A Frank Conversation About Testicular Cancer

While an uncomfortable topic for many men, testicular cancer is a common cancer in American men between the ages of 15 and 35. While a highly curable disease if caught early, the reluctance to talk about it can lead to delays in diagnosis and treatment. Mina Fam, M.D., director of uro-oncology at Hackensack Meridian Jersey Shore University Medical Center and director of robotic surgery at Hackensack Meridian Ocean University Medical Center, notes early detection is crucial – not only for successful treatment but also for managing the long-term effects that can surface decades later. He shares more during this Testicular Cancer Awareness Month.

Understanding the Numbers

While testicular cancer is relatively rare, accounting for about 1 in every 250 male cancer diagnoses, its incidence has been on the rise in the United States and many other countries for several decades. The American Cancer Society estimates that in 2026, there will be about 9,810 new cases of testicular cancer diagnosed and about 630 deaths from the disease. The good news is that the lifetime risk of dying from testicular cancer is very low, about 1 in 5,000, largely because it is so treatable. The 5-year relative survival rate for localized testicular cancer is 99 percent.

The Power of Early Detection: Know Your Body

One of the most critical messages of Testicular Cancer Awareness Month is the importance of self-examination. Being familiar with your own body is the first line of defense. The most common sign of testicular cancer is a painless lump or swelling in a testicle. Other symptoms can include a feeling of heaviness in the scrotum, a dull ache in the lower abdomen or groin, or a change in the size or feel of a testicle. It's important to remember that these symptoms can also be caused by non-cancerous conditions, but it's always best to get checked by a doctor. Delaying a visit to the doctor can allow the cancer to spread, making it more difficult to treat.

How to Perform a Testicular Self-Exam

Performing a monthly self-exam is a simple and effective way to detect any changes early. The best time to do this is during or after a warm shower or bath when the scrotal skin is relaxed. Stand in front of a mirror and check for any swelling on the skin of the scrotum. Check each testicle separately, using both hands to gently roll each testicle between your thumbs and fingers. Feel for any hard lumps, smooth rounded bumps, or any changes in the size, shape, or consistency of the testicles.

You will also feel a soft, cord-like structure on the top and back of each testicle called the epididymis, which is a normal part of the anatomy. It's normal for one testicle to be slightly larger or hang lower than the other.

Embarrassment should never be a barrier to your health. Doctors are professionals who are there to help, and they have these conversations every day. If you notice any changes or have any concerns, schedule an appointment with your doctor.

Treatment and Survivorship

For those who are diagnosed with testicular cancer, a multidisciplinary team approach is essential. This means having a team of specialists, including urologists, oncologists, and fertility experts, working together to create a comprehensive treatment plan. A crucial part of this plan is addressing concerns about fertility. Treatments like chemotherapy can affect sperm production, so fertility preservation options, such as sperm banking, should be discussed before treatment begins.

The future of testicular cancer treatment is bright, with ongoing clinical trials and biomarker research exploring new approaches for advanced and recurrent disease. There is also a growing awareness of the importance of survivorship and addressing the long-term needs of patients, including fertility, sexual function, and the potential for late side effects of treatment.

Study identifies autoimmune disease associated with testicular cancer

Scientists have discovered an autoimmune disease that seems to affect men with testicular cancer.

Known as 'testicular cancer-associated paraneoplastic encephalitis', it causes severe neurological symptoms in men. They progressively lose control of their limbs, eye movements and in some cases, speech.

The disease begins with a testicular tumour, which appears to cause the immune system attacks the brain. Affected men often find themselves misdiagnosed or undiagnosed and appropriate treatment is delayed.

The study identified a highly specific and unique biomarker for the disease by using a variation of "programmable phage display" technology. Their refined version of this technology simultaneously screens more than 700,000 auto-antibody targets across all human proteins.

Using this powerful tool, the researchers evaluated cerebrospinal fluid from a 37-year-old man who had a history of testicular cancer and debilitating neurological symptoms, including vertigo, imbalance, and slurred speech. The enhanced phage technology identified auto-antibodies targeting Kelch-like protein 11 (KLHL11), which is found in the testes and parts of the brain.

These results were correlated and validated with additional patient samples from a clinic. In addition to identifying the cause of this mysterious neurological disease, the results point the way to using this protein biomarker as a diagnostic test for men with testicular cancer-associated paraneoplastic encephalitis.

Obesity causes early onset of puberty

Obese boys and girls are likely to enter puberty at an early age, which can result in stunted growth or depression, finds a new study.

The study showed total body obesity and excess belly fat in boys aged four-seven years were associated with greater odds of starting puberty before age nine. 

“With the increase in childhood obesity worldwide, there has been an advance in the age at which puberty begins in girls. But in boys the evidence has been controversial,” said lead researcher.

For the study, the researchers included 527 Chilean boys. 

Among boys aged five or six, those with obesity had nearly 2.7 times the odds of starting puberty early. Whereas those with excess belly fat had almost 6.4 higher odds of puberty before age nine, said a researcher.

She said excess belly fat more closely related to fat mass, because a higher BMI may reflected increased muscle, especially in athletes.

Precocious or early puberty has been linked to possible problems, including stunted growth, emotional-social problems like increased risks of depression and substance abuse. 

In addition, in boys, it could lead to higher incidence of testicular cancer in adulthood, she said.

Controlling the obesity epidemic in children could be useful in decreasing these risks, she noted.

Five Lesser Known Cancers That Every Man Should Be Aware About

The five widely ignored cancers among men are prostate, bladder, kidney, testicular and penis (penile) cancer. It is therefore important to be aware of the causes, symptoms and treatment for each of these for timely detection and treatment.
While prostate cancer is the most commonly spoken about, the others are equally significant.

Prostate Cancer

Prostate cancer occurs in the gland cells of the prostate, the gland surrounding the neck of the bladder. According to doctors, prostate cancer is the second leading cancer among males in India with the highest incidence among those who are more than 50 years of age.
Prostate cancer does not have significant symptoms initially but in the advanced stages, it can lead to obstruction in passage of urine and/or show blood in urine.
Common treatment methods include Radical Prostatectomy, Radical Radiotherapy, Hormonal Treatment or Chemotherapy.
Prostate cancer can be prevented by adopting lifestyle changes such as regular exercise, avoiding consumption of red meat, quitting smoking and consumption of alcohol and maintaining a healthy body weight.

Bladder Cancer

Bladder cancer occurs due to abnormal growth of the urinary bladder. While bladder cancer is primarily known to affect the elderly, there is a rising incidence among the younger population. Smoking is responsible for up to 66% of bladder cancers.
The most common symptoms are blood in urine, dark coloured urine, frequent urination or pain during urination. Please avoid smoking, drink enough fluids and eat a well-balanced diet.

Renal Cancer

Kidney cancer is a condition in which the kidney cells become malignant and form tumours.

While this disease has primarily affected the elderly over decades, there is a growing incidence among younger people.
Initial symptoms are blood in urine, persistent nagging pain or heaviness on the side of the back, unexplained fever, loss of appetite, anaemia, unexplained weight loss and swelling in ankles or legs.
Renal cancer can be prevented by eating a healthy diet, keeping blood pressure levels in control, exercising regularly and quitting smoking and consumption of alcohol.

Testicular Cancer

Testicular cancer is cancer of testicles, which are male reproductive organs. Testicular tumours are most common in young men aged between 15-34 years. The warning signs for this cancer include painless enlargement of the testes, heaviness of the scrotal sac and lumps in the testicles.
Testicular cancer is best treated through a multi-modality therapy including chemotherapy, surgery and radiotherapy.

Penile Cancer

Penile cancers develop in the penis. This is one of the few cancers, which can be prevented by maintaining good hygiene especially in the glans penis (bell end of the penis).
The incidence of penile cancer is more in developing countries such as India and it is more common in rural areas as compared to the urban population.
The cancer starts spreading from the foreskin or the glans penis. The early signs include red discoloration of the glans or foreskin, presence of painless or painful ulcers or any kind of abnormal growth.
