Portable 'electronic nose' can accurately screen Barrett's oesophagus

A new Portable 'electronic nose' can accurately screen Barrett's oesophagus.

A portable 'electronic nose' can accurately pick up the precursor condition to food pipe (oesophageal) cancer, known as Barrett's oesophagus, indicates a proof of principle study, published online in the journal Gut. Researchers found that the accuracy of the electronic nose was high, especially among patients taking a high-dose proton pump inhibitor, an acid reduction medicine.


This inexpensive and non-invasive technique may be a promising test for diagnosing oesophageal cancer in primary care, say the researchers. 


The number of new cases of this cancer has risen sixfold over the past few decades, with most cases being diagnosed when the disease is already advanced and difficult to treat. 


Barrett's oesophagus, a condition in which healthy cells lining the food pipe are replaced by abnormal cells, is a known precursor to the disease. 


It can be diagnosed using a long flexible tube with a camera on the end (endoscope). But the procedure is expensive and unpleasant for patients, making it unsuitable for population screening. 


So scientists have been exploring other less invasive and cheaper options, including analysis of volatile organic compounds, or VOCs for short. 


VOCs are gaseous compounds produced by metabolic processes in the body, including inflammation and abnormal cellular activity, which can be detected in a person's breath. 


The researchers wanted to see how effective and accurate a portable 'electronic nose' device would be for picking up Barrett's oesophagus. 


Some 402 adults scheduled for endoscopy breathed into a highly sensitive electronic nose, designed to detect subtle differences in VOC patterns, for 5 minutes. 


Of this group, 129 patients had Barrett's oesophagus; 141 had acid reflux disease, including 50 who had an inflamed oesophagus as a result; and 132 had a normal oesophagus or hiatus hernia that accounted for their symptoms. 


Analysis of the VOC profiles detected by the electronic nose showed that these differed significantly among patients with Barrett's oesophagus, acid reflux, and those with a normal oesophagus/hernia.

The sensitivity of the device was 91%--in other words, its ability to pick up Barrett's oesophagus--and its specificity was 74%--in other words, its ability to detect those without the condition. 


When the analysis was further restricted to patients who had been taking drugs (proton pump inhibitors) to curb stomach acid production for at least a month or those with a hiatus hernia, both of which are likely to muddy the diagnostic waters, its accuracy was still good. 


"The findings of our study provide evidence that patients with [Barrett's oesophagus] have VOC breath prints different from those without," they write. 


While it's still not clear exactly how breath VOCs indicate cancerous cell changes, the sensitivity and specificity of VOC breath testing for Barrett's oesophagus is comparable to that of breast cancer and bowel cancer screening, they explain. 


Chemical analytical techniques for VOC analysis have also been used, and are very effective, but they are also expensive. What's more, they are time-consuming and require highly skilled staff to operate them and analyse the results, point out the researchers. 


Further research will be required to validate these findings in a much larger study, they emphasise. But they conclude: "Given the high tolerability, high acceptability and low costs, breath testing may be a promising approach to be used for non-invasive screening for [Barrett's oesophagus] in a primary care setting."

A doctor explains why period cramps can hurt way more than a heart attack


Period cramps can be really intense.
 
Two years ago, according to a published story in which a professor said period cramps can be "almost as bad as having a heart attack." 

Last week, the story resurfaced, going viral on social media. 

But gynecologist, who blogs about women's health, wrote that the comparison between heart attacks and period cramps doesn't really work. 

That's because heart attacks are often just mildly painful — and in many cases, they're not painful at all. 

It would be more accurate, she wrote, to compare cramps to being in labor.               

Back in 2016,according to an article, on the monthly misery faced by women all over the globe: period cramps. The story acknowledged that menstrual pain can be serious and even debilitating — and yet, there's been disappointingly little research on how we can treat it.

But the story had a resurgence on social media last week after a lady zeroed in on a single quote from the piece. Reproductive health professor  told  that patients have described cramp pain as "almost as bad as having a heart attack."

That quote sparked immediate interest. Other news outlets  picked up the story, and it spread like wildfire on Twitter. To many women, the comparison wasn't so much surprising as it was glaringly obvious.

But gynecologist, who writes a popular health blog, has called that comparison into question. In a new post published, the Dr. wrote that period cramps are often much more painful than heart attacks. 

"If you are waiting for terrible , excruciating chest pain to tell you that you are having a heart attack, well, you are going to miss the heart attack," the Dr. wrote. "Heart attacks often produce vague symptoms or mild pain, that is why many people ignore them ... In addition, more than 40 % of women  have no pain with heart attacks. It would be dangerous for women to think that a heart attack should be at least as bad as their menstrual cramps."

Besides pain, heart attack symptoms can also include shortness of breath, a lightheaded feeling, nausea, vomiting, and unexplained tiredness, according to the specialists.

But primary dysmenorrhea — the scientific term for painful periods — always hurts.

Period pain happens mostly because of substances called prostaglandins, the Dr.  explained in the post. They're released from the lining of the uterus and make it contract. And during these period contractions, pressure on the uterus can be just as high as it is during the "pushing" stage of labor, she added.

"So if you need an analogy to describe period pain," the Dr. wrote, "use labor or cutting your finger off without an anesthetic."

Cervical cancer: Myths and facts


Of all the cancers affecting women, cervical cancer has emerged as the second most common cause of cancer-related deaths in India.

Women between 15 and 44 years of age are particularly at risk. There are about 122,000 new cases of cervical cancer annually in the country, with 67,500 women succumbing to the disease.

"Cervical cancer is mostly caused by the Human Papilloma Virus or HPV.It is a condition that affects lining of the cervix, or the lower part of the uterus. This cancer develops gradually and becomes full-blown over time," said a Dr.

HPV infection can spread through sexual or skin-to-skin contact. Though this infection usually goes away on its own over time in most women, in others, it can persist and cause precancerous changes in the cells of the cervix.

"Some symptoms of this cancer include abnormal vaginal bleeding, vaginal bleeding after menopause or sex, bleeding or spotting between periods, longer or heavier menstrual periods than usual, other abnormal vaginal discharge, and pain during sexual intercourse," the Dr. added.

As with any other disease, there are certain myths associated with cervical cancer as well.

# Myth 1 Women without a family history of cervical cancer do not need to get screened.
Those without a family history of this condition may also be at risk. This is because the HPV infection can spread through sexual contact. It is, thus, important to get take preventive measures and get Pap tests done.

# Myth 2 Pap tests should be done every year
There is no need to get a PAP test every year if this test and the one for HPV are normal. The recommended schedule is once in three years in women between 21 and 29; and once every five years in women between 30 and 64.
Those above the age of 64 should follow their doctor's advice.

# Myth 3 It is not possible to prevent cervical cancer
Cervical cancer is one of the most preventable cancers. A Pap test can help determine any kind of changes in the cells of the cervix. Once any such change is detected, it is possible to start treatment early and prevent the cancer from developing. Pap test is imperative for anyone who has been sexually active or in women who have HPV and are smokers.

# Myth 4 Women with no symptoms need not get tested
HPV infections do not show any symptoms in most cases. While there are different types of HPV, some high-risk types are associated with cervical cancer and can go undetected until the development of abnormal cells. This makes it important for women to get tested on a regular basis.

# Myth 5 Women who have had a hysterectomy do not need to get tested
It is imperative to undergo a screening for cervical cancer irrespective of whether a woman has had a hysterectomy. Only those who have undergone a total  hysterectomy (a process where both cervix and uterus are removed) need not get screened.

# Myth 6 Pap test is similar to a pelvic exam
Pelvic exam is a physical examination of the pelvis, vagina, and pelvic floor (the area encompassed by the hip bones), whereas, in a Pap test, cells from the cervix are gently scraped away and examined for abnormal growth.

# Myth 7 Condoms can prevent HPV
Condoms can help prevent certain sexually prevented infections (STI). However, this may not be true for HPV as the virus can inhabit areas that condoms do not cover.

# Myth 8 Cervical cancer treatment can cause infertility
Advances in medicine and surgery have ensured that cervical cancer treatment does not affect fertility in a woman. These surgical treatments can help a woman conceive by removing only the cervix and not the uterus.

"Timely screening and detection are two very important aspects of combating cervical cancer, as it is a completely treatable condition. Apart from this, women should make some basic lifestyle changes to avoid contracting an infection. These include avoiding sexual contact with multiple partners; getting screened on a timely basis; quitting smoking; consuming a healthy diet rich in fruits, vegetables, and whole grains; and maintaining a healthy weight. All these can go a long way in preventing cervical cancer," the Dr. concluded.

New finding confirms link between chemical in plastic and cancer developmen

Researchers have found liver tumors in mice exposed to the Bisphenol A (BPA) - chemical present in plastic - via their mothers during gestation and nursing.

The study is one of the first to show a significant association between BPA and cancer development.
Caren Weinhouse, University of Michigan doctoral student in the School of Public Health's Department of Environmental Health Sciences, said that they found that 27 percent of the mice exposed to one of three different doses of BPA through their mother's diet developed liver tumors and some precancerous lesions. The higher the dosage, the more likely they were to present with tumors.
Mice whose mothers received the highest dosage, 50 mg of BPA per kg diet, were seven times more likely to have tumors than those whose mothers were not exposed to BPA.

Bisphenol A, or BPA, is a chemical most commonly found in plastics, cash register receipts and the lining of food cans

The researchers fed 6-week-old female mice diets containing one of three environmentally relevant doses of BPA prior to mating, then throughout pregnancy and nursing. They then took one male and one female from each litter and followed them through to 10 months.

In this research, it was the mothers who were exposed before conception. The offspring, therefore, were exposed as developing fetuses and pups, not as adults.

The study has been published in journal Environmental Health Perspectives.
