Stephanie Hopkins, a Nurse Practitioner at NYU Langone, agrees that most healthy individuals can manage their symptoms at home. “Over-the-counter medications won’t shorten the length of the illness but may be helpful for mild upper respiratory symptoms, including fever, cough, sore throat; and nasal and sinus congestion,” she said, adding that it’s also a good idea to contact your healthcare provider for individual advice.

Dr. Fisher recommends looking for cold medicine that contains acetaminophen to reduce minor fever and aches, a decongestant to minimize nasal pressure, and throat lozenges for irritation. Hopkins notes that it’s suggested to use acetaminophen (like Tylenol) instead of Ibuprofen (like Advil) as a fever reducer in suspected or confirmed coronavirus cases. “If you have chronic liver or kidney disease or ever had a stomach ulcer or GI bleeding, talk with your health care provider before using these medicines, “ she adds.

According to Hopkins, other over-the-counter medicines that can help alleviate symptoms include cough expectorants like Mucinex or the generic version for the ingredient guaifenesin, cough suppressants like dextromethorphan or Robitussin, and decongestants like Sudafed or pseudoephedrine—though anyone with high blood pressure, glaucoma, or thyroid conditions should avoid decongestants.

Dr. Fisher and Hopkins stress that if your symptoms worsen or become severe—for example, if you have a fever over 100.4 degrees Fahrenheit (for adults) or difficulty breathing—you need to call your doctor or get medical help immediately.

Chromosome Length May Indicate Development of Respiratory Infection

The rate of experimentally induced upper respiratory infection increased in healthy adults with shorter telomere length (a structure at the end of a chromosome) in specific leukocytes, according to research in JAMA. The results were identified in comparison to participants with longer telomeres.

Telomeres, DNA-protein complexes at the end regions of chromosomes, decrease in length with every cell division.

“Telomere shortening in leukocytes (white blood cells) has implications for immunocompetence and is associated with increased synthesis of proinflammatory cytokines and poorer antibody response to vaccines,” according to background information accompanying the article. “Shorter leukocyte telomere length also is associated with aging-related morbidity and mortality from conditions with immune system involvement, including infectious diseases, cancer, and cardiovascular disease.”

Investigators assessed telomere length in 152 healthy adults, then quarantined participants to administer nasal drops containing a common cold virus; participants were then monitored for 5 days for development of infection and clinical illness.

Among all patients, 69% developed respiratory infections and 22% developed a cold. An association was seen between those with shorter telomere lengths in all 4 cell types and increased odds of infection following exposure to the virus.

Those with the shortest CD8CD28 telomere length had a rate of infection of 77%, compared to 50% for those in the group with the longest telomeres. Among participants with the shortest telomeres, 26% became clinically ill. The rate for clinical illness was 13% for those in the group with the longest telomeres.

“In this study of healthy young and midlife adults, shorter CD8CD28 cell telomere length was associated with upper respiratory tract infection and clinical illness following experimental exposure to rhinovirus,” the authors concluded. “Because these data are preliminary, their clinical implications are unknown.”

Children up to 4 years worst target of antibiotics prescribed by private doctors

  • A study shows that from May 2013 to April 2014 antibiotic prescription rates were highest for children aged 0–4 years.

Children between 0 to 4 years have been found to be the most targeted segment among patients for antibiotics prescribed by private doctors, a study has shown.
 
The study done by Public Health Foundation of India (PHFI), a public private initiative in public health has revealed that from May 2013 to April 2014 antibiotic prescription rates were highest for children aged 0–4 years. Also, approximately one-fifth antibiotic prescriptions were dispensed for upper respiratory infections, which rarely require an antibiotic therapy, the study said.

The researchers used a 12-month period (May 2013 to April 2014) medical audit dataset from IQVIA (formerly IMS Health) an American Contract Research Organization and Pharmaceutical Service Consulting. The researchers also calculated and reported antibiotic prescription rate per 1,000 persons per year, by age groups, antibiotic class and disease conditions.

Presenting new evidence on outpatient antibiotic prescription rates and pattern in the private sector in India, the study said that around 519 million antibiotic prescriptions were dispensed in 2014, which translate into 412 prescriptions per 1,000 persons per year. The antibiotic prescription rates were found to be highest for children aged 0–4 years (636 prescriptions per 1,000 persons) and lowest in the age group 10–19 years (280 prescriptions per 1,000 persons).

“Our estimates suggest high proportion of antibiotic prescription for upper respiratory tract infections (acute upper respiratory infections (20.4%), cough (4.7%), acute nasopharyngitis (4.6%), and acute pharyngitis (3.9%)). Generally, these infections are viral in origin and are self-limiting in nature," said Sakthivel Selvaraj, director, Health Economics, Financing and Policy, PHFI.

The findings also stated that the prescriptions for unspecified acute lower respiratory infection were 12.8%; disorders of urinary system were 6.0%; 4.7% for cough; and 4.6% for acute nasopharyngitis.
“In the light of evidence based medicine and standard treatment guidelines, it may be argued that a significant proportion of these antibiotic prescriptions might be inappropriate in nature. Previous research on prescription practices also highlighted the problem of inappropriate use of broad-spectrum antibiotics in India," Selvaraj said.

The findings highlighted that across all age groups, beta-lactam, cephalosporins had the highest prescription rates (38.3% of all antibiotic prescriptions) followed by beta-lactam, penicillins (22.8%) and quinolones (16.3%). “Cephalosporins were the most commonly prescribed antibiotic across all diagnoses with the exception of disorders of urinary system where quinolones were more commonly prescribed," said Selvaraj.

The researchers have argued that overall antibiotic prescription rates in India are still much lower than Europe. However, prescription rates for broad-spectrum beta-lactam antibiotics are much higher as compared to European nations, especially in children.

“Our findings highlight that primary care physicians in the private sector can play a key role in reducing antibiotic misuse and overuse. This baseline information can also be used as a benchmark for measuring the impact of current and future interventions directed towards reducing inappropriate antibiotic use," said Selvaraj.
