Vasectomy Reversal Outcomes Vary Widely Between Surgeons

OUTCOMES following vasectomy reversal vary considerably between surgeons, according to a large multi-institutional study that suggests 1-year semen analysis may provide the most reliable measure of procedural success.

Researchers analysed data from 4,568 men undergoing vasectomy reversal for fertility purposes across eight centres and 12 surgeons. The findings revealed substantial differences in postoperative patency and sperm concentrations, despite relatively similar preoperative patient characteristics.

Wide Variation in 1-Year Outcomes

The study combined prospectively maintained datasets with retrospective chart reviews, examining preoperative, operative, and postoperative factors. Primary outcomes were patency and sperm concentration 1 year after reversal.

Of the 4,568 patients included, 2,398 underwent at least two semen analyses, while 1,222 had at least one assessment more than 1 year after surgery.

At 1 year, patency rates ranged substantially between surgeons, from 54% to 93%, with a median across surgeons of 82%. Median sperm concentrations also varied markedly, ranging from 0–18.5 million/mL, with a median of 10.3 million/mL.

Researchers reported statistically significant differences between surgeons for almost every operative and postoperative factor examined. Rates of epididymovasostomy, a more complex form of vasectomy reversal, increased in line with the length of time since vasectomy.

Sperm Measures Decline Over Time

Overall, sperm patency and concentrations tended to decline over time following reversal, although the pattern differed considerably between surgeons.

Unexpectedly, longer time since vasectomy appeared to be associated with better long-term patency in some analyses. However, the authors cautioned against overinterpreting this observation because of the retrospective nature of much of the data.

Patients who underwent semen testing more than 1 year after surgery also had poorer outcomes than those without later testing. According to the researchers, this could introduce selection bias, as patients experiencing poorer initial results may be more likely to continue undergoing semen analyses. Consequently, currently reported long-term outcomes may underestimate true success rates.

Defining Success After Vasectomy Reversal

Importantly, semen testing at 1 year appeared to distinguish outcomes between surgeons more effectively than testing at earlier time points. Results were similar regardless of whether patency was defined using sperm concentration thresholds of greater than 0, at least 1 million/mL, or at least 5 million/mL.

The researchers therefore suggested that 1-year sperm outcomes may represent a preferable standard for defining vasectomy reversal success.

The findings highlight both the variability in outcomes following vasectomy reversal and the importance of longer-term follow-up when evaluating surgical success.

Researchers Observe Dementia Reversal In Mice

Dementia patients have renewed hope of being cured of their ailment following a successful study in which researchers managed to reverse dementia in mice.
 
The study, which was conducted recently, represents the very first time that tau pathology reversal was documented in an animal model.

Tauopathy is a term used to refer to a specific type of neurodegenerative disease, which results in the second-most significant lesion that develops in Alzheimer’s sufferers’ brains. Tauopathy presents itself when proteins get tangled after dissociating from microtubules and form aggregates of protein on the brain that are insoluble.

"We show that we can intervene after the disease is established and pharmacologically rescue mice that have tau-induced memory deficits," said senior researcher. 
 
The breakthrough came through the observation of leukotrienes, which are inflammatory molecules that play a significant role in dementia, with particular reference to its latter stages of development.
"At the onset of dementia, leukotrienes attempt to protect nerve cells, but over the long term, they cause damage," the researcher explained. "Having discovered this, we wanted to know whether blocking leukotrienes could reverse the damage, whether we could do something to fix memory and learning impairments in mice having already abundant tau pathology."
It must be said that testing this particular hypothesis was no mean feat – the team used mice that were specifically engineered with the tau pathology as they aged. The researchers waited until the animals were 12 months old, or 60 years old in human years before they treated them with zileuton, a leukotriene inhibitor.
 
The mice used in the study were administered zileuton treatment over the course of 16 weeks, then put through a variety of mazes to test their memory. Zileuton-treated mice were seen to fare much better than those that were administered none. As a result, researchers believe that zileuton treatment can actually reverse memory deficiencies in mice.

Further analysis determined that mice treated with zileuton experienced a 90% reduction in leukotrienes compared to the mice that were administered none. What’s more is that the treated mice had a 50% lower concentration of insoluble tau – known to damage brain synapses – than their untreated counterparts.
"The inflammation was completely gone from tau mice treated with the drug. The therapy shut down inflammatory processes in the brain, allowing the tau damage to be reversed," the researcher said.

"Leukotrienes are in the lungs and the brain, but we now know that in addition to their functional role in asthma, they also have a functional role in dementia. This is an old drug for a new disease. The research could soon be translated to the clinic, to human patients with Alzheimer's disease,” he added.

A fasting diet may help to reverse diabetes

A type of fasting diet may reprogramme pancreas cells, promote the growth of new insulin-producing pancreatic cells and reduce symptoms of Type 1 and Type 2 diabetes, a study has showed.

In the study, led by researchers from the University of Southern California, mice were placed on fasting mimicking diet (FMD) for four days each week which showed remarkable reversal of diabetes.

The mice regained healthy insulin production, reduced insulin resistance and demonstrated more stable levels of blood glucose -- even in the later stages of the disease, the researchers said in the paper published in the journal Cell.

The genes normally active in the developing pancreas of embryonic/foetal mice are reactivated in diabetic adult mice when cycling FMD with normal diets.

This increases production of the protein neurogenin-3 (Ngn3) and, as a result, promotes the creation of new, healthy insulin-producing beta cells.

Researchers also examined pancreatic cell cultures from human donors and found that, in cells from Type 1 diabetes patients, nutrients mimicking fasting also increased expression of the Ngn3 protein and insulin production.

"These findings warrant a larger FDA trial on the use of the Fasting Mimicking Diet to treat diabetes patients," said Valter Longo from the University of Southern California. 

"People with diabetes could one day be treated with an FDA-approved Fasting Mimicking Diet for a few days each month, eat a normal diet for the rest of the month, and see positive results in their ability to control their blood sugar by producing normal levels of insulin and improving insulin function," Longo added.

Researchers have shown that losing less than one gram of that fat through weight loss reverses diabetes.

"For people with Type-2 diabetes, losing weight allows them to drain excess fat out of the pancreas and allows function to return to normal," said lead researcher Roy Taylor from Newcastle University in England.

"So if you ask how much weight you need to lose to make your diabetes go away, the answer is one gram! But that gram needs to be fat from the pancreas. At present the

only way we have to achieve this is by calorie restriction by any means -- whether by diet or an operation," Taylor noted.

In a trial, 18 people with Type-2 diabetes and nine people who did not have diabetes were measured for weight, fat levels in the pancreas and insulin response before and after bariatric surgery.

The patients with Type-2 diabetes had been diagnosed for an average of 6.9 years.

The people with Type-2 diabetes were found to have increased levels of fat in the pancreas.

The participants in the study had all been selected to have gastric bypass surgery for obesity and were measured before the operation then again eight weeks later.

After the operation, those with Type-2 diabetes were immediately taken off their medication.

Both groups lost the same amount of weight, around 13 percent of their initial body weight.

Critically, the pool of fat in the pancreas did not change in the non-diabetics but decreased to a normal level in those with Type-2 diabetes.

This shows that the excess fat in the diabetic pancreas is specific to Type-2 diabetes and important in preventing insulin being made as normal.

When that excess fat is removed, insulin secretion increases to normal levels. In other words, they were diabetes free, the researchers said.

The study was published online in the
journal Diabetes Care.
