Surgery to improve walking ability in children with cerebral palsy

A recent research has discovered a surgical procedure that can improve walking ability in children with cerebral palsy. The procedure, known as selective dorsal rhizotomy, involves cutting some of the sensory nerves from the legs as they enter the spinal cord in order to relieve stiffness, improve mobility and reduce children's pain levels.

The operation is irreversible, and it had not previously been fully established whether it helped to improve children's quality of life, or what the longer-term effects were. Because of the lack of evidence of the benefits of the procedure, doctors in England established an innovative study that assessed eligible children with cerebral palsy before and after the operation.

Cerebral palsy is the name for a group of lifelong conditions that affect movement and coordination, caused by abnormal development or damage to the brain that occurs before, during or soon after birth. In the UK around 1,700 children, every year are born with cerebral palsy. Treatments include medicines to relieve muscle stiffness, pain relief, and physiotherapy to aid walking.

The team of researchers was instructed to find out whether selective dorsal rhizotomy did improve outcomes for these children with cerebral palsy. "NHS England has now decided that this procedure will be funded as a direct result of this innovative project. It's great to get this decision so that it will make a difference to patients," said one of the lead researchers of the study.

For the research, five centres were selected to perform selective dorsal rhizotomy, resulting in 137 children receiving the procedure. The researchers then collected and analysed data on patients' progress for two years after their operations.

Their results, showed the procedure benefited children by improving their movement, quality of life and levels of pain. They also found that there were no significant health risks to the children from the procedure.

"This procedure is for children with cerebral palsy who can walk but are having problems because the stiffness in their legs leads to pain and a lack of control over movement. Because it involves cutting nerves, the procedure is irreversible," the author explained.

Researchers claim this study shows objectively that the procedure does improve motor function, and doesn't have dangerous side effects. For some children with cerebral palsy and their families, this could really improve the quality of life and help children walk more easily and without the use of walking frames and other aids.

Women's Pain Is Different From Men's—the Drugs Could Be Too

Men and women can’t feel each other's pain. Literally. We have different biological pathways for chronic pain, which means pain-relieving drugs that work for one sex might fail in the other half of the population.

So why don’t we have pain medicines designed just for men or women? The reason is simple: Because no one has looked for them. Drug development begins with studies on rats and mice, and until three years ago, almost all that research used only male animals. As a result, women in particular may be left with unnecessary pain—but men might be too.

Now a study reveals differences in the sensory nerves that enter the spinal cords of men and women with neuropathic pain, which is persistent shooting or burning pain. The first such study in humans, it provides the most compelling evidence yet that we need different drugs for men and women.

"There’s a huge amount of suffering that’s happening that we could solve," says a professor of neuroscience, and an author. “As a field, it would be awesome to start having some success stories.”

Modern-day pain control is notoriously dismal. Our go-to medicines—opioids and anti-inflammatories—are just new versions of opium and  willow bark, substances we’ve used for thousands of years. Although they are remarkably effective in relieving the sudden pain of a broken bone or pulled tooth, they don’t work as well for people with persistent pain that lasts three months or longer.

NEUROPATHY- PREVENTION AND TREATMENT

The term Neuropathy is short for 'peripheral neuropathy'. It relates to nerve damage suffered by the peripheral nervous system, which is in charge of our nerves outside the brain and spinal cord. 
 
Neuropathy is a complication that can be caused by a number of various conditions. Physical trauma, repetitive injury, infections, metabolic problems and exposure to toxins and some drugs can all lead to peripheral neuropathy. 

Signs and symptoms of neuropathy
Neuropathy usually starts as a tingling or burning sensation at our extremities, such as fingers and toes. There is also a loss of sensation at the edge of the nerves that patients have reported feeling like they are wearing a thin stocking or glove on their hands. 
The precise symptoms differ from patient to patient based on the types of nerves affected. There are three types of nerves that may be affected by neuropathy, namely sensory, motor and autonomic.

Sensory nerves
Sensory nerves are responsible for collecting sensory information for the body, such as touch. Neuropathy of the sensory nerves can be expressed as:
- Spreading numbness and tingling in hands and/or feet (which can spread to the arms and legs)
- Burning, sharp or electric-like pain
- Extreme sensitivity to touch
- Problems with coordination
Motor nerves
Motor nerves are the nerves responsible for activating our muscles and control movements.
- Muscle weakness
- Paralysis
Autonomic nerves
Autonomic nerves are responsible for internal actions of the body, such as regulating digestion, heat and blood pressure.
- Intolerance to heat
- Problems with digestion, bladder and bowel control
- Dizziness (or light-headedness) brought about by problems with blood pressure
 
 A common cause of neuropathy: Diabetes
For diabetics, neuropathy can be quite common. That said, about 50% of diabetics who have neuropathy won't notice the symptoms, and it will stay on a very low level. For the other 50%, however, the symptoms will be unavoidable. Pain is the most common complaint, usually a 'prickling', 'stabbing' or 'burning' pain, that happens mostly at night. This, along with a numbness that feels as if the limb is 'asleep' - occurs predominantly in the toes, feet and legs.
What do doctors prescribe for this condition?
Most doctors will stick to the old medical regime of anti-depressants, anti-convulsants, steroid and cortisone injections, lidocaine patches and pain killers. These are powerful drugs that address the painful sensations for short and long term relief. However, as powerful drugs, they have a host of side-effects and must be administered very carefully. Some medical practitioners recommend electrical stimulation of the pain area and of the spine. The medical community is united in agreement that more solutions to this problem are required, but progress has been very slow from the medical and drug industry so far.
 

Natural solutions to neuropathy 
First , if you are suffering from diabetes related neuropathy, you must do the following:
- Give up smoking
- Cut down on alcohol consumption or give it up altogether
- Maintain a healthy weight
- Exercise
Other recommended habits for those suffering from neuropathy include: Wearing clothing that causes less irritation, such as cotton, covering the sensitive areas with wound dressing or cling film and using cold packs. 
Stress relief is also a big help when it comes to neuropathy, and so relaxation techniques such as yoga, meditation and acupuncture will come in handy.
 Vitamin therapy
Clinical studies have shown that certain supplements can have a cumulative effect on the symptoms and causes of neuropathy. These herbal extracts can be taken without worry that they may clash with a parallel medical treatment. Certain herbal extracts and formulas offer a level of relief that has rarely been reached by just using medical procedures. This must be taken daily to achieve the desired relief.
Vitamins B1, B2, B6 and B12
Vitamin B deficiency is one of the major causes of neuropathy, and also one of the best natural solutions. All herbal extracts contain large amounts of vitamin B1, usually in its common form of thiamine. 
Recent natural medications have reverted to using benfotiamine, which has been found to be significantly more effective in delivering vitamin B than thiamine is (x3).
Stabilized R-Alpha Lipoic Acid (R-ALA)
