Hypothyroidism may exacerbate diabetes, hypertension-related complications

On World Thyroid Day, May 25th, researchers review the burden of hypothyroidism in India and explain how timely detection and treatment of this condition might prevent related complications from occurring

What is the incidence and disease burden of hypothyroidism in India? How do these figures compare to the rest of the world?
Hypothyroidism, also called underactive thyroid, is when thyroid gland does not produce enough thyroid hormone. The initial symptoms are difficult to notice, and include fatigue, weight gain and feeling cold. Though the condition may progress slowly, if left untreated, hypothyroidism can lead to various health consequences. Thus, the condition can contribute significantly to the non-communicable disease burden in the country.

Studies show that the prevalence of hypothyroidism in developed countries is about 4–5 per cent whereas in India its 10 per cent. Even if we do not take into consideration that rural areas of India may under-report this condition, rates in India are more than double developed countries.

Why is early diagnosis critical?
Diagnosing hypothyroidism is challenging. The same symptoms of thyroid disorder are also common in other diseases. An analysis of newly diagnosed patients in India shows that fatigue is the most common symptom, followed by weight gain with poor appetite, poor memory and concentration, constipation, shortness of breath and feeling cold. If you consider that almost everyone has experienced one of these symptoms at some point without having a thyroid disorder, you can see how easy it is to ignore symptoms. Moreover, the symptoms may not be severe enough or recur noticeably to prompt a patient to consult a doctor.

Nonetheless, hypothyroidism needs to be taken seriously because the condition can progress and cause impairment in health-related quality of life. Patients with hypothyroidism show a greater propensity for comorbidities and complications as compared to the general population. There are evidences which suggest hypothyroidism may exacerbate diabetes- and hypertension-related complications. Moreover, pre-menopausal women with thyroid frequently report menstrual abnormalities such as menorrhagia, irregular cycle and inter-menstrual bleeding. Early diagnosis allows us to treat and manage the condition in a timely manner, which helps in improving health-related quality of life and might prevent related complications from occurring.

What are the treatment and management strategies?
Treatment of choice for hypothyroidism is thyroxine sodium, a once-a-day oral medication, which should only be taken in consultation with your doctor.

Conventionally, hypothyroid patients are advised to take thyroxine sodium tablet on an empty stomach half an hour before breakfast to prevent impairment of absorption by food. Monitoring thyroid function during therapy involves measuring TSH. It is recommended that the TSH is measured 6–8 weeks after initiation of, or a change in thyroxine sodium dose. Once the patient is on a stable dose of thyroxine sodium, annual monitoring of TSH is recommended.

Given the link between hypothyroidism and diabetes / hypertension in India, what are the latest insights on screening in the Indian population?
Screening can help ensure early detection and thus better management of hypothyroidism, before it leads to serious cardiovascular and neurological complications. Data shows high prevalence of hypothyroidism in Indian patients with Type 2 diabetes (26.9 per cent) and hypertension (31.2 per cent). There are evidences which suggest hypothyroidism may exacerbate diabetes- and hypertension-related complications. Moreover, data from India’s largest registry of newly diagnosed hypothyroidism patients shows that more than 70 per cent of patients were women, 86 per cent resided in urban areas, 65 per cent were in the age group of 18-45 years and more than 75 per cent were in upper or upper-middle socio-economic classes. While such trends are only indicative, we can consider them and increase focus on screening for young and middle aged women.

What difference has screening and early diagnosis made in other countries?
Screening for thyroid dysfunction allows early detection and appropriate treatment. Although universal screening for thyroid dysfunction in asymptomatic non-pregnant adults is not recommended, international guidelines from the American Thyroid Association and American Association of Clinical Endocrinologists recommends aggressive case finding in those with autoimmune diseases, family history of thyroid diseases, those with a history of neck radiation to the thyroid gland, patients with psychiatric disorders, dyslipidemia, hypertension. Diagnosing hypothyroidism in these patients early in the course of disease has helped to manage the condition better.

10 Symptoms Of An Underactive Thyroid And Natural Remedies That Can Help You

Can you imagine that a small organ, which weighs about 15g, can turn your health upside down? However it’s true.

Thyroid gland is a tiny organ, located in your neck, in the neighborhood with the voice-box and windpipe.

It normally produces hormones called thyroxine and tri-iodothyronine. Owing to this, thyroid gland has ability to control your metabolic rate, body temperature, menstrual cycle, mental function and heart working.

When something goes wrong in your body, this gland may significantly cut down generation of the hormones, causing hypothyroidism.

In the majority of cases it occurs because of autoimmune disorder, when person’s immune cells begin to destroy his or her thyroid tissue by mistake.

Other common causes of hypothyroidism include inflammatory process, surgical treatment of hyperthyroidism, exposure to radiation and regular consumption of certain medications.

According to statistics, this disorder affects about 4.6% of American adults.

Actually specialists say that women are more prone to the thyroid dysfunction than men. Risks of hypothyroidism raise with passing years.

Though mild hypothyroidism often stays unnoticed, it’s worth getting thyroid examination, if noticed any of these symptoms in yourself:

1. Weight gain – lack of thyroid hormones may dramatically impair your metabolism. That leads to weight gain, even if you eat healthy foods and exercise regularly.

2. Depression – each of us feel upset from time to time. The problem is, if your depression has become a permanent condition. In addition to this, people with hypothyroidism often experience causeless anxiety.

3. Cold intolerance – as your metabolic rate gets decreased, your body may start to produce less of heat. As a result, stay in the cold environment may be a real problem for you.

4. Mental fog – it may appear difficult to concentrate and remember certain familiar things, if your thyroid gland creates too few of the hormones.

5. Menstrual changes – women with thyroid hypo-function usually complain of irregular periods and heavy menstrual bleeding.

6 Fatigue and drowsiness – insufficient synthesis of thyroid hormones may be a culprit of your inexplicable long-term tiredness and irresistable want to nap.

7. Dry skin – decreased metabolism influences not only on the way you spend calories. Your sweating glands stop to work properly too. That’s why your skin isn’t moisturized as needed and gets dry and itchy.

8. Constipation – hypothyroidism may influence on the motility of your digestive tract, causing frequent constipation.

9. Hair loss – deficiency of thyroxine and tri-iodothyronine may affect hair follicles, leading to intensive hair loss and its brittleness.

10. Low libido – it was found that underactive thyroid gland may be a reason of your reduced sex drive.

Fortunately it’s possible to fulfill lack of thyroid hormones by consumption of synthetic hormones, available in the pills.

Besides this, you’re able to correct your thyroid dysfunction and improve the treatment with natural remedies.

Like wide variety of other diseases, hypothyroidism may be prevented and corrected by changing your diet. Experts recommend staying off sugar-rich products, as they boost risks of inflammation and aggravate symptoms of the disease.

Hashimoto’s disease (autoimmune disorder, which was recognized as the most frequent cause of hypothyroidism) may significantly lower levels of the selenium. This micro-element plays a great role in the synthesis of thyroid hormones.

Add more selenium-rich foods like tuna, Brazil nuts and turkey to your diet, or go for special supplements.

Several studies found a link between hypo-function of the thyroid gland and imbalance of the intestinal microflora. Enhance your gut bacteria, taking yogurt, kefir, kombucha or supplements.

Low levels of thyroid hormones are associated with vitamin B12 deficiency. You can find it in dairy products, fortified cereals, eggs and sea-foods.

However you need to visit your doctor, if you’re suspected to have underactive thyroid gland.
