the skin. 


The Sartorius staging system is more complex and marks each stage using determinants such as the areas of the skin on which the lesions have occurred (with earlier stages having more localized lesions), volume and type of lesions (which can also include tracts, nodules, abscesses, and fistulas), distance between lesions, and finally, the presence of unaffected skin in -between lesions. 


Depending on the stage the disease presents itself at, both the individual lesions, as well as the ailment itself, can be treated. However, there is a limited number of medical treatments and treatment guidelines available​ due to a lack of scientific studies into the disease itself. It has also been hypothesized that diagnostic delays have caused this disease to continue and go untreated in numerous cases due to a lack of unawareness in both patients and physicians.   


What causes HS?
The lesions related to HS tend to arise when there are certain abnormal blockages around the hair follicles, which causes the immune system to react, with the latter believing these blockages to be external bacteria attacking the body. This response by the immune system leads to the formation of abscesses and lesions and causes further itchiness, redness, swelling - all common symptoms of inflammation. Most cases of this disease are reported to be mild to moderate, particularly with respect to the inflamed bumps. 


While the cause of the inflammation has been identified, the difficulty in preventing, diagnosing and treating this illness arises when attempting to discover the underlying cause. A number of factors could potentially be involved in the initial development of the disease, including auto-inflammatory issues, hormonal discrepancies, genetic predisposition or specified environmental interactions.

Studies have suggested there could be a pattern related to dominant genes passing down the illness, but research into the same has not been studied enough to confirm the same. It is important to note, however, that unlike certain other skin conditions, the cause and development of HS are not linked to poor hygiene or any infection, contagious or otherwise.    


How to Treat HS
Dealing with this ailment can be overwhelming given that it results in chronic pain, often caused by inflammation and itchy, sore lesions that develop under the arms, the breasts, and other areas where skin folds. While this disease is not curable, it is treatable, both medically and with lifestyle changes, and the symptoms tend to peak as one ages, particularly once the person suffering has crossed 30-40 years of age.

Medical treatments include the use of Dapsone therapy, as well as Humira therapy, either of which may be prescribed by your doctor along with regular dosages of antibiotics, anti-inflammatory drugs, and corticosteroids. When oral, topical and similar medicinal treatments have been applied and the condition continues to worsen, the concerned doctor may recommend surgery to cover the lesions and abscesses. 


While medicinal options are a must-have for treating HS, especially in the later stages, there are also numerous things that patients can do to reduce the flaring up of lesions, and attempt to stop them from spreading or worsening. These include:

1. Avoiding Heat
Under ordinary circumstances, sweating might be good for your skin, but in the case of HS, the goal should be to stay cool and dry, which can be a difficult task given the presence of lesions in areas that are rich in sweat glands. Excessive heat can trigger painful flare-ups, as can excessive sweating. For added safety, it may be good to discuss the brands of antiperspirants to best use with your doctor to make sure they're not too harsh on the skin. 


2. Not Smoking
If you’re a cigarette smoker with HS, there are high chances you suffer from flare-ups more than non-smokers. To reduce the number of leaky lesions, it’s best to quit cigarettes, as this can also improve your overall stamina and health. 


3. Staying Healthy
Now that you’ve quit smoking, you can work on shedding those extra pounds. This is yet another lifestyle change that can help you cope with the stress and pain of HS. Maintaining a good diet and regular exercise can go a long way in helping your body fight the triggers of inflammation. Reduction in weight can often mean a reduction in the number of lesions that flare-up.  


4. Not Stressing Out
This is a tricky one, especially given that HS can cause anxiety and depression, and it can be even trickier if you’ve just quit smoking cigarettes and are finally starting to diet. Nonetheless, it is important to know that stress can be a major trigger of flare-ups, so whether it is deep breathing or meditation, it is vital to find methods of keeping calm and staying centered. 


5. Keeping Away From Razors
Let us not forget that many abscesses related to HS begin at the base of hair follicles. While it may be a matter of simple personal hygiene, it is advisable to use alternative methods of hair removal, as most lesions caused by Acne Inversa occur in regions of the body where hair growth is prevalent. Even minor outbreaks of HS on the skin can cause irritation, and razors can result in further inflammation and even the spread of lesions.  


6. Loose Clothes Are the Best
It can often be tempting, or even habitual to wear a nice tight sweater on a cold day or a well-fitted t-shirt when exercising. However, it's best to avoid these clothes if you're suffering from HS. Acne Inversa tends to break out in areas of the body where skin rubs together. A similar effect occurs when synthetic fibers rub against the skin, causing friction, which can result in additional painful lesions, as well as trigger flare-ups.

Fat cells may explain why melanoma becomes 'aggressive and violent'

New research may have found the answer to a question that has preoccupied scientists for years: What makes melanoma become 'aggressive and violent'? The answer brings with it some important therapeutic implications.

Despite making up only 1% of all skin cancers, melanoma  is the cause of most skin cancer-related deaths.

According to doctors' estimates, in 2019 and in the United States, doctors will diagnose 96,480 new cases of melanoma, and 7,230 people will die from the disease.

When the cancer is localized, or in situ, the 5 year outlook is close to 100%. However, as the cancer progresses, the survival rate can drop as low as 23%.

But what causes melanoma to become aggressive? A new study has found an intriguing answer.

How fat cells help melanoma advance

Researchers collected dozens of tissue samples from people who had been diagnosed with melanoma.

Examining the biopsy samples revealed that, at first, tumors grow in a "lateral" phase in the upper epidermal layer of the skin. Although this stage is proliferative, it is highly treatable.

However, at a certain point, tumors enter a "vertical," more aggressive, stage, wherein they start to invade the deeper layers of skin that contain fat. So, the researchers asked, what causes this "metastatic switch"?

Clinical analyses of in situ melanoma cells — from melanoma in its highly treatable stage — showed that adipocytes, or fat cells, which usually reside in the deeper layers of the skin, had risen to the upper dermis layer, close to melanoma cells. This event correlated with the aggressiveness of the disease.

Furthermore, co-culture experiments of fat cells and melanoma cells revealed that fat cells secrete two cytokines that triggered the metastatic switch.

Namely, they secreted interleukin-6 and tumour necrosis factor-alpha, which are cytokines, or proteins, that have the ability to alter gene expression.

"We asked ourselves what fat cells were doing [near melanoma cells] and began to investigate," explains the Prof.

"We placed the fat cells on a petri dish near melanoma cells and followed the interactions between them."

"Our experiments have shown that the main effect of cytokines is to reduce the expression of a gene called miRNA 211, which inhibits the expression of a melanoma receptor of [transforming growth factor-beta (TGF-beta)], a protein that is always present in the skin," reports the Prof.

"The tumor absorbs a high concentration of TGF-beta, which stimulates melanoma cells and renders them aggressive."

Returning melanoma to its 'calm' state

"It is important to note that we found the process reversible in the laboratory: When we removed the fat cells from melanoma, the cancer cells calmed down and stopped migrating," explains Prof. Levy.
So, the researchers also used a mouse model of melanoma to see if they could block this metastatic process. Repressing miRNA 211 causes melanoma to metastasize to distant organs in the body, while expressing the gene stopped the metastasis.

The team went on to experiment with drugs, which, although never used to treat melanoma specifically, had the potential to inhibit cytokines and TGF-beta.

"We are talking about substances that are currently being studied as possible treatments for pancreatic cancer  and are also in clinical trials for prostate, breast, ovarian, and bladder cancers," notes the Dr.

"We saw that they restrained the metastatic process and that the melanoma returned to its relatively 'calm' and dormant state."
  
"We have answered a major question that has preoccupied scientists for years: What makes melanoma change form, turning aggressive and violent?" says the Prof.

"Locked in the skin's outer layer, the epidermis, melanoma is very treatable; it is still stage 1, it has not penetrated the dermis to spread through blood vessels to other parts of the body, and it can simply be removed without further damage," continues the Prof.

"Melanoma turns fatal when it 'wakes up,' sending cancer cells to the dermis layer of skin below the epidermis and metastasizing in vital organs."

"Blocking the transformation of melanoma is one of the primary targets of cancer research today, and we now know that fat cells are involved in this change."

"Our findings can serve as a basis for the development of new drugs to halt the spread of melanoma — therapies that already exist, but were never used for this purpose," the Prof. concludes, adding that in the future, the researchers plan to collaborate with pharmaceutical companies in order to develop such treatments.

Daily ibuprofen may prevent Alzheimer's

Taking over-the-counter medication ibuprofen daily may prevent the onset of Alzheimer's disease, a study claims.

A team has successfully carried out studies suggesting that, if started early enough, a daily regimen of the non-prescription NSAID (nonsteroidal anti-inflammatory drug) ibuprofen can ward off Alzheimer's.

In most people, the rate of production of peptide amyloid beta protein 42 (Abeta42) is almost exactly the same regardless of sex or age, said a researcher.

However, if that rate of production is two to three times higher, those individuals are destined to develop Alzheimer's disease.

Contrary to the widely held belief that Abeta 42 is made only in the brain, the team demonstrated that the peptide is made in all organs of the body and is secreted in saliva from the submandibular gland.
As a result, with as little as one teaspoon of saliva, it is possible to predict whether an individual is destined to develop Alzheimer's disease.

This gives them an opportunity to begin taking early preventive measures such as consuming non-prescription non-steroidal drugs (NSAIDs) such as ibuprofen, researchers said.

"Knowing that the prevalence of clinical Alzheimer's Disease commences at age 65, we recommend that people get tested ten years before, at age 55, when the onset of Alzheimer's would typically begin," he said.

"If they exhibit elevated Abeta 42 levels then, that is the time to begin taking daily ibuprofen to ward off the disease," he said.

According to the 2016 report, the disease affects an estimated 47 million people worldwide, costing health care systems over USD 818 billion per year.

Most clinical trials to date have focused on patients whose cognitive deficits are already mild to severe, and when the therapeutic opportunities in this late stage of the disease are minimal, researchers said.

Consequently, every therapeutic trial has failed to arrest the disease's progression, they said.

Anti-ageing serum can do more harm than good for elderly people

A team of researchers has found that by removing senescent cells, it may bring us one step closer to the "fountain of youth". However, experts warned that an anti-ageing serum could be a few years off yet as the drugs may be unsafe for elderly people.
 
A drug has previously been found to help elderly mice regrow their hair, run faster and live for longer. It works by removing cells in skin tissue that naturally accumulate as the rodents grow older.

But the senescent cells - which are unable to reproduce themselves and prevent tissue growth - are also found in humans. The study was published in the journal Trends in Molecule Medicine.

"This strategy may bring us one step closer to the " fountain of youth," but it's important to be cautious and not hype," said researcher of aging Peter de Keizer of the Erasmus University Medical Center in the Netherlands.

They noticed that these permanently arrested cells accumulate in mature tissue and that some of them secrete factors that are harmful to tissue function and impair their neighboring cells.

To explain what causes this noise in the system, de Keizer proposes a "senescence-stem lock model" in which the chronic secretion of pro-inflammatory factors by these senescent cells keeps neighboring cells in a permanent stem-like state and thereby prevents proper tissue renewal.

"A perfect anti-senescence therapy would not only clear senescent cells, but also kick-start tissue rejuvenation by stimulating differentiation of nearby stem cells. This may be complementary with, for instance, the exciting approaches recently made in the field of transient expression of stem cell factors," de Keizer stated.
There's still much basic research to be done before humans visit their local rejuvenation clinic for their annual shot of anti-aging serum.

Senescent cells do have a temporary role in wound healing, so you don't want to eliminate them when you are injured or at the wrong point in time.

"I would also advise caution for claiming too much, too soon about the benefits of the fast-growing list of therapeutic compounds that are being discovered. That being said, these are clearly very exciting times, and I am confident we will find applicable anti-senescence treatments that can counteract age-related pathologies," de Keizer explained.

Researchers will also need to think about when such treatments should be administered (such as before or after the onset of certain conditions) and who would benefit the most.

The potentially high cost of an anti-aging therapy, as well as off-target toxicity, could also be limiting factors for widespread market use as it is translated.

De Keizer, who plans to co-found a start-up based on the discovery of anti-senescence compounds from his lab, is hopeful that cell-penetrating peptides that can block specific activities of these retired cells could be the path forward over broad-range inhibitors.

A team of researchers has found potential source of insulin-producing cells in the adult human pancreas, offering new hope to diabetics.
Cells that express neurogenin 3 (NGN3) may one day be harnessed to create a plentiful supply of insulin-producing beta cells for the treatment of diabetes, the University of South Florida study suggests.
NGN3 is the master gene driving development of the human endocrine pancreas, including the beta cells that make and secrete the hormone insulin, which helps control blood sugar levels. In type 1, or juvenile, diabetes, insulin-producing beta cells are destroyed by the person’s immune system, and patients need insulin injections to survive. Patients with the more common type 2 diabetes, referred to as adult-onset diabetes, produce insulin but their bodies cannot use it properly, and they often require extra insulin.
In the study, researchers from the Children’s Research Institute, USF Health Morsani College of Medicine; Johns Hopkins University School of Medicine; and the University of Illinois at Chicago, detected the NGN3 protein in histologically normal pancreatic biopsies from two sources-cadavers and patients requiring biopsy for diagnostic purposes.
NGN3 expression in the adult pancreas was unexpected, because it cannot be detected in the adult rodent pancreas – only during fetal development, said principal investigator Michael Shamblott.
Now that researchers know these NGN3 cells are a normal part of adult human pancreas biology, they can learn to increase them and to coax them towards becoming differentiated pancreatic endocrine cells by using specific drugs, Shamblott noted, adding that the goal is to regenerate functional beta cells that can cure diabetes.
The study is published in the journal PLOS ONE.
