Artificial Pancreas To Boost Diabetes Treatment

Australian researchers have created the world’s first machine intelligent artificial pancreas that could improve insulin dosing for diabetics. According to researchers, the artificial intelligence-based (AI) technique can also transform the way aeroplane engine’s wear & tear is monitored. “Our technology can recommend the best insulin dosage to keep each individual patient’s blood glucose levels under control with unprecedented stability and safety,” he said. “It will allow for better and more accurate treatment than we’ve ever seen,” he said.

The researchers also applied the same AI technique to aviation turbine engines and their related systems to predict engine component degradation and plan services to improve performance. The AI “allows us to evolve computational models of aviation engines as if they were organisms and it can explain explicitly what it thinks is happening inside the engine”, said a mechanical engineer. The researcher said the AI system learns by forcing mathematical models to evolve, quite literally by using simulated chromosomes, to fit known information.

Giving diabetic women an artificial pancreas could prevent stillbirths and save thousands of lives

Pregnant women have been given an ‘artificial pancreas’ for the first time, in a breakthrough experts hope will save the lives of thousands of babies.

Sixteen British women with type 1 diabetes gave birth to healthy infants during the Cambridge University-led trial.

They are the first women in the world to go through pregnancy with the devices, which use a tablet computer to automatically regulate their insulin levels.

This puts their unborn babies at risk of a number of complications, including premature birth, obesity and even stillbirth.

Half of all babies born to mothers with type 1 diabetes have some form of complication, research suggests.

Stillbirths affect very few of these - less than two per cent of all births - but that rate is about five times higher than it is among healthy mothers.

Some 400,000 people in the UK have type 1 diabetes, an autoimmune disease which means the body cannot produce its own insulin, stopping proper regulation of blood sugar.

It usually first strikes in childhood or adolescence, and unlike type 2 diabetes, is not caused by lifestyle or diet.

People with the condition usually have to inject themselves with insulin several times a day, carefully monitoring their blood sugar levels with finger prick tests.

The artificial pancreas enables this whole process is done automatically.

Scientists found a way to combine two existing devices – pumps which deliver insulin and glucose monitors – into a simple system controlled by a tablet computer.

Blood sugar is continuously monitored by a small sensor inserted beneath the skin of the tummy, this information is sent to a tablet computer, which calculates the right amount of insulin required, and this is pumped into the blood stream via a small machine worn on the belt.

The Cambridge trial, funded by Diabetes UK and the Government’s National Institute of Health Research, found the devices improved women’s control of their blood sugar by 25 per cent.

The results, published in the New England Journal of Medicine, need to be repeated in larger trials, but experts hope
the artificial pancreas could be available on the NHS within two years.

Dr Zoe Stewart, the lead author on the study, said: ‘Managing Type 1 diabetes in pregnancy can be really difficult.

‘Hormonal changes that occur in pregnancy make it difficult to predict the best insulin doses for each woman.

‘The artificial pancreas automates the insulin delivery giving better glucose control than we can achieve with current available treatments.

‘We are so pleased this technology is closer to being a reality for women with type 1 diabetes that want to have a child.’

The devices have previously been trialled on children and adults, but this is the first time that pregnant women - considered a much more risky group - have been able to use the gadgets.

Dr Elizabeth Robertson, director of research at Diabetes UK, said: ‘Artificial pancreas technology has the potential to transform the treatment of Type 1 diabetes, and could be particularly pivotal for women during pregnancy who often struggle with managing their blood glucose levels.

‘This study represents a real breakthrough in helping women to take control of their condition, and we’re very excited about the direction this research is moving.’

She added: ‘Diabetes UK have been investing in the development of the artificial pancreas since 1977 when we bought the first machine in the UK – which was the size of a filing cabinet.

‘Now we’re watching people use tablets or mobile phones, and it’s an incredible achievement.’

Type 1 Diabetes

In people with type 1 diabetes, the pancreas cannot make insulin. This vital hormone helps the body's cells convert sugar into energy. Without it, sugar builds up in the blood and can reach dangerous levels. To avoid life-threatening complications, people with type 1 diabetes must take some form of insulin for their entire lives.

Warning Sign: Unusual Thirst

The symptoms of type 1 diabetes tend to come on suddenly and may include:
  • Feeling more thirsty than usual
  • Dry mouth
  • Fruity breath
  • Frequent urination
 Weight Loss
As blood sugar levels remain high, type 1 diabetes often leads to:
  • Unintentional weight loss
  • Increase in appetite
  • Lack of energy, drowsiness

Skin Problems

Many people with type 1 diabetes experience uncomfortable skin conditions, including:
  • Bacterial infections
  • Fungal infections
  • Itching, dry skin, poor circulation 
Girls with type 1 diabetes are more likely to get genital yeast infections. Babies can develop candidiasis, a severe form of diaper rash caused by yeast that can easily spread from the diaper area to the thighs and stomach.

More Dangerous Signs

When blood sugars are not controlled, type 1 diabetes can cause more serious symptoms, such as:
  • Numbness or tingling in the feet
  • Blurred vision
  • Low blood sugar/hypoglycemia
  • Loss of consciousness
Some patients have no obvious warning signs before falling into a diabetic coma, which requires emergency treatment.

Ketoacidosis
Without treatment, type 1 diabetes deprives the body's cells of the sugar they need for energy. Your body starts burning fat for energy instead, which causes ketones to build up in the blood. These are acids that can poison the body. High levels of acid in the blood and the other abnormalities that result from the change in your blood's pH level may trigger a life-threatening coma known as diabetic ketoacidosis. This is an emergency that must be treated quickly and oftentimes in the hospital.

Type 1 vs. Type 2 Diabetes

In type 1 diabetes, the body's immune system mistakenly attacks and destroys the pancreatic cells that produce insulin. In type 2 diabetes, the pancreas is not under attack and usually produces enough insulin. But for numerous reasons, the body doesn’t use the insulin effectively. The symptoms of the two forms are similar, but usually come on more rapidly in people with type 1.

Causes Type 1 Diabetes

 Doctors aren't sure what makes the immune system turn against the pancreas, but most suspect a combination of genetic susceptibility and environmental factors. Scientists have identified 50 genes or gene regions that raise the risk of developing type 1 diabetes. But genetics alone don't account for all the risk, so having these genes doesn’t mean that you’ll develop type 1 diabetes. Some researchers believe that environmental triggers, such as a virus, or dietary or pregnancy-related factors may play a role as well.

 

Who Gets Type 1 Diabetes

 Type 1 diabetes can develop at any age. However, it accounts for two-thirds of the new cases of diabetes diagnosed in those under the age of 19. There appear to be two peaks in the "age of onset": the first in early childhood and the second during puberty. The condition affects males and females equally, but is more common in whites than in other ethnic groups. According to the World Health Organization, type 1 diabetes is rare in most African, Native American, and Asian population.
