Cancer growth in the body could originate from a single cell – target it to revolutionise treatment

Cancer remains a frightening and largely incurable disease. The toxic side effects of chemotherapy and radiation make the cure often seem as bad as the ailment, and there is also the threat of recurrence and tumour spread.

Cancer treatment still follows a practically medieval method of cut, burn or poison. If the growth can’t be cut out through surgery, it may be burnt away with radiation or poisoned by chemotherapy. As a result, cancer therapy remains a daunting diagnosis for patients and treatment options seem limited for a disease which causes one in six deaths globally.

The failure to innovate in cancer treatment may lie in the very poor success rate of clinical trials. Approximately 95%-98% of new anti-cancer drugs actually fail phase III clinical trials, the phase in which new treatments are compared with existing therapy options. This is a staggering statistic. No other business could possibly survive with such an abysmal success rate.


 Chemotherapy and radiotherapy are broad-based treatments which attack the bulk of cancer cells but also damage healthy tissue. 


Most drugs are made to target “bulk” cancer cells, but not the root cause: the cancer stem cell. Cancer stem cells, also known as “tumour-initiating cells”, are the only cells in the tumour that can make a new tumour. New therapies that specifically target and eradicate these cancer stem cells are needed to prevent tumours growing and spreading, but for that there needs to be more clarity around the target.

Our new research may have discovered such a target. We have identified and isolated cells within different cancerous growths which we call the “cell of origin”. Our experiments on cancer cells derived from a human breast tumour found that stem cells – representing 0.2% of the cancer cell population – have special characteristics.


They generate vast amounts of energy and proliferate rapidly. We believe that they resemble the cancer cell of origin that has escaped senescence – the natural process of cell ageing and “death” which concludes a healthy cell life cycle. These are thought to be the first cancer cells which start the process of uncontrolled cell multiplication and cause tumours to form.

These cancer stem cells undergo anchorage-independent growth, also known as growth in suspension, without any tissue attachment. This is how metastasis occurs – spreading via the blood vessels and lymphatic vessels. These features put them front and centre as a new target for anti-cancer therapy.
 

Cancer stem cells grow in suspension in the bloodstream and spread throughout the body. 

With astonishing luck, these energetic cancer stem cells are colour-coded which means they have a natural phosphorescent glow, making them easy to identify and target.

Now that we have found them and we know how they behave, it should be relatively simple to find drugs to target cancer stem cells. In our new paper we have already shown that they are easily targeted with a mitochondrial inhibitor or a cell cycle inhibitor such as Ribociclib, an FDA-approved drug in the US which would prevent their proliferation.

Ultimately, this means that if we focus on energetic cancer stem cells, we may be able to directly hit the target. We might be able to turn cancer into a manageable chronic disease, like diabetes. We believe that we have arrived at the start of a new, more fruitful, road in cancer therapy. As a consequence, “big pharma” drug screening should actually focus on cancer stem cells and their relevant targets.

A scientist builds' breakthrough" cytotron device for cancer treatment

" Cytotron stops the cancer tissue from multiplying. The device also inhibits it from spreading to other organs.

Earlier this month, the ' Cytotron', a medical invention by Dr. Rajah Vijay Kumar, a Bengaluru based scientist, received the honour of being called a ' breakthrough devices' in cancer care by the Centre for Devices and Radiological Health, a branch of the U.S> Food and Drug Administration (USFDA), responsible for the pre-market approval of all medical devices.


The machine, " intended to stop the cancer tissue from multiplying, growing and spreading", is the outcome of 32  years of continuous research and had been designated as a breakthrough device since it is an entirely new way to treat cancer.


Speaking to the media, Dr. Kumar, who is the Chairman and Chief Scientific Officer at the Centre for Advanced Research and Development (CARD), a research wing under the Organisation de Scalene Foundation, talked about the happiness and hope at receiving this letter of approval.


It is something we've worked on for more than 3 decades, and research is till ongoing. Naturally, the process of developing any new medical device takes a long time taking into account the need for a lot of tests, addressing safety aspects and adverse effects. Nevertheless, in receiving the ' breakthrough device' designation from the USFDA, we feel that our work has been good enough. We hope it will also offer a breakthrough in the lives of cancer patients, he said.


The cytotron works on the tissue engineering principles of human cells, altering how certain proteins are regulated to prevent them from multiplying and spreading in case of cancer, or promote growth of cells where required, and is essentially a new way to think about repairing cellular disorders.


We have moved away from ' destroying, killing or obliterating cancer tissues'  to a method of communicating with the cellular command and control, and stop it from growing and spreading to other parts of the body. Cancer cells are cells that have just gone out of control. By appropriately addressing the right methodology and specific protein pathways, you can drive the cancer cells into old age, push them into accelerated ageing, and finally. apoptosis   ( programmer cell death). Once this stage is achieved, your body's immune system takes over time and the cancer cells are recycled. This is the philosophy of Cytotron, he argues.


So, what does Cytotron do in cancer cells?


Any cell in our body can multiply anywhere upto 50 times before becoming senescent, which means it can no longer further that process. After the 50th division, there is a flag planted down, where a protein called p53, a programmed cell death protein, is expressed in old age cells. This ensures the cell doesn't multiply any further.


This process is driven by the pro-apoptotic protein p53, which regulates apoptosis. Unfortunately, in cancer cells, this process doesn't happen because the protein is updated or is not expressed, as they do not receive the signal to do so.


Naturally, these cells don't know when to stop multiplying and eventually progress to grow as an organ in the body or a life form. As they grow, they land up with a resource crunch.


The, these cells look are forming new colonies and start spreading with their ' survival instincts' taking over. Cancare produces a very special kind of cell, called EMT cells or cancer stem cells ( CSCs), that spread to another location and then it builds up new colonies.


As long as the cancer is growing, the body's immune system does not attack it. On the contrary, it protects the cancer tissue. Cancer cells take advantage of this situation, and conquer different organs of the body, until the body can't function normally.


Cytotron stops the cancer tissue from multiplying and spreading to other organs Once the tumour stops growing or spreading, what we do at Cytotron is plant the flag (protein) down, which determines that the cell is old enough to die. The moment this happens, the cells stops growing and once that happens, the 1st reaction of the immune system is to grab the cell and throw  it out. The cancer tissue is recycled, and this device also works to convert or transform the cancer stem cells into cells that don't multiply. These are 2 things that happen when we treat a cancer patient with Cytotron. Depending on which stage of cancer you're being treated at, it stops the cancer cells from spreading, The earlier you go inside the Cytotron, your cancer cells get arrested at that point. Once you stop the tumour from growing and spreading, there are other way you can manage the disease, says the Dr.


The Cytotron itself is a vast machine and has 1000s of components. Genetically known as rotational field quantum magnetic resonance, Cytotron produces fast radio bursts, which modulate the life systems and communicate with cells in the body, at present, the machine is being made at the CARD campus in Bengalaru. When we need to scale up production, we may rope in another entity, he says.
The device is approved for clinical use by the Medical Devices Directive (MDD) of the European Council, the SFDA in the Middle East and the Commision Federal para la Proteccion contra Riesgos Sanitarios (COFEPRIS) in South America. Cytotron cancer treatment is currently in use in about 9 countries and has treated 100s of patients already, claims Dr. Kumar.


In India, this machine is not yet available, although they give free treatment to as many people at their centre. From January 2020, they will start supplying these machines to hospitals and institutions.

However, addressing cancer is just one such aplication. The development of this machine is only one aspect of their broader research in tissue engineering, like chondrogenesis, where they grow cartilage tissue and bone tissue.


Cytotron first, the rest later


In India, the Cytotron will cost anywhere between 2.5-3 crore Rs., says Dr. Kumar. For patients diagnosed with cancer, he suggests that before any surgery, they should ideally be sent straight into the Cytotron so the tumour can be stopped from growing or spreading.


In my opinion, you essentially cut down the risk of cancer spreading by going through the Cytotron. If there is a tumour that is causing pain or discomfort, you  could do a surgery later on. But before doing it, you must go through the Cytotron. Cancer activity stops, people feel more energetic, they out in weight and the effects are generally good after patients undergo Cytotron treatment , he argues.


While humans haven't found cure for cancer, what we've here is the next best thing !

A Single “Cell of Origin” Might Trigger All Forms of Cancer

A team of scientists are claiming that all cancer growth in the body starts from a single “cell of origin” — and that they’ve found that cell.

If they’re right, the discovery could dramatically change how we treat cancer, potentially saving millions of life.
Not all cancer cells are alike. A particular type, called cancer stem cells, are the only cells that can make a new tumor.

In a paper published, the team describes how its study of cells from a human breast tumour allowed it to identify a sub-class of cancer stem cells, which it calls “energetic” cancer stem cells (e-CSCs).

According to the researchers, these cells produce large amounts of energy and proliferate rapidly. They also grow in suspension, without attaching to any tissues, allowing cancer to spread through blood and lymphatic vessels.

Ultimately, they believe it’s e-CSCs that lead to the growth of all cancers in the body.

“It feels like finding the proverbial needle in a haystack,” a researcher  said in a news release, “and it crucially gives us a new window on cancer and how we might stop it.”

Targeted Treatment

 Now that they’ve identified e-CSCs as the “cell of origin,” the team suggests researchers shift their focus from cancer treatments that target “bulk” cancer cells and toward ones that go after e-CSCs in particular. And thanks to the cells’ natural phosphorescent glow, hunting them down won’t be as difficult.

“Now that we have found them and we know how they behave, it should be relatively simple to find drugs to target cancer stem cells,” he wrote. “In our new paper, we have already shown that they are easily targeted with a mitochondrial inhibitor or a cell cycle inhibitor such as Ribociclib, an FDA-approved drug in the U.S. which would prevent their proliferation.”

New technique increases survival time for glioblastoma patients

The rapid spread of a common and deadly brain tumor has been slowed down significantly in a mouse model by cutting off the way some cancer cells communicate, according to  researchers.

The technique improved the survival time for patients with glioblastoma by 50 percent when tested in a mouse model, said a researcher.

Researchers focused on disrupting the cell-to-cell communication that allows cancer stem cells to spread. For that, they targeted a channel that cancer cells use to transfer molecules. By cutting off their communications pathway, the cancer cells stay in check, it was found.

Glioblastoma is the most common brain tumor in adults and there is no effective long-term treatment and patients usually live for 12 to 15 months after diagnosis. Glioblastoma tumors, which are highly malignant, typically start in the largest part of the brain and spread rapidly.

The research focused on connexin 46, a protein that is an essential component of cancer stem cells and known as gap junction.  It was found that intercellular channel, which allows cells to exchange molecules and ions, is crucial to the growth of a glioblastoma tumor, researchers found. By shutting these channels in the cancer stem cells, researchers could significantly reduce the tumour forming abilities of those cells.

Tumor growth was significantly delayed in mouse models that were treated with a combination of the gap junction inhibitor and a chemotherapy drug. After 100 days, all of the mouse models that had the connexin 46 protein suppressed genetically were still alive. But those  that didn't have the protein suppressed died within two months.

While the technique has yet to be tested in humans, the researchers said the implications are clear and relevant. For now, a glioblastoma patient can expect to survive about 12 to 15 months. Patients can also develop a resistance to temozolomide when it is used for chemotherapy, further shortening their life expectancy.

 The researchers need to find the proteins most effective and tolerable concentration and also understand more about the mechanism which make the protein work.

Treating glioblastoma is especially difficult because its cells can vary drastically, even within a single tumor -- so breaking the chain of cell-to-cell communication is yet another potential weapon to fight the disease. If the new therapy is approved following a clinical trial,  it would likely be put to use alongside traditional chemotherapy and radiation treatments.

To treast such a deadly disease, multiple approach is needed than just one approach for it to be successful.
