Menstrual cycle influences sleep quality in women

Young women are more likely to experience sleep disruption in the days leading up to their menstrual period, a study has found.

" Sleep is  more disrupted in the several days directly prior to menses in young healthy women," said a researcher.

" Increased sleep disruption was found in the late luteal phase, which corresponds with the days directly prior to menses," said the researcher.

Menstrual phase affected sleep efficiency, wake after sleep onset (WASO), number of awakening per night, and sleep fragmentation index, in keeping with increased sleep disruption in the late luteal phase.

Compared with the early follicular phase, sleep efficiency decreased by 3.3 %, WASO increased by 15 minutes, and the number of awakenings per night increased by 3 in the late luteal phase.

Researchers collected daily sleep data from 10 healthy women between the ages of 18-28 who had regular menstrual cycles. The researchers tracked the women\s sleep during 2 of their cycles.

The women wore actigraphic sensors on their wrist to record patterns of activity and rest over 578 sleep episodes and they provided morning urine samples for measurement of concentration of luteinizing hormone (LH), estrone-3-glucuronide (EIG) andpregnanediol-3-glucuronide (PDG).
All participants ovulated in both cycles.

The women also completed 5 day diets during the early follicular phases of each cycle. The diet during one cycle contained neutral energy availability, and the diet during the other cycle contained 55% fewer calories.

Menstrual cycle lengths were standardized to 14 day follicular and 14 day luteal phases, centered on the day of ovulation.

" Short term caloric restriction had negative effects on sleep in both the late follicular phase, just before ovulation, and in the late luteal phase, just before the onset of menses, " said the researcher who performed this study.

Decreased energy availability increased sleep disruption, with less sleep efficiency, greater WASO and a higher sleep fragmentation index in the late follicular phase in addition to the effects noted above in the late luteal phase.

It is likely these effects are mediated by the dynamic changes in ovarian hormones across the menstrual cycle.

Their study found that EIG was linked with more awakenings, and PDG was linked with a trend toward higher sleep fragmentation index.

The study validates perceptions using objective measures, and further documents the negative impact of dieting on sleep.

" These findings suggest that women need to be particularly cognisant pf practising good sleep hygiene in the week before menses and with decreased caloric intake," she noted.

Why do women develop heart disease later than men?

Investigating why heart disease generally develops later in women than men, researchers have found that the interplay between female ovarian hormones and a circadian “clock” molecule protects the heart health of women as they age. The findings could lead to new treatment to help prevent heart disease as we grow older. “This is the first study to demonstrate a link among female ovarian hormones, the circadian system which regulates the body’s day-night cycle, and the observation that women enjoy significant protection against heart disease when compared to men,” said a Professor. In earlier studies, Prof.’s team found that heart attacks were worse for males than for females of similar age. The research also uncovered a time-of-day effect: heart attacks in males were more severe during sleep.

This led to the idea that the circadian mechanism – tiny clocks in all of our body’s cells which regulate our 24-hour day and night processes – might work differently in male and female hearts. To test that possibility, the researchers studied old mice with a genetic “clock” mutation that desynchronises the circadian mechanism. The team discovered that female heart cells are actually different from those of males. The males had worse cardiac glucose and energy profiles. By contrast, female hearts had a healthy cardiolipin profile and better energy.

However, the advantage for females was lost when the ovaries were removed, a clear sign that hormones such as estrogen protect the heart even when the circadian mechanism is disturbed, Prof. said. The findings may lead to clinical benefits for women and men, she added.
