Dementia Breakthrough: Brain Scans Predict Disease Up to 9 Years Early

Diagnosing dementia early gives us more time to put precautions in place and to study exactly how the condition progresses – and a new method for predicting conditions such as Alzheimer's disease is promising up to nine years of advance warning.

The method, developed by a team from the Queen Mary University of London in the UK and Monash University in Australia, involves a neurobiological model that analyzes brain scans captured by functional magnetic resonance imaging, or fMRI.

In tests, the model was more than 80 percent accurate at predicting the development of dementia. That has huge potential in terms of early diagnosis, and it also addresses another challenge: the large number of people with dementia who don't get diagnosed at all.

"Predicting who is going to get dementia in the future will be vital for developing treatments that can prevent the irreversible loss of brain cells that causes the symptoms of dementia," says neurologist Charles Marshall, from the Queen Mary University of London.

Specifically, the team looked at a key part of the brain's system, called the default mode network (DMN). It's the first part of the brain to be affected by damage caused by Alzheimer's disease, the most common form of dementia, and it plays a key role in connecting up different parts of the brain to keep our thoughts in order.

The test included brain scans of 81 individuals, who were undiagnosed at the time but who later went on to develop dementia, as well as 1,030 matched controls. Using their model, the team was able to spot signs of dementia in some brains up to nine years ahead of time. The fMRI scans are easy to administer, and only take a few minutes to complete.

As well as hitting that 80 percent accuracy rate, the technique could also predict when a dementia diagnosis would be made, within a two-year margin. This is all hugely useful information, and can be cross-referenced against other known risk factors for dementia to build up a better picture of who this disease affects and how it emerges.

"Using these analysis techniques with large datasets we can identify those at high dementia risk, and also learn which environmental risk factors pushed these people into a high-risk zone," says neurologist Samuel Ereira, from the Queen Mary University of London.

The researchers also linked risk factors for Alzheimer's, such as genetics and social isolation, to changes in DMN connectivity. Knowing what triggers these neurobiological conditions is going to be vital in the search for a cure.

Another scenario where the analysis technique is going to be helpful will be in the study of people whose brains show signs of dementia, but who develop symptoms much later on. Understanding why this happens should teach us more about how the condition progresses and how it might be slowed down.

"We hope that the measure of brain function that we have developed will allow us to be much more precise about whether someone is actually going to develop dementia, and how soon, so that we can identify whether they might benefit from future treatments," says Marshall.

The research has been published in Nature Mental Health.

A Single Brain Scan Halfway Through Your Life Can Reveal How Fast You're Aging

The rate at which our bodies age and wear down doesn't necessarily match our actual age, and the differences can help predict lifespan and disease risk. Now, researchers have developed a new tool for assessing biological age from a single brain scan taken halfway through our lives.

The tool, put together by an international team of scientists, is based on a dataset of 1,037 people born in Dunedin in New Zealand in 1972 and 1973. The health of these individuals has been carefully tracked over time, giving scientists a useful long-term database of stats that reflect the body's actual age – not how many birthdays have passed.

Here, the researchers used those stats to assess biological aging, and train a tool they've called DunedinPACNI – Pace of Aging Calculated from NeuroImaging. That sums up its function, which is to match aging to markers in the brain.

The system has the potential to quickly assess the body's age to a good degree of accuracy, and from there health and disease risks, including the chances of dementia. Rather than needing multiple tests over time, just a single brain scan is required.

"What's really cool about this is that we've captured how fast people are aging using data collected in midlife," says neuroscientist Ahmad Hariri from Duke University in North Carolina.

"And it's helping us predict diagnosis of dementia among people who are much older."

 DunedinPACNI takes in 99 key brain measurements to make its assessment, including the thickness of the cerebral cortex – which affects language and thinking – and the volume of gray matter in the brain.

Once the researchers had developed DunedinPACNI, they tested it on a variety of data from other health research projects, covering more than 50,000 people in total. It was shown to work well at estimating biological age, and at predicting future health problems such as cognitive impairment and heart attacks.

It's not a tool that offers perfect accuracy each time, but it scores as well as or better than current biological age assessment methods, the researchers say. What's more, it was shown to be useful across different demographic and socioeconomic groups.

"The link between aging of the brain and body is pretty compelling," says Hariri. "It seems to be capturing something that is reflected in all brains."

If we know that someone's body is aging faster than their chronological age, it means measures can be taken to reduce the risk of health problems – years or even decades before those health problems might otherwise become evident. Changes in diet or exercise at that point could make a major difference.

The researchers are particularly interested in predicting the risk of the various types of dementia, including Alzheimer's disease. As people live longer across the world, rates of dementia are increasing, and the best way to tackle the condition could be to stop it developing in the first place.

"We really think of it as hopefully being a key new tool in forecasting and predicting risk for diseases, especially Alzheimer's and related dementias, and also perhaps gaining a better foothold on progression of disease," says Hariri.

The research has been published in Nature Aging.

Dementia: THESE changes can act as early predictors of brain decline

Changes that can occur decades before dementia-related memory loss kicks in

Dementia is a group of illnesses that affects at least two or more brain functions including memory, thinking and/or judgment. It is not a specific disease rather can be an outcome of several diseases.

According to the World Health Organization (WHO), around 55 million people have dementia worldwide, with over 60% living in low- and middle-income countries. It is currently the seventh leading cause of death among all diseases and one of the major causes of disability and dependency among older people around the world, reports the global health agency.

However, an illness so concerning does not have a cure so far. The only way it can be prevented is through early detection and that can be done by understanding the changes that take place in the brain and the body.

Brain changes can signal cognitive decline

As it is known, dementia is a general term to refer to a group of disorders caused by abnormal brain changes and activities. These then lead to a decline in one's thinking abilities and also affect a person's memory, impairing their day to day functions.

Doctor Sara Imarisio, Head of Research at Alzheimer’s Research UK believes "brain changes linked to the diseases that cause dementia can start in the brain up to 20 years before symptoms like memory loss appear," reports.

“A key early brain change in Alzheimer’s disease is a build-up of the amyloid protein.

"While we can detect levels of this protein with expensive brain scans and other biological tests, not everyone with high levels of amyloid will go on to develop the symptoms of Alzheimer’s," she adds.

What is amyloid protein?

Amyloid is a group of abnormal proteins, usually produced in the bone marrow and can be deposited in any tissue or organ. It can affect different organs in the body including the heart, kidneys, liver, spleen, nervous system and digestive tract. Amyloids are also associated with disorders such as Alzheimer’s disease, a progressive neurologic disorder that affects thinking and memory.

Other changes that can appear years before dementia onset

Apart from certain brain changes, Doctor Imarisio shares that research has also highlighted other changes that precedes classic dementia symptoms.

"People can experience subtle changes in things like sleep, speech, movement, mood and brain activity," the doctor explains.

"These changes might not be noticeable to the human eye, but smart devices could be sensitive enough to detect patterns in them," she adds.

According to her, chronic pain is one such change that can act as an early predictor of dementia.

Study finds chronic pain may be associated with dementia

As per the study, partly funded by National Institute on Aging and published in Journal Pain, those with dementia may experience rising pain 16 years before their diagnosis.

The researchers claimed that both dementia and chronic pain could cause changes to the brain and affect cognitive functions.

Another study conducted by researchers at Université de Paris, which has been collecting data on participants for as many as 27 years, found that those who were diagnosed with dementia complained of slightly more pain as early as 16 years before their diagnosis, driven mostly by differences in pain interference.
