Two strains of one bacterium combine to cause deadly flesh-eating infection

A new study used genetic analysis to reveal how two different strains of a single species of flesh-eating bacteria worked in concert to become more dangerous than either one strain alone. The work suggests that other difficult-to-treat infections may be polymicrobial and treating only one organism in a polymicrobial infection could be the cause of many secondary infections and chronic infections that resist treatment. The results of the study were published in the Journal Proceedings of the National ,Academy of Sciences, 2019.

In recent years, scientists have found that serious infections that progress rapidly and resist treatment are often caused by multiple microbes interacting with one another. Very little is known about these so-called polymicrobial infections, but traditional diagnostic methods often misidentify them as monomicrobial, or single-microbe, infections.

"This research provides clear evidence that a very severe infection considered to be caused by a single species of a naturally occurring bacterium actually had two strains," said Rita Colwell, a Distinguished University Professor in the University of Maryland Institute for Advanced Computer Studies and a co-author of the study.

"One of the strains produces a toxin that breaks down muscle tissue and allows the other strain to migrate into the blood system and infect the organs." The original infection -cultured from a patient who developed the severe flesh-eating disease known as necrotising fasciitis- was diagnosed as a monomicrobial disease. Traditional diagnostics could only determine that the infection was caused by a single species of bacteria called Aeromonas hydrophila.

But the disease baffled clinicians when it rapidly turned deadly, requiring a quadruple amputation to save the patient's life. Through genetic analysis of the culture, Colwell and her team discovered important differences among the individual bacterial cultures that could not be detected through standard diagnostic methods.

The ability to identify the agents involved in polymicrobial infections, whether they are different species or variant strains of a single species, can significantly improve treatment outcomes for infected patients.

"When we treat with a given antibiotic, we're clearing an organism out of the body," Colwell said. "But if there's another organism that's participating in the infection and that's also pathogenic, then any antibiotic treatment that doesn't also target that organism may just be clearing ground for it to grow like crazy."

Treating only one organism in a polymicrobial infection could be the cause of many secondary infections and chronic infections that resist treatment. According to Colwell, a mixture of antibiotics or therapeutic drugs may be necessary to treat polymicrobial infections. Routinely using the genomic approach that was developed in this study to analyze infections could result in more effective targeted treatments for diseases caused by polymicrobial infection.

Grass pollen can help predict asthma, hay fever

The presence of different strains of grass pollen in the atmosphere can help predict when hay fever and asthma could strike, researchers have found.

A team of researchers, tracked grass pollen for seasonal variations and found it was released into the atmosphere later in areas further from the equator.

“Using this method, we may be able to better predict when allergenic pollen is present and allow people affected by asthma, chronic obstructive pulmonary disease (COPD) and rhinitis to more effectively manage their condition,” said an Associate Professor.

“(And) with the advent of personalized medicine, more and more people are becoming aware of which allergen is responsible for their allergy,” the Associate Prof. said.

He said the research would help allergy sufferers prepare for the hay fever season and doctors to prescribe more personalised treatments.

“People who fail to manage their asthma are at greater risk of asthma attack and being forced to visit hospital emergency departments,” he said.

“Having a more accurate forecast of when a patient is at risk will allow people to better manage their disease.”

Scientists hope to expand on the research to create a unique profile of each grass pollen species to determine the most harmful strains.

For this, they are examining hospital and GP records and seeing if demand for these services involving asthma and rhinitis correlates with the presence of one grass species over another.

“Eventually – possibly within three to four years – we hope this will allow us to produce a better forecast of when and where exposure to pollen occurs,” he added.

Many Causes of Chronic Back Pain and How to Treat Them

Back pain can be a real… well, pain. It limits mobility and often shoots up to the shoulders, spine and can cause neck pain so intense that it can seem like a migraine. Roughly eight out of ten Americans will experience severe back pain at some point in their life, and it is the leading musculoskeletal reason for sick leaves in the U.S. There can be a wide array of reasons for recurring back pain, which makes identifying the cause rather difficult. Here are several possible explanations for the back pain that’s hounding you:

1. Underdeveloped Lats
The latissimus dorsi (or lats, as it's commonly referred to) is the single largest muscle in the human torso, and also one that’s rather hard to build up, as it is impossible to isolate in training. Strong lats provide support to the spine and a stable posture. Conversely, underdeveloped lats combined with overdeveloped frontal muscles or excess fatty tissue in the chest or abdomen can strain the back, causing pain. The best exercises for the lats are rowing and pull-ups.
2. Bloating and Other Digestive Problems
While not connected directly to the back muscles, gastrointestinal conditions which cause bloating or inflammation of the stomach and intestines (such as IBS, ulcerative colitis and celiac disease) may cause pain to radiate to the back, due to the proximity of the digestive tract to spinal nerves.
3. Strains and sprains
Injuries to the back muscles and ligaments can be a major reason for back pain, which can be the result of careless physical activity (such as lifting heavy weights) and poorly-developed back muscles. Though sprains and strains are often simple enough to address, we should nevertheless treat them as a wake-up call, as an unconditioned back can lead to all sorts of misfortunes, such as:
4. A herniated disc
Chronic back pain: herniated disc

The spinal discs are rubbery rings of cartilage surrounding a nucleus of gel. A tear in the outer ring causes the gel to surge forth, pressing against the nerves, causing acute pain. One way to recognize a herniated disc is if the pain shoots down to the legs.
5. Pregnancy
Though usually a cause for celebration, pregnancy can cause serious back pain as weight starts shifting to the front and hormones do their number on the body. By exercising and practicing better posture, some of these pains can be alleviated.
6. Endometriosis
Chronic back pain: endometriosis

This cruel malady affecting women causes uterus lining tissue to grow outside of the uterus, causing intense pelvic and lower back pain, and ultimately sterilization. If the pain is especially acute during menstrual periods, this may be a sign of endometriosis. Though endometriosis is currently incurable, there are treatments that inhibit pain and delay tissue growth. Due to potential loss of fertility, it may be crucial to catch endometriosis in time and seek treatment.
7. Fibromyalgia
Chronic back pain: fibromyalgia

Another vicious medical condition that affects women at a disproportionate rate, fibromyalgia causes widespread chronic pain (including the back), fatigue and depression. Like endometriosis, there is no known cure, but symptoms can be managed through medication, therapy and exercise. Treatment aimed at addressing the mental state of those suffering from fibromyalgia has been shown to be effective in curbing its physical manifestations, as well.
8. Depression
Though not a cause for chronic pain in and of itself, people who complain about chronic pain suffer from depression at an inordinate rate, which is easy enough to understand, considering how disheartening chronic pain can be. And while depression may not be the source of your ailment, it certainly exacerbates it. As mentioned before, by treating the depression aspect of chronic pain, many people have reported an improvement in physical symptoms.
9. Cancer
While not the most common cause for back pain by any stretch of the imagination, several strains of cancer can cause back pain. Obviously, spinal bone cancer will cause pain in the back, but any cancer affecting organs in the vicinity of the spine (such as colon, breast, testicular and lung cancer) can be the catalyst for back pain. About 25% of people diagnosed with lung cancer have reported experiencing back pain as a symptom.
10. Arthritis
Osteoarthritis, a condition characterized by degeneration of joint or spinal cartilage, may be the offender in cases of back pain. Patients suffering from osteoarthritis of the spine often report weakness and numbness in their limbs. As a degenerative disease, it’s unfortunately incurable, but can be managed by weight loss, physical therapy and painkillers.

4 Crucial Things You Need to Know About the Flu Shot

Flu season is around the corner, and according to the experts, the best way to ward off the virus is to get the flu shot. The information below will guide you through factors about the flu shot you may not know. This includes the best time to get the shot (and no, as soon as possible is not the right answer); the best vaccine to ask for (there are several to choose from); where to go for it; and how much it will cost, as well as how effective it will be. Below are four crucial factors you need to know about this year's flu shot:

1. When should you get the flu shot?
The flu vaccine is widely available in most doctors' offices and pharmacies. But you may want to wait to get yours until October - this is the optimal time to get the vaccine, according to the experts. The doctors also recommend getting vaccinated by the end of October. The reason is that some researchers suggest the vaccine's effectiveness begins to drop after four months or so, particularly in older adults.
The flu season typically runs from October to March, but it is possible that you may still get the flu in May. For this reason, getting your shot too early could leave you more vulnerable to the disease later on, particularly toward the end of the peak period for catching the flu, which is primarily from December to February. In case you're worried you miss the window, it's never too late to get vaccinated, even in November, December or January.
2. Which type of shot should you get?
There are several different types of flu vaccines, all of which have been updated for the 2018-19 flu season to better match the viruses that will be circulating. Nearly all of this year’s varieties will be quadrivalent vaccines, which protect against four different strains of flu. If you are over 50, but younger than 65. Any of the regular-dose vaccines are fine, except for nasal spray, which is recommended for people up to the age of 49.
Those aged 65 or older who are especially vulnerable to the flu and often get sicker when they come down with it should ask for one of two stronger vaccines available. They can either opt for the high-dose version, Fluzone High-Dose, which contains four times the amount of antigen as a regular flu shot. Or FLUAD, a vaccine with adjuvant, an ingredient that creates a stronger immune response.

3. Where should you get the shot?
You can get the vaccine from your doctor, drugstore chains and most local supermarkets. Many supermarkets and stores also sell the Flu shot. Some offices also offer the option of getting the flu shot done at work. According to a study conducted nearly 60% of employers now offer on-site flu vaccinations. As such it doesn't matter where you get the flu shot from. Just get it from somewhere where it is convenient for you.
Bonus: Is the flu shot safe and effective?
The flu shot is very safe and it is a myth that the shot can give you the flu. The vaccine takes 10 to 14 days to become effective after getting the shot, so it is possible that exposure to the flu in that window may make you sick. It typically lowers your risk of getting sick by 40% to 60% and reduces the severity of the disease if you do catch it.
However, in any particular year, how effective the vaccine will be is what's on everyone's minds, since the viruses in circulation change from year to year. You can further reduce your chances of getting the flu with simple, commonsense precautions, this means staying away from people who are sick and washing your hands frequently. Following this advice has been shown to reduce the spread of the disease by up to 50%.

Causes Of Knee Pain

The knee joint consists of small bone structures, the kneecap, supporting ligaments and cartilage of the knee. This joint bears the full weight of the body, which makes it very vulnerable to injury.
Causes of knee pain
Knee pain is usually caused by injuries to the knee such as exposure to a direct force on the knee, abnormal twisting of the knee or falling on your knees. Some causes of knee pain are -

  • Knee strains and sprains - Overworking the knee joints can damage the tissues of the knee joint resulting in knee sprains and strains
  • Osteoarthritis - it is condition where the protective cartilage around the kneecap is damaged, resulting in knee pain
  • Bursitis - Excessive movement of the knee or kneeling down for extended periods can irritate the bursa (a fluid sac below the skin above the knee joint) that causes swelling and pain, thereby giving rise to this condition
  • Gout - This condition is characterized by the accumulation of uric acid in crystal form around the knee joint, causing inflammation and pain
  • Tendon disorders - Tendons connect the muscles to the bones, and overworking the knee can cause the tendons around the knee to become sore and painful
  • Kneecap dislocation - Injuries may cause the kneecap to shift out of its position, this causes swelling and pain in the knee joint
Symptoms of knee pain

Severe knee pain can restrict movements such as walking or standing. The symptoms vary according to the extent of the damage suffered by the knee. Most common symptoms of knee pain are problems in climbing stairs, inability to extend the knees, limping and swelling of the knee joint. In some cases, knee pain may also cause fever. If you wish to discuss about any specific problem, you can consult your physician or orthopedic.
