Hyperacidity: Not hyped enough

Have you ever felt a burning sensation that runs up from your chest area and into your throat? It starts off feeling like an annoyance but in some cases, it becomes persistent, turning into a disease.

Jim Palmer, one of the greatest pitchers in the history of baseball, had chronicled about his battle with heartburn. “I had frequent and persistent heartburn for many years during my baseball career. I thought it was normal. I tried changing my eating habits in an effort to allow three hours between eating and going to bed. None of these efforts solved the problem. I have found that more and more people are identifying their own experiences with frequent and persistent heartburn as reflux disease,” he had chronicled. Today, at 74 he is considered one of the famous spokespersons, in addition to former US presidents Bill Clinton and George Bush, who’s been helping in increasing awareness about gastroesophageal reflux disease (GERD) — which in layman term means ‘acidity’.
Hyperacidity is a common symptom that is synonymous with burning pain in the upper abdomen and or chest, usually secondary to bacterial infection, intake of certain drugs, lifestyle habits such as alcohol consumption and smoking. And it doesn’t only affect older people — even active, healthy teens can suffer from acidity, explain experts.

Dr G.V. Rao, Director, Chief of Surgical Gastroenterology, Transplantation Services and Minimally Invasive Surgery, Asian Institute of Gastroenterology, explains it further. “Eating large amounts of food and inadequate chewing of food causes upper abdominal fullness, bloating, nausea, and vomiting. When one has acidity, one experiences a burning sensation under the breastbone, accompanied by reflux of bitter contents. It usually worsens on bending forward or lying down,” says Dr Rao.

What causes acidity?
According to Dr Nitesh Pratap, Consultant Gastroenterologist, KIMS Hospitals, there are multiple causes of acidity, including consuming spicy food, eating a heavy meal and eating citric foods. “In fact, chocolates can also cause acidity. And smoking and alcohol intake can worsen the symptoms. Stomach infections with H. pylori, pain medications such as ibuprofen and aspirin and high levels of stress/anxiety can also trigger acidity,” adds Dr Pratap.

Another commonly ignored aspect that induces acidity in most people is improper timing and spacing between meals during the day. According to Dr Rao, eating large quantities of food late in the night and immediately retiring to bed can also aggravate symptoms.

How can you treat the disease?

The treatment of acidity includes lifestyle modification as well as medication. “Lifestyle modification includes eating small frequent meals, avoiding spicy food, excess tea and coffee,” advises Dr Pratap.

The doctor also adds that antacids, proton pump inhibitors and antibiotics for H. pylori, as per medical advice are some of the medications used to treat acidity.

In some cases, people take antacids but still do not respond to it. In such cases, the treating doctor will evaluate for diseases causing acidity. “These tests include blood test and, if required, an upper gastrointestinal endoscopy in which a flexible scope is passed into the stomach to look for the cause of acidity. If a person with acidity has significant loss of weight, loss of appetite, vomiting or black-coloured stool, he should consult the specialist at the earliest,” the doctor adds.

Common acid reflux medications linked to increased kidney disease risk

Proton pump inhibitors (PPIs), which include well-known brand names Prilosec, Nexium and Prevacid, are among the most commonly prescribed medications in the world. Approximately 10 percent of adults in the United States take these drugs for frequent heartburn, acid reflux and gastroesophageal reflux disease.

In the study, the team found that patients who took PPIs were more likely to experience kidney disease than people who took histamine-2 receptor antagonists, another form of antacid that treats the same conditions and includes the brands Pepcid and Zantac.

"Post-marketing data collected by the FDA and deposited in the FAERS database allows us to look for potential adverse effects beyond what was found in a clinical trial, which may not have lasted as long or included as much diversity as the FAERS does," said senior author.

The FAERS database contains more than 10 million patient records -- all voluntary reports of adverse effects while taking a medication. The research team focused on patients who took PPIs and no other medications, narrowing their study population down to approximately 43,000 patients. They applied a mathematical algorithm to look for statistically significant differences in reported kidney-related complications between patients who took PPIs and the control group, approximately 8,000 patients who took histamine-2 receptor blockers, such as Zantac or Pepcid, and no other medications.

Here's what they found: Patients who took only PPIs reported a kidney-related adverse reaction at a frequency of 5.6 percent, compared to 0.7 percent for patients who took only histamine-2 receptor antagonists.

Drilling down, the team found that, compared to the control group, patients who took only PPIs were 28.4 times more likely to report chronic kidney disease, as well as acute kidney injury (4.2 times more likely), end-stage renal disease (35.5 times more likely) and unspecified kidney impairment (8 times more likely). Patients who took PPIs were also more likely to experience electrolyte abnormalities, but this varied more by individual PPI, while the kidney-specific effects held true for all five PPIs examined.

Researchers cautioned that this study does not reveal the absolute frequency of these kidney-related complaints for all people taking PPIs, since reporting in the FAERS is voluntary. He also says it's possible, though unlikely, the effect could be due to unidentified confounding factors. A large, randomized, controlled clinical trial would be needed to definitively show causality between PPI usage and absolute risk of kidney disease in humans.

As the World Health Organization notes, PPIs are essential medicines for many people, helping them to control symptoms that are often painful and disruptive to daily life. But researcher hopes this initial data will prompt health care providers to provide the appropriate warnings, education and monitoring for patients who require PPIs, particularly if they are already at elevated risk for kidney disease and electrolyte abnormalities. Researchers made similar recommendations following a study that found evidence in mice and humans that PPIs promote chronic liver disease.

PPIs are relatively inexpensive medications, retailing for approximately for a recommended two-week course of generic, over-the-counter Prilosec (omeprazole). But the frequency of use adds up -- one study estimated Americans spend $11 billion on PPIs each year. There are inexpensive and readily available alternatives to PPIs. However, non-PPI-based antacids (e.g., Pepto-Bismol, Tums, histamine-2 receptor antagonists) may not be as effective.

The Relationship between Antacids and the Risk of Stroke

Many people of different ages often suffer from heartburn which is caused by rising stomach acid and whose main symptom is a burning sensation in the chest and throat. This occurs because of physical factors such as pregnancy, defects in the esophageal sphincter that do not prevent the rise of acid, or because of eating fatty foods and drinking carbonated beverages. The burning sensation is usually intolerable, which causes many people to take antacid medications, either prescription or OTC, and assume that they have treated the problem.

However, in a recent study it was found that these drugs significantly increase the risk of ischemic stroke, a fact that sheds a completely new light on the dangers of using them. In the next article, you’ll learn more about the findings of this study, and also a few natural and effective ways to deal with heartburn that will prevent you from having to reach for the medicine shelf.

How do antacids work?

As mentioned above, heartburn is caused by the rise of gastric acid to the esophagus that causes a burning sensation in the chest area. Antacids used to treat heartburn which are included in the Proton Pump Inhibitors (PPI's) group, treat the problem by reducing acid secretion in the stomach so that it can’t rise up through the esophagus. There are few differences between the various antacids, but most of them work in this method and contain Proton Pump Inhibitors.

What is the possible risk of taking antacids?

According to a study, these common drugs are not as safe to use as they were thought to be. During the course of the study, doctors analyzed medical records of nearly 245,000 people  aged 57, who underwent an endoscopy procedure to find the causes of digestive difficulties and stomach aches. When the subjects returned to follow-up tests after the procedure, 9,500 of them had an ischemic stroke caused by a disturbance in blood flow to the brain.
A researcher and his colleagues examined whether patients with stroke took one of four common PPI drugs: omeprazole, pantoprazole (Protonix), lansoprazole (Prevacid) and esomeprazole (Nexium).

The results of the comprehensive study

The researcher and his colleagues found that there was a link between taking one of these drugs and an increased risk of stroke, since people who regularly took these drugs were 21 percent more likely to have ischemic stroke than non-users. Furthermore, subjects taking high-dose PPIs had a 30 to 94% chance of having the stroke of the type described.
These results are preliminary and diagnostic, but there appears to be a high correlation between taking antacids and the risk of ischemic stroke. Therefore, if you are taking PPI’s regularly or considering doing so, you may want to consult your doctor to avoid excessive amounts or to consider alternative treatment options.

Natural heartburn treatments

The temptation to reach for a pill when you feel heartburn coming on is great, but as mentioned, there may be several health risks. In order to avoid this problem, and still treat the heartburn, you can make use of a number of different and varied methods while also changing your diet. Just pick one of the following natural ways to soothe heartburn and eliminate the need to take medicine:
Eating bananas - This tasty yellow fruit contains natural acids which work against those that are formed in the stomach which rise to the esophagus. It is recommended to eat one ripe banana a day to enjoy its long-term benefits and to reduce heartburn.
Drink soda water – Baking soda has many wonderful benefits one of which is calming heartburn. This is due to the fact that baking soda has a basic acidity level (pH = 7), so it contradicts the acid in the stomach and neutralizes it. In order to enjoy this natural solution, add between 1/2 - 1 teaspoon of baking soda to 200 ml of water, mix until dissolved and drink. Drink no more than 3 ½ teaspoons in 24 hours.
Protect the body with aloe vera juice - The wonderful aloe vera plant helps not only to soothe sunburn but also burning caused by a rise in gastric acid. This is due to the anti-inflammatory properties of the juice produced from the plant, which calms the irritation in the stomach during heartburn. If you experience heartburn, try drinking a glass of aloe vera juice to prevent the condition from getting worse.
Treat yourself with Chamomile Tea - This fragrant tea helps greatly in cases of digestive difficulties and heartburn. Drinking a cup of chamomile tea between half an hour and an hour before bedtime will help relieve the inflammation and irritation in the stomach that lead to heartburn. Moreover, chamomile relaxes the body and mind and therefore contributes to the decrease in tension that also causes heartburn.
Apple Cider Vinegar  - Apple cider vinegar is used to treat stomach acid buildup, as ironic as that may sound. The cider helps digestion by breaking down fats, despite the acidity factor. Mix one or two teaspoons of the apple cider vinegar with a glass of water and drink it before meals, or whenever you feel an acid reflux attack coming on.

Prolonged use of antacids harms kidneys

Recent global studies suggesting that the prolonged use of widely prescribed anti-acidity pills to treat “gas” and heartburn might be linked to long-term kidney damage, acute renal disease and chronic kidney disease have sparked fresh debate in the medical community here.

Though a few initial reports about the association of these drugs — also called PPIs (proton pump inhibitors) — with kidney disease have been published in reputed international medical journals over the last couple of years, it is only now that there are studies suggesting that it’s more serious and linked to both acute kidney injury and chronic kidney disease, doctors told.

Also, since these reports are mostly restricted to nephrology journals and limited to only specialists, many physicians may not be really aware of these adverse effects. Increasing concerns about these drugs could be the reason their safety profile was discussed at the Annual Conference of Nephrologists and at an Association of Physicians conference.

PPIs rank among the top-10 prescribed classes of drugs and are commonly used to treat acid reflux, indigestion, and peptic ulcers. They are prescribed across specialities, from orthopaedics and cardiology to internal medicine and surgery. A US-based nephrologist, spoke about the drug’s risks at the annual conference recently. He told “PPIs should be ideally prescribed in the approved indications, if possible for less than eight weeks. Beyond this, if a patient is on PPIs, kidney function and magnesium levels need monitoring.”

Most patients do not know about side-effects like CKD (chronic kidney disease) as in the early stages CKD from any cause usually shows no symptom., said a nephrologist. Though no advisory has been issued against the medicine, the Dr. said drug regulatory authorities will be informed.
