Advanced kidney disease patients can survive, with quality life, without dialysis

People with advanced chronic kidney disease who decide against maintenance dialysis treatment still can survive for many years with a high quality of life, an analysis published on Monday found.

However, this may change late in the course of their illness, when many of them may require hospitalization or surgical procedures to address complications due to declines in kidney function, the researchers said. 

"Many patients who forgo dialysis can live for several years and experience sustained quality of life after their decision," study co-author Dr. Susan P. Y. Wong told UPI in an email.

"Our study highlights how dialysis shouldn't be the only option for treating advanced kidney disease, although it can often seem this way to patients," said Wong, a kidney specialist at the University of Washington and the VA Puget Sound Health Care System in Seattle. 

In dialysis, a machine is used to remove excess water and toxins from the blood, according to the National Kidney Foundation.

These are functions normally performed by the kidneys, when they are healthy, the foundation says.

Dialysis treatments can last up to four hours and are often needed several times per week, it says. 

Advanced chronic kidney disease, or when the organs' function has declined enough to be life-threatening, can be a complication of a number of health problems, including Type 2 diabetes and severe heart disease, according to the Mayo Clinic.

However, starting dialysis earlier does not necessarily improve the prognosis in people with advance kidney disease, recent studies suggest, and it may cause side effects, including an increased risk for infections, the National Kidney Foundation says.

As a result, many patients, working with their care team, opt for a more "conservative" approach that includes prescription medications that help manage symptoms and reduce the risk for complications, Wong said.

For this study, she and her colleagues analyzed data from 41 studies that included more than 5,100 patients with advanced kidney disease.

Patients who opted against maintenance dialysis survived four up to four years after forgoing the treatment, the data showed.

Those who skipped the treatments reported improved quality of life and mental well-being, based on results from several commonly used measures, compared with those who decided to pursue dialysis, the researchers said.

However, in a typical year, those who opted against dialysis were admitted to the hospital twice, spent up to 16 days in the hospital, visited a healthcare clinic eight times and needed emergency room care twice due to symptoms and complications of kidney failure, according to the researchers.

"Each option has different pros and cons, and each set of pros and cons will be different for every patient," Wong said.

"I encourage patients to talk with their clinicians about what is most important to them and their health goals," she said.

Dealing with loss looks different for everyone

We are in the third wave of the global pandemic and loss and grief have now become familiar words that have become run of the mill, especially since the second wave hit everybody so hard. In addition to the three waves of coronavirus that claimed several lives, a lot of other reasons have continued to bring grief and loss upon people. While loss is always inevitably followed by grief, it is also common for people to lose complete control while grieving the death of a loved one. Unexpected losses, of life and otherwise, brought on by the pandemic have also been a great reason for people grieving and not being able to recover from the shock and sadness of loss. Clinical psychologists and experts have their own take on the process which will help anyone who is dealing with the two emotions.

To each, his own

Grief comes in different forms to different people. While for some people it may come in the form of an identity crisis, to others it may look like a feeling of disorientation or guilt or at times regret. Other forms include overwhelming feelings, mood swings, feeling of lack of control, loneliness, shock or anxiety. While some people learn to heal themselves, others just learn to live with sorrow. The only way to actual heal from something is to feel it completely and go through the painful process of feeling it till you learn to live with it.

The first and foremost step towards dealing with grief is acceptance, and it is probably the hardest thing to achieve. Acknowledging one’s loss is the most important factor while dealing with grief and embracing the pain of loss by seeking support from those around you should be normalized. No matter how individualistic you might consider the process to be, it is imperative to seek help when things become too overwhelming. Accepting the fact that the loss has brought about a permanent change in your identity is equally important and most importantly, being guilt free about the way you feel makes things very easy.

Survive and emerge victorious

Here are a few tips that experts have shared which will help you survive loss. The first thing to do is to express yourself. It is often said that grief is love, only that amount which has not been expressed. Use art or writing or any other form of self -expression to deal with your feelings. These activities will surely make you feel good. Movement and activity will also help you feel better, therefore, exercising or a simple walk can do wonders for your state of mind during such times. Make sure you invest in something like plants or a pet, both of which demand love, acre and attention. This is another way of channeling sorrow into something worthy. Lastly, deep breathing and grounding exercises will always come in handy in overwhelming times so make sure that you indulge in both.
