Why noise pollution is bad for your heart

In 2011, Germany's Frankfurt Airport – the country's busiest – unveiled its fourth runway. The addition sparked major protests, with demonstrators returning to the airport every Monday for years. "It's destroying my life," one protester told Reuters a year later. "Every time I go into my garden, all I can hear and see are planes right above."

The new runway also channeled dozens of aircraft directly over the house of Thomas Münzel, a cardiologist at the University Medical Center of Mainz. "I have lived close to the German Autobahn and close to inner city train tracks," he says. "Aircraft noise is the most annoying by far." Münzel had read a 2009 World Health Organization (WHO) report linking noise to heart problems, but evidence at the time was thin. Driven in part by concern for his own health, in 2011 he shifted the focus of his research to learn more.

Exposure to loud noise has long been linked with hearing loss. But the ruckus of planes and cars takes a toll beyond the ears. Traffic noise has been flagged as a major physiological stressor, second to air pollution and on roughly equal footing with exposure to second-hand smoke and radon.

In the last decade, a growing body of research has linked noise from aircraft and road traffic to a heightened risk for a number of cardiovascular ailments. And scientists are also beginning to pinpoint the mechanisms at play.

Estimates suggest that roughly a third of people in Europe and the US are regularly exposed to unhealthy levels of noise, typically defined as starting around 70 to 80 decibels. For comparison, normal conversation is typically about 60 dB, cars and trucks range around 70 to 90 dB and sirens and airplanes can reach 120 dB or more.

Numerous studies link chronic exposure to such environmental noise to an increased risk of heart-related troubles. People living near the Frankfurt Airport, for example, have as much as a 7% higher risk of stroke than those living in similar but quieter neighbourhoods, according to a 2018 study that investigated health data of more than one million people. An analysis of nearly 25,000 cardiovascular deaths between 2000 and 2015 among people living near Switzerland's Zurich Airport saw significant increases in night time mortality after airplane flyovers, especially among women, a team reported recently in the European Heart Journal.

As researchers probe the physiology underlying noise's cardiovascular consequences, they are zeroing in on a culprit: dramatic changes to the endothelium, the inner lining of arteries and blood vessels. This lining can go from a healthy state to one that's "activated", and inflamed, with potentially serious ramifications.

The path from noise to blood vessels goes something like this: when sound reaches the brain, it activates two important regions – the auditory cortex, which interprets noise, and the amygdala, which manages emotional responses to it. As noise gets louder, and especially during sleep, the amygdala activates the body's flight-or-fight response – even if the person isn't aware of it.

Once initiated, this stress response releases hormones such as adrenaline and cortisol into the body. Some arteries constrict, others dilate, blood pressure rises, digestion slows while sugars and fats flood the bloodstream for quick use by the muscles.

The cascading stress response also prompts the creation of harmful molecules that cause oxidative stress and inflammation in the lining of blood vessels. This dysfunctional endothelium meddles with blood flow and affects numerous other processes that, when impaired, contribute to a range of cardiovascular illnesses, including high blood pressure, plaque build-up in the arteries, obesity and diabetes.

Studies on people and mice show that the endothelium doesn't work as well after just a few days of night time airplane noise exposure, suggesting that loud noise isn't a concern only for people already at risk for heart and metabolic problems. Healthy adults subjected to train recordings as they slumbered had impaired blood vessel function almost immediately, according to a 2019 study by Münzel and his colleagues.

"We were surprised that young people, after hearing these sounds for just one night, had endothelial dysfunction," says Münzel, who also co-authored a review of noise and cardiovascular health. "We always thought this was something that takes years to develop."

While the data continues to accumulate, untangling cause and effect can be tricky. It's not easy to conduct long-term sleep experiments or to distinguish between the effects of daytime and night time noise, or effects of the noise itself versus the combined effects of noise and air pollution (which often go hand-in-hand).

The consequences of environmental noise are also tough to parse due to the subjective nature of sound, says Andreas Xyrichis, a health services scientist at King's College in London. Xyrichis studies hospital intensive care units, where ringing telephones and clattering food dishes can be comforting or counteractive to recovery, depending on the patient. "We are really trying to make this distinction between decibel levels and perception of noise," he says. 

But despite the remaining questions, there is a growing recognition of the connections between noise pollution and reduced physical health. A 2018 report by the WHO noted that each year, western Europeans are collectively losing more than 1.6 million years of healthy life because of traffic noise. This calculation is based on the number of premature deaths caused directly by noise exposure as well as the years lived with noise-induced disability or illness.

And that number is likely to grow. In 2018, 55%of people lived in cities, and by 2050 that count is expected to reach nearly 70%, the United Nations estimates.

Some governments, heeding public protests, have tried to quiet the clamour of urbanisation by adopting night time flight bans, incentivising quieter technologies and issuing fines for noise complaints. Individuals can help themselves by ensuring that bedrooms are as quiet as possible by retrofitting windows or hanging noise-reducing curtains or, if they can afford it, moving to quieter neighbourhoods.

Cheaper solutions may be wearing earplugs at night or moving bedrooms to a quieter part of the house, according to Mathias Basner, a psychiatrist and epidemiologist at the University of Pennsylvania and president of the International Commission on the Biological Effects of Noise. He believes people should take such steps even if they don't find themselves especially fazed by noise.

"If you're living in Manhattan, you won't notice how loud it is after a while because it is normal," he says. "But if you have habituated to it psychologically, that doesn't mean it doesn't have negative health consequences."

10 Simple Steps to Help Prevent Hearing Loss

Hearing loss is becoming an increasing problem among young people and the Baby Boomer generation. Although hearing damage is irreversible, the problem is preventable with a healthy dose of knowledge.

Hearing loss is caused by exposure to sounds that are too loud or continued exposure to loud noises for extended periods. In the past hearing loss was largely due to working conditions with noisy equipment. Protective gear and safety laws have made that a thing of the past. Hearing loss is now mainly caused by exposure to overly loud noises in our leisure pursuits and home environments.

The thing with hearing loss is that we don’t realize we’re losing our hearing immediately. It’s gradual and incremental. The sounds we hear don’t have to be unpleasant or physically painful. Things like loud television, radio, traffic, neighborhood construction, and appliances all contribute to a noisy environment, and you can accumulate damage without even realizing it. 
 
It’s also important to remember that the louder the noise, the less time you should listen to it without protection. We measure sounds in decibels (db). The higher the decibel number is, the louder the noise. Experts agree that any sound below 75 db isn’t likely to cause damage, but any regular prolonged exposure to sounds above 85 db will contribute to hearing damage and eventual loss.

Tips to Prevent Damaging Your Hearing 
 
1. Have a noise detox
If you are exposed to loud noise unintentionally, go on a hearing detox afterwards. You need time to let your ears recover. Give yourself a sound rest of 16 hours or more to recuperate, with at least 16 hours if you have spent 2 hours hearing 100 db. 
 
 
2. Distance: Don’t stand so close
If you are exposed to a loud noise try to keep your distance from it. The closer you are to a loud sound physically, the more damaging it is. Try standing at an angle from the noise, and don’t be directly in front of the source of the sound. 
 
3. Beware snorers

If you sleep next to a snorer, consider sleeping with ear plugs because loud snoring can reach 80 - 90db. 
 
 
4. Regular quiet periods

Try limiting your time spent in noisy venues. If you are in a noisy location for a long time, like a conference, make sure to go outside regularly and give your ears a break from the noise. 
 
 
5. The 60:60 rule

If you listen to music via headphones, avoid trying to drown out any outside noises with volume and instead consider investing in noise-cancelling headphones. Apply the 60/60 rule: set the volume at a maximum of 60% and listen for no more than 60 minutes a day. 
 
 
6. Take care in the car
Music in the car is an excellent way to relieve the stress or boredom of a long commute but beware of the dangers. Listening to loud music in a confined space has more of a risk of damaging your hearing than listening in a more open space. 
 
 
7. Recognize harmful sound levels
Understand sound levels in your environment. Once you have learnt what is damaging, you can practice healthy hearing habits. There are even smartphone apps that measure sound for you. Many young people engage in unhealthy hearing behavior. Be proactive in educating them so they too can have lifelong good hearing. 
 
 
8. Wear protective gear
If you have to hear loud sounds (working at a music event or doing home repairs with noisy equipment), then you should get protective gear. Cotton wool in the ears will not suffice. There are many varieties of earplugs and ear muffs available on the market. You can get wax or foam plugs, or custom made molds. 
 
 
9. Give your ears a rest

Allow your ears regular breaks from over exposure to noise. Peaceful days out of the city are recommended if you live in a loud urban area.  
 
 
10. Test your hearing
Schedule an appointment with a professional to learn your baseline hearing rate, for future reference. If you are diagnosed with any damage or loss, consider a hearing aid to slow down the process and seek support of professionals to establish good hearing practices.

Age Is a Significant Factor in These 13 Common Conditions

Now I’m getting on a bit in years, I’ve started to become concerned about my health in a way I never used to be. I always think I’ve got some condition or other, because I know that some diseases occur more frequently in people as they get older. But which ones should I watch out for? According to gerontologist and professor of medicine at Calgary University, David Hogan, the 13 most common diseases where age is a significant risk factor are: 
 
1. Cardiovascular Disease
One of the leading causes of death throughout the world, cardiovascular disease is, in fact, the US’s number one killer. Coronary artery disease is the most commonly occurring form of the condition – this is a narrowing or blockage of the heart’s main blood supplying arteries. Such obstructions and blockages can occur suddenly (an acute rupture) or gradually develop over time, causing heart attacks that may be fatal.


2. Cerebrovascular Disease (Strokes)

When a blood vessel in the brain is disrupted and blood cannot flow to one part of the brain, a stroke occurs. Because brain cells deprived of oxygen from blood quickly die, strokes are extremely serious.There are two different types of stroke: ischemic – when a blood vessel is blocked by a blood clot; and hemorrhagic – when a bleed in the brain is caused by a ruptured blood vessel. 
 
A stroke can lead to serious disability or even death, according to the blockage/rupture’s area and severity.

3. High Blood Pressure (Hypertension)
While the heart is pumping, the force the blood exerts on your artery walls can be measured, this is known as your blood pressure. When you sleep or rest, your blood pressure count is low. When you are excited or under stress, your count is high. Furthermore, your blood pressure count usually rises with age. If someone’s blood pressure is chronically elevated, they can face serious health problems in their blood vessels, kidneys, heart, and elsewhere.


4. Cancer
Cancer – when abnormal cells grow at an uncontrollable rate – is said to be more likely to occur at a later stage in life. The American Cancer Society has found that 77% of cancers are diagnosed in patients aged above 55. In Canada, cancer is the number one cause of death for both males and females. 
 
The most common cancer types for seniors include the following: stomach cancer, non-Hodgkin’s lymphoma, bladder, prostate, colorectal, lung, skin, and breast cancer.

 
5. Type-2 Diabetes
Diabetes is the name for the disruption in the way your body makes use of the glucose and sugars digested from food. Type-1 diabetes, which usually first occurs in people aged below 30, is when no insulin is produced. Type-2 diabetes, which is much more common, is when some insulin is produced but the body is resistant to it – meaning the body can’t properly process glucose. 
 
Diabetes causes sugar levels to rise dangerously high, which can lead to health problems, including heart attacks, strokes, kidney failure, blindness, and nerve damage. 
 
Type-2 diabetes is becoming more and more common, as modern changes alter the way we eat and lead our lives. This can be arrested though by leading a healthier lifestyle with plenty of regular exercise, and a good well-balanced diet.
 
 
6. Parkinson's Disease
Parkinson’s is a progressive neurological disorder characterized by halting movement, stiffness, and visible tremors. Unsurprisingly, around 75% of all Parkinson’s cases begin after the patient has reached 60 years of age. Yet age is still considered just one of the risk factors. Other reasons why people might be more likely to contract the condition include, whether they’ve been exposed to chemical toxins, suffered head injuries, or have a family history of the disease. Men are also more likely than women to suffer from Parkinson’s disease.


7. Dementia (including Alzheimer's Disease)

Dementia is characterized by a loss of brain function, which may manifest as memory loss, mood changes, poor judgement, confusion, and difficulty communicating. The most common cause of such dementia is Alzheimer’s disease, though other causes include, Huntington's disease, Parkinson’s disease, and vascular dementia. Even though dementia is more likely to occur during old age, it is in no way seen as part of the natural aging process.
 
 8. Chronic Obstructive Pulmonary Disease (COPD)
Though chronic obstructive pulmonary disease (COPD) is incurable, it is treatable and preventable. The condition occurs when inflammation in the airways, a thickening of the lungs’ lining, and an overproduction of mucus in the air tubes, leads to a significant reduction of airflow into and out of the lungs. Common symptoms of COPD include shortness of breath, wheezing, and a chronic cough. 
 
The disease is usually caused by prolonged exposure to airborne irritants such as tobacco smoke (first and second hand), work-related contaminants, or industrial pollution. The single biggest risk factor in contracting the disease is cigarette smoking. 
 
 
9. Osteoarthritis

Osteoarthritis – a degenerative joint disease – is the most common type of arthritis. Though the condition occurs more frequently in old age, women are much more likely to suffer than men. Those who’ve suffered a prior injury or are obese are also more susceptible to osteoarthritis. 
 
The swelling and joint pains that characterize osteoarthritis can be treated with pain relief and anti-inflammatories, while lifestyle changes such as weight loss, physiotherapy, and exercise (see our yoga guide here) can also make a difference. However, osteoarthritis is still incurable.


10. Osteoporosis
Popularly referred to as brittle bone disease, osteoporosis is characterized by a loss in bone mass, resulting in weaker and thinner bones. The condition becomes more likely because of age, particularly for white and Asian females. Those with osteopenia – low bone density – are also at risk. 
 
The U.S. National Institutes of Health claim that up to half of all women aged over 50 and nearly a quarter of men of the same age break a bone due to osteoporosis. Hip fractures are of most concern to elderly adults, because they lead to a loss of mobility and independence. Around 25% of hip fractures lead to death within 12 months. 
 
To help prevent osteoporosis, try to maintain a diet rich in vitamin D and calcium. Also, refrain from smoking and second hand smoke.

 
11. Cataracts

A cataract is a gradual cloudiness in your eye’s lens, which may result from several causes, including smoking, diabetes, and exposure to ultraviolet light. The U.S. National Institutes of Health assert that more than half of all over 65s have a cataract of some kind. Though in its initial phase a cataract may not be noticeable, over time vision can become blurry and will worsen. Nowadays, surgery to remove and replace the lens can be performed as an outpatient procedure, taking around one hour only. 
 
 
12. Age-Related Macular Degeneration (AMD)

Age-related macular degeneration typically occurs in adults over the age of 50, and is thought to be the most common cause of blindness in the elderly. While peripheral vision is preserved, the deterioration of the eye’s macular stops the patient from seeing well in the center of their field of vision. Aside for age, other risk factors are smoking, family history, and race (white people are more likely to suffer than black, for example). Though no firm conclusions have yet been reached, researchers are confident that regular exercise and eating a diet rich in fish and colorful vegetables, as well as abstaining from cigarettes and keeping blood pressure and cholesterol levels down, can help prevent AMD.



13. Hearing loss

As you age, you can expect a noticeable degeneration in your hearing ability, with the deterioration of the miniscule hairs inside your ear which aid in processing sounds. Because of these changes, you may find that you can’t follow conversations in noisy areas, are unable to distinguish between some consonants, find some sounds louder than normal, and hear muffled voices. Factors that lead to hearing loss, aside from age, include prolonged exposure to loud sounds, smoking, and family history. An estimated half of all over 70s experience a degree of age-related hearing loss. 
 
Thinking About Age-Related Diseases 
 Aging is of course not a disease, but it is a risk factor for many different conditions. There is no guarantee that you will suffer from an age-related disease, but the chances of doing so increase with age. The rate of decline in adults over time can be accelerated because of many differing factors, including inflammation, exposure to pollutants and radiation, smoking, a poor diet, low fitness levels, and simple wear and tear.

Seniors, Warning! Study Finds New Dementia Risk Factor

Age is one of the biggest risk factors for dementia. The older we get, the higher our of dementia. But what is it about advanced age that makes us more likely to develop this debilitating disease? 
 
Cardiovascular issues, chronic inflammation, and even hearing loss are all correlated with dementia and are more common in seniors. Japanese scientists have added a new and rather surprising risk factor - sarcopenic obesity - to that list.  
 
Dementia in Japan
The latest statistical estimates suggest that 15% of older adults in Japan (aged 65 and older) suffer from dementia, and the statistics are gradually increasing every year. To compare, the Population Reference Bureau suggests that around 10% of American seniors aged 70 and older suffer from this severe medical issue. For Japan, a country that historically had one of the lowest rates of dementia in the world, this was a wake-up call. 
 
For understandable reasons, Japanese scientists were concerned. Why are the cognitive abilities of more and more seniors withering away before their eyes? 
 
The Study 
To investigate this alarming tendency, researchers at the Juntendo University in Japan involved 1615 Japanese seniors in the 65-84 age range from the Bunkyo Health Study, an ongoing cohort study of over 10 years. Their theory was that the increase in dementia is linked to two other factors - sarcopenia and obesity - both of which have been shown to increase the risk of cognitive issues in past studies. 
 
As Dr. Yoshifumi Tamura, the lead researcher of the study, pointed out to Scitech Daily, “If the association between sarcopenic obesity and dementia is established, appropriate preventive measures can be taken to reduce the occurrence of this condition and the risk of dementia in elderly patients.” 
 
The study was published in the March 2022 issue of the Clinical Nutrition journal.  
 
What is sarcopenic obesity?

Like dementia, obesity has gradually become increasingly common in Japan. In the senior population, this condition often appears hand in hand with sarcopenia - an age-related loss of muscle mass and strength that typically begins after age 40. Together, these two factors create a condition called sarcopenic obesity, where weakness co-occurs with excessive weight. 
 
Sarcopenic obesity is assessed by taking two measures into consideration: handgrip strength to test sarcopenia and body mass index (BMI) to measure obesity. 
 
Having considered these measures, the participants were evaluated based on their sarcopenia and obesity status and divided into four groups: participants with obesity, sarcopenia, sarcopenic obesity, and control subjects with neither obesity nor sarcopenia. The researchers then compared each group’s cognitive and mental functions to see if there’s a connection between sarcopenic obesity and dementia or moderate cognitive impairment (MCI).

The four groups were divided as follows: 
4.7% of the participants had sarcopenic obesity 
14.6% had sarcopenia 
21.2% had obesity 
59.4% had neither obesity nor sarcopenia. 
 
Despite making up the smallest percentage of the study population, the sarcopenic obesity group had the greatest rate of dementia and MCI. Sarcopenia was the second most significant risk factor, followed by obesity. The study also pointed out sarcopenia is a significant risk factor for dementia in females but not males. As expected, the controls came last, meaning they had the lowest prevalence of MCI and dementia. 
 
The Takeaway 
To sum up, the results of the study are pretty clear-cut: sarcopenic obesity can increase one’s risk of developing dementia and moderate cognitive impairment. As Dr. Tamura stated, “This study clearly demonstrates that sarcopenic obesity, defined by the combination of BMI and hand grip strength is associated with MCI and dementia among Japan’s elderly people.” We’ll be eagerly waiting if studies with a more diverse population confirm these findings too. 
 
Meanwhile, for the average person, the results of this study can be translated into real-world, actionable advice. The research proves that maintaining a healthy weight and exercising to keep muscle strength in your senior years can do a lot more than make you look fit. It means having a clear and healthy mind within a capable and fit body.
