Poll reveals 64% Indians don’t sleep enough; here’s what doctors want you to know about sleep issues

If there was one thing that we took for granted, it was sleeping. A decade ago you would not hear about sleep problems like you do today. If we slept late one day, we would make up for it the next. But what if the sleep eludes you and you are left staring at your ceiling waiting for it to set in? Over the past few years, we have been made aware that lack of sleep is linked to weight gain, is an indicator of an underlying condition and in extreme cases can lead to dangerous consequences. 


ETimes conducted a sleep poll where it was revealed that 64% of the participants feel that they do not sleep enough. Up to 51% people shared that they sleep for 4-6 hours only and 10% clock less than 4 hours of sleep.


Sleep issues are highly under-reported

Dr. Sanjiv Badhwar is Consultant and Head, ENT services at Kokilaben Dhirubhai Ambani Hospital shares sleep disorders have been largely under reported in our country but we have noticed an increase in the number of patients coming forward over the past 1 year. We are seeing the whole spectrum of sleep disorders, be it sleep deprivation, sleep fragmentation, difficulty in sleep initiation and sleep Apnea. We are also seeing a number of children presenting with sleep Apnea. The overall incidence of sleep Apnea in our country for adults is around 15% and in children around 3.4 to 5%.

How to identify a sleep disorder

But how do you know you have a sleep disorder? Dr Badhwar adds, “A sleep disorder is diagnosed by a careful history, the use of some basic questionnaires, namely, the stop-bang questionnaire and the Epworth sleepiness scale and clinical examination. After this the patient undergoes relevant investigations for the same which may include some basic blood tests, x-rays and scans. The mainstay of diagnosis is the sleep study (polysomnography) which essentially is a test which tells us about the quality of sleep, the number of times an individual’s breathing stops during sleep, vital parameters and the oxygen saturation during sleep. This study gives us an indication if the patient has sleep apnea or not and if so - is it mild, moderate or severe. An adjunctive test is the sleep endoscopy where the exact level of obstruction during sleep is ascertained.”

There are some signs that one must not ignore when it comes to sleep. For example, if you have excessive daytime sleepiness, snoring, disturbed sleep, gasping for air during sleep, loud snoring during sleep, extreme fatigue during the day, early morning headaches, irritability, mood swings, dry mouth, forgetfulness and recent onset of cardio vascular events, you may have sleep apnea and need to consult your doctor.

It needs to be noted that the long term effects of sleep deprivation are real. There is a drain on mental ability and the individual's physical health is at risk. There can be memory issues, mood changes, trouble with thinking and concentration, accidents during driving, weakened immunity, risk for diabetes, risk of heart disease, high BP and at times a low sex drive, the doctor added.

How to differentiate between regular snoring and sleep apnoea

Snoring is loud noise whilst breathing during sleep. This may or may not be sleep apnoea. For a lay person the most obvious way to differentiate is to identify how one feels during the day. Normal snoring does not interfere with the quality of sleep, so you are less likely to experience fatigue, irritability and sleepiness during the day. However the only way to be sure is a careful clinical examination and a sleep study.

COVID-19: Government issues revised guidelines for discharge of patients

Following the easing of restrictions of the nation-wide COVID-19 lockdown in India, there have been a sharp and alarming spike in positive cases. Considering the fact that there is no cure or vaccine for this disease as yet, we may have to live with it for some time to come. So, we need to fall back on preventive measures and social distancing rules. The government is also doing its best to contain the spread of the disease. But COVID-19 has a way of defying all efforts to subdue it. In light of all these developments, the government of India has come out with revised guideline for the discharge of patients from health care facilities across the country. 

REVISED DISCHARGE POLICY FOR COVID-19
These guidelines are different from the previous rules. According to the earlier guidelines, a positive case had to be tested on day 14 once and then again in a span of 24 hours. If both the results are negative, only then is the patient fit to be discharged from the health care facility.


For mild/very mild/pre-symptomatic cases

Such cases admitted to a COVID Care Facility will undergo regular temperature and pulse oximetry monitoring. The patient can be discharged after 10 days of onset of symtoms and provided that he ahs no fever for 3 days. There will be no need for testing prior to discharge. At the time of discharge, the patient will be advised to follow home isolation for further 7 days. If at any point of time, prior to discharge, the patient’s oxygen saturation dips below 95 per cent, he or she will be moved to a dedicated COVID health centre. After discharge from the facility, if the patient develops any symptoms like fever, cough or breathing difficulty, he will have to contact the COVID care centre or state helpline or 1075. Healthcare professionals will then follow the patient’s health via tele-conference on 14th day. 


For moderate cases

This includes patients whose symptoms resolve within 3 days and who maintains oxygen saturation above 95 per cent for the next 4 days. Such patients will undergo monitoring of body temperature and oxygen saturation. If the fever goes down within 3 days and the patient maintains saturation above 95 per cent for the next 4 days without oxygen support, he or she will be discharged after 10 days of symptom onset in case of absence of fever without antipyretics, resolution of breathlessness and no oxygen requirement. There will be no need for testing prior to discharge. At the time of discharge, the patient will be advised to follow the home isolation for 7 days.

On the other hand, patients on oxygenation whose fever does not resolve within 3 days and whose demand of oxygen therapy continues will be discharged only after resolution of clinical symptoms and when they display the ability to maintain oxygen saturation for 3 consecutive days.


For severe cases including immunocompromised (HIV patients, transplant recipients, malignancy)

Discharge criteria for severe cases will be based on clinical recovery and only if the patient tests negative once by RT-PCR after resolution of all symptoms.

Researchers develop algorithm for India's first Health monitoring wearable device

Researchers have developed the algorithm for the country's first medical-grade, wearable device with algorithms for deriving vitals in a non-invasive manner, called "VinCense."Developed jointly, which works on mobile health technology, VinCense could be used for unobtrusive monitoring. Vital signs including, pulse rate, oxygen saturation, respiratory rate, skin temperature, blood pressure and others were duly recorded through this device. ''The real-time data are stored in the patent-pending secured cloud from where healthcare providers can access vitals using an intuitive interface'', the researchers on Monday said.

VinCense could be deployed for Home Healthcare delivery, Insurance/Analytics and Corporate/Industrial Occupational Health Screening. It also has applications for Non-Communicable Diseases Screening and Surveillance, general Public health, and Community Health.
