How Artificial Sweeteners Affect Blood Sugar and Insulin

Sugar is a hot topic in nutrition.

Cutting back can improve your health and help you lose weight.

Replacing sugar with artificial sweeteners is one way to do that.

However, some people claim that artificial sweeteners aren't as "metabolically inert" as previously thought.

For example, it's been claimed that they can raise blood sugar and insulin levels.

This article takes a look at the science behind these claims.
What are Artificial Sweeteners?
Artificial sweeteners are synthetic chemicals that stimulate the sweet taste receptors on the tongue. They are often called low-calorie or non-nutritive sweeteners.

Artificial sweeteners give things a sweet taste, without any added calories.

Therefore, they're often added to foods that are then marketed as "health foods" or diet products.

They're found everywhere, from diet soft drinks and desserts, to microwave meals and cakes. You'll even find them in non-food items, such as chewing gum and toothpaste.

Here's a list of the most common artificial sweeteners:

    Aspartame
    Saccharin
    Acesulfame Potassium
    Neotame
    Sucralose

What Causes Blood Sugar and Insulin Levels to Rise?

We have tightly controlled mechanisms to keep our blood sugar levels stable.


Blood sugar levels increase when we eat foods containing carbohydrates.
Potatoes, bread, pasta, cakes and sweets are some foods that are high in carbohydrates.

When digested, carbohydrates are broken down into sugar and absorbed into the bloodstream, leading to an increase in blood sugar levels.

When our blood sugar levels rise, our body releases insulin.

Insulin is a hormone that acts like a key. It allows blood sugar to leave the blood and enter our cells, where it can be used for energy or stored as fat.

But small amounts of insulin are also released before any sugar enters the bloodstream. This response is known as cephalic phase insulin release. It is triggered by the sight, smell, and taste of food, as well as chewing and swallowing.

If blood sugar levels drop too low, our livers release stored sugar to stabilize it. This happens when we fast for prolonged periods, like overnight.


    The sweet taste of artificial sweeteners triggers cephalic phase insulin release, causing a small rise in insulin levels.


    Regular use changes the balance of our gut bacteria. This could make our cells resistant to the insulin we produce, leading to both increased blood sugar and insulin levels.

    Bottom Line:

    Eating carbohydrates causes a rise in blood sugar levels. Insulin is released to bring blood sugar levels back to normal. Some claim that artificial sweeteners may interfere with this process.

Do Artificial Sweeteners Raise Blood Sugar Levels?

Artificial sweeteners won't raise your blood sugar levels in the short-term.

So, a can of diet coke, for example, won't cause a rise in blood sugar.

However, in 2014, Israeli scientists made headlines when they linked artificial sweeteners to changes in gut bacteria.

Mice, when fed artificial sweeteners for 11 weeks, had negative changes in their gut bacteria that caused increased blood sugar levels.


When they implanted the bacteria from these mice into germ-free mice, they also had increases in blood sugar levels.

Interestingly, the scientists were able to reverse the increase in blood sugar levels by changing the gut bacteria back to normal.

However, these results haven't been tested or replicated in humans.

There is only one observational study in humans that has suggested a link between aspartame and changes to gut bacteria .

The long-term effects of artificial sweeteners in humans are therefore unknown.

It is theoretically possible that artificial sweeteners can raise blood sugar levels by negatively affecting gut bacteria, but it hasn't been tested.

    Bottom Line:
    In the short-term, artificial sweeteners won't raise blood sugar levels. However, the long-term effects in humans are unknown.

Do Artificial Sweeteners Raise Insulin Levels?

Studies on artificial sweeteners and insulin levels have shown mixed results.

The effects also vary between different types of artificial sweeteners.


Sucralose

Both animal and human studies have suggested a link between sucralose ingestion and raised insulin levels.

In one study, 17 people were given either sucralose or water and then administered a glucose tolerance test.

Those given sucralose had 20% higher blood insulin levels. They also cleared the insulin from their bodies more slowly.

Scientists believe sucralose causes insulin increase by triggering sweet taste receptors in the mouth — an effect known as cephalic phase insulin release.

For this reason, one study that injected sucralose into the stomach, bypassing the mouth, did not detect any significant rise in insulin levels.


Aspartame

Aspartame is perhaps the most well-known and most controversial artificial sweetener.

However, studies have not linked aspartame with raised insulin levels.
Saccharin

Scientists have investigated whether stimulating the sweet receptors in the mouth with saccharin leads to an increase in insulin levels.

Results are mixed.

One study found that mouth washing with a saccharin solution (without swallowing) caused insulin levels to rise .

Other studies have found no effects.


Acesulfame Potassium

Acesulfame potassium (acesulfame-K) can increase insulin levels in rats.


One study in rats looked at how injecting large amounts of acesulfame-K affected insulin levels. They found a massive increase of 114-210%.

However, the effect of acesulfame-K on insulin levels in humans is unknown.
Summary

The effect of artificial sweeteners on insulin levels seems to be variable, depending on the type of sweetener.

Sucralose appears to increase insulin levels by triggering receptors in the mouth. However, few high-quality human trials exist, and it is currently unclear whether other artificial sweeteners have similar effects.

    Bottom Line:
    Sucralose and saccharin may raise insulin levels in humans, but the results are mixed and some studies find no effects. Acesulfame-K raises insulin in rats, but no human studies are available.

Can You Use Artificial Sweeteners if You have Diabetes?

Diabetics have abnormal blood sugar control due to a lack of insulin and/or insulin resistance.

In the short-term, artificial sweeteners won't raise your blood sugar levels, unlike high intakes of sugar. They are considered safe for diabetics.


However, the health implications of long-term use are still unknown.

    Bottom Line:
    Artificial sweeteners do not raise blood sugar levels, and are considered safe alternatives to sugar for diabetics.

Should You Avoid Artificial Sweeteners?

Artificial sweeteners have been declared safe by regulatory bodies in the US and Europe.

Although artificial sweeteners may not be "healthy," they are at the very least significantly "less bad" than refined sugar.

If you eat them as part of a balanced diet, then there is no strong evidence that you should stop.

However, if you're concerned, then you can use other natural sweeteners instead or just remove sweeteners altogether.

5 Harmful Sugar Substitutes

We all know the harmful effect that excessive sugar consumption has on our weight and our overall health, and therefore many of us try to avoid it as much as possible. However, we still want sweet tasting foods and drinks, and this has led to the invention and development of various artificial substitutes that mimic the effects of sugar.

However, while artificial sweeteners have attracted quite a few fans, research has shown that their effects may be very far from positive. Meet the 5 best known artificial sweeteners, some of which we add to our food ourselves and some of them we meet on labels, which can endanger your health and cause much more harm than good

1. Acesulfame K

This ingredient, marked on food labels as E950, is especially sweet and does not break down when heated or baked. Thanks to these two characteristics, many companies use it in baked goods, soft drinks and soda, jellies, chewing gum and various dairy desserts, as well as hygiene products such as toothpaste and mouthwash. However, this sweet substitute contains methylene chloride, which at high concentrations is considered to be carcinogenic. Long-term exposure to it can cause headaches, depression, and nausea, and in some cases can also impair thyroid function.

2. Aspartame

This artificial sweetener is 180 times as sweet as standard sugar, made up of two amino acids and is used in the paint industry as a solvent. In recent years, this sweetener has become particularly popular, so if you consume diet drinks, cereal or sugarless chewing gum, you’ve likely come across it under the E951 label. Although this substitute is labeled safe for use by the Food and Drug Administration, it is one of the most controversial substitutes and is associated with a variety of side effects and medical problems.

Several animal studies have found that aspartame can cause cancer, and other studies have linked it to an increased risk of brain tumors. Other claims examined in the studies touched on questions about the effects of Aspartame on mental state and headaches, and some found that there was an increased risk of worsening the existing situation in those who had suffered from depression and migraines in the past.

3. Saccharin
Saccharin, approved by the US Food and Drug Administration in 1970, is used by many people as a sweetener for tea or coffee, and food companies add it to jams, candies, sauces, and pastries. However, back in1977, a study was published in which rats who had consumed a large amount of saccharin had an increased incidence of bladder cancer - leading to a warning on products containing the supplement.
Since then, many trials and studies have been conducted to examine the association between saccharin and cancer, and because the findings were inconclusive, the Food and Drug Administration ordered the warning to be removed. However, doctors are still divided on the question of the safety of the supplement to use in general, and by pregnant women in particular, as sucrose passes the placenta and may remain in the body of the fetus.

4. High-fructose corn syrup
The source of this elusive artificial supplement, which you may not have noticed in the ingredients of food you buy, is the sweet and sticky agave plant syrup, which is considered a natural and healthy alternative to sugar because of its low glycemic value. The reaction of the commercial companies to the popularity of agave syrup, which contains fructose, was to take the fructose (fruit sugar) and combine it with harmful corn syrup. The result is many sweet foods such as ice creams, yogurts, and various snacks, which contain excessive amounts of sugar that can be harmful to your health.
When large amounts of fructose reach the liver, it's quickly converted into triglycerides, fatty components linked to a variety of problems, such as cardiovascular disease and even obesity.

5. Sucralose

This sweetener is twice as sweet as saccharin and three times as sweet as Aspartame, and commercial companies use it mainly in pastries or products that require a long shelf life. Sucralose is often marked as E955. While this is an ingredient approved for use by the FDA, various studies suggest that it may negatively affect our body, but its long-term health effects are not clear.

A small-scale study of obese people that did not consume artificial sweeteners in the past found that their blood sugar levels were increased by 14% and insulin levels increased by 20%. Another study found that adding sucralose to a mouse’s diet had negative effects on the intestinal microbial environment.

Other researchers have advised against the use of sucralose in cooking and baking because they found that at high temperatures, substances that could damage intestinal health were released. Hence, although no conclusive evidence of damage to humans has been found due to the use of sucralose, it is recommended to reduce use as much as possible and to avoid cooking and baking with it altogether.


In conclusion:
Many of us assume that as long as we don’t use artificial sweeteners in large quantities, there is nothing to worry about. In the case of saccharin, for example, the recommendation is to consume no more than 18 tablets per day for an adult weighing about 70 kilograms - a quantity that is, of course, far from most of our daily consumption.
However, it is important to know that many drinks and foods combine a number of artificial sweeteners to eliminate their aftertaste and contribute to the shelf life of different products, so we actually consume a much greater amount of sweeteners than we might think.

The most recommended solution is to reduce the amount of artificial sweeteners we consume as much as possible, carefully read the labels of processed food we buy, and to know the healthy and natural sugar substitutes found in almost every kitchen that are suitable for cooking, baking and adding flavor to our favorite foods.

How safe are artificial sweeteners?

Sucralose is made from sugar and is made from a chemical process where the chlorine atoms replace the Hydrogen-oxygen atoms.

There are several alternatives to sugar available to those watching their weight or diabetics. These alternatives promise less sugar but are not always good for consumption by children.

Some of these options can be quite questionable. A report published by the American Academy of Paediatrics (AAP) said that more research needs to go into sweeteners and their effect on kids with risk of type 2 diabetes and obesity.
The AAP will also soon make it mandatory to mention the amount of sweeteners present in each product so that families can keep a track of their consumption levels. They will also be required to mention any non-nutritive sweeteners present in the food.

“Considering how many children are regularly consuming these products — which have become ubiquitous — we should have a better understanding of how they impact children’s long-term health,” said the lead author and a paediatric cardiologist.

What are non-nutritive sweeteners?
Studies have shown that these types of sweeteners are consumed by 1 in 4 children and 80 per cent of these kids consume them on a daily basis.
There are 8 such types of sweeteners in the market that are approved by the Food and Drug Administration (FDA). They are:

Saccharin: This is 300-400 times sweeter than sucrose but it leaves a bitter, metallic aftertaste in the mouth. This is often used to sweeten yoghurt, candies, drinks and even medicines. Studies show that this, along with aspartame, acesulfame potassium, and sucralose should be avoided as far as possible. There is a lack of research on how safe they are, therefore it is advisable to regulate consumption.
 
Aspartame: Aspartame is 200 times sweeter than sucrose and it is most commonly used as a sugar substitute in fast food and drinks such as diet soda. It is also the main component for artificial sweeteners available in the market.

Acesulfame potassium: It is used to lend sweetness to food and drinks without adding any calories. It is most commonly added to fruit drink and they generally have ‘no sugar added’ written on the packaging. It is one of the artificial sweeteners that is well researched about.

Sucralose: The famous Splenda is made from sucralose. Sucralose is made from sugar and is made from a chemical process where the chlorine atoms replace the Hydrogen-oxygen atoms.

Neotame: This is quite sweet compared to the others and is 8000 times sweeter than sugar. It enhances the existing flavours of the food.

Advantame: This sweetener is a derivative of aspartame. Its widely used in sweet beverages, soft drinks and in baking products.

Stevia: This is naturally derived from the leaves of the stevia plant. When consumed in moderation, it is safe and won’t cause side effects.

Luo Han Guo/Monk Fruit: This sweetener is generally considered safe by the FDA but  as there is not enough testing done, people are advised to use it with caution.

What Is an Artificial Sweetener?

You may hear many names for sweeteners: sugars, reduced-
calorie sweeteners, low-calorie sweeteners. Only some of 
these sweeteners are "artificial." Use this list to compare 
sweeteners:
  • Sugars are naturally occurring carbohydrates. They 
  • contain calories and raise your blood glucose levels -- the 
  • level of sugar in your blood. Examples are brown sugar, 
  • cane sugar, confectioner's sugar, fructose, honey, and 
  • molasses.

  • Reduced-calorie sweeteners are sugar alcohols. These 
  • sweeteners have about half the calories of sugars and are 
  • considered a separate type of carbohydrates. They can 
  • raise your blood sugar levels, although not as much as 
  • other carbohydrates. Examples include isomalt, maltitol, 
  • mannitol, sorbitol, and xylitol. You'll often find these 
  • reduced-calorie sweeteners in sugar-free candy and gum.

  • Low-calorie sweeteners are "artificial." This means they 
  • were created in a lab rather than found naturally. Low-
  • calorie sweeteners are considered "free foods." They have 
  • no calories and do not raise your blood sugar levels.

Types of Artificial Sweeteners for Diabetes Patients

 These artificial sweeteners are considered  to be safe for 
use by the general public. 

  • Saccharin can be found as Sweet 'N Low and Sugar Twin. 
  • You can use it in both hot and cold foods. Avoid this 
  • sweetener if you are pregnant or breastfeeding.

  • Aspartame is found as NutraSweet and Equal. You can use 
  • it in both cold and warm foods. It may lose some sweetness 
  • at high temperatures. People who have a condition called 
  • phenylketonuria should avoid this sweetener.

  • Acesulfame potassium or acesulfame-K is found as 
  • Sweet One, Swiss Sweet, and Sunett. You can use it in 
  • both cold and hot foods, including in baking and cooking.

  • Sucralose is found as Splenda. You can use it in hot and 
  • cold foods, including in baking and cooking. Processed 
  • foods often contain it.

Finding Artificial Sweeteners for Diabetes Patients in 

Prepared Foods

No sugar, low-sugar, naturally sweetened, no added sugar -- 
the list of what you encounter on products while shopping can 
be overwhelming. Use this "cheat sheet" to identify which 
products are sweetened the way you want them.
  • No sugar means the product does not contain sugar at all. 
  • It may contain sugar alcohols or artificial sweeteners.
  • No added sugar means that during processing, no extra 
  • sugar was added. However, the original source might have 
  • contained sugar such as fructose in fruit juice. Additional 
  • sweeteners such as sugar alcohols or artificial sweeteners 
  • might have been added.
  • Sugar free means that the product contains no sugars. It 
  • may contain sugar alcohols or artificial sweeteners, 
  • however.
  • Dietetic can mean a lot of things. It's likely that the product 
  • has reduced calories.
  • All natural simply means that the product does not contain 
  • artificial ingredients. It may contain natural sweeteners, 
  • such as sugars or sugar alcohol.

When in Doubt, Read the Nutrition Label

To know for sure what kind of sweetener a food product 
contains, check the Nutrition Facts label. Under the 
Carbohydrate section, you can see how many carbohydrates 
the product contains. You can also see how much of these 
carbohydrates are in the form of sugar or sugar alcohol.
For even more information on diabetic nutrition, read the 
Ingredients list. It should indicate any added sweeteners, 
whether they are sugars, sugar alcohols, or artificial.


So, after reading all these, diabetics should know what type of sweets are allowed for them, which will not raise their blood sugar levels.
