Hormonal changes during menopause could worsen arthritis symptoms

Menopause is a testing phase for every woman, especially when it happens earlier than expected. The fluctuating hormonal changes – including hot flashes and vaginal dryness, sleep disturbances and sometimes even anxiety and depression – tend to take a toll on them.

For women who are already suffering from symptoms of rheumatoid arthritis, hitting menopause at the same time could make it worse, a study has said.

According to the study, the hormonal changes caused by menopause make rheumatoid arthritis symptoms worse.

Researchers analysed 8189 women with rheumatoid arthritis and found that pre-menopausal women experienced a slower physical decline than those that were post-menopausal.

Physical function is an important aspect of study in patients with rheumatoid arthritis as it impacts their quality of life. Women experience rheumatoid arthritis at a rate three times greater than men, and also have more severe decline and increased disability, yet the sex-based differences in the condition remain poorly understood.

Previous studies have shown that women with rheumatoid arthritis experience shifts in their disease surrounding reproductive and hormonal life events, such as childbirth.

During pregnancy, women have decreased incidence of rheumatoid arthritis, yet they have an increased incidence of disease development and flare during the post-partum period.

Similarly, women who experience early menopause are more likely to develop rheumatoid arthritis compared to those who experience normal or late menopause.

Given these connections between hormonal or reproductive life events and rheumatoid arthritis in women, researchers conducted an observational study to investigate the association of menopause with functional status in women with the disease.

The results indicate that menopause has a significant impact on the level and rate of functional decline in women with rheumatoid arthritis and is associated with a worsening progression of the effects of the disease.

"Further study is needed as to why women with rheumatoid arthritis are suffering a greater decline in function after menopause," said the paper's lead author.

"Not only is this decline causing suffering for women, it is costly to both individuals and the healthcare system as a whole. Research is specifically needed on the mechanism connecting these variables with the eventual goal of identifying interventions that can maintain or improve function in postmenopausal women with rheumatoid arthritis."

The R 50 miracle drug that prevents maternal deaths from excess bleeding

An inexpensive and widely available drug given intravenously within three hours of childbirth could save the lives of one in three new mothers who would otherwise bleed to death after delivery, reports a study in The Lancet.

A global WOMAN (World Maternal Antifibrinolytic) Trial of more than 20,000 women from 193 hospitals in 21 countries, including India, found that giving drug called tranexamic acid (TXA) within three hours after childbirth reduced death from bleeding by 31% and lowered the need for emergency surgery to control bleeding (laparotomy) in one in three (36%) cases.

TXA stops bleeding by stopping blood clots from breaking down too quickly. Severe bleeding after childbirth (known as post-partum haemorrhage or PPH) is the leading cause of the 100,000 maternal death worldwide.

TXA injections are manufactured by several pharma companies in India and cost between R 30 and R 50.

The study found 89 women given tranexamic acid died compared with 127 given placebo in addition to standard care. There were no side effects for either mothers or babies. The study builds on previous research involving 20,000 trauma patients, which showed that tranexamic reduced deaths due to bleeding by almost a third if given within three hours.

India’s maternal mortality rate (deaths per every 100,000 live births) is 167, shows data from the sample Registration System. In 2015, around 45,000 women died from childbirth-related causes, including post-partum haemorrhage -- defined as a blood loss of more than 500ml -- within 24 hours of giving birth.

“We now have important evidence that the early use of tranexamic acid can save women’s lives. It’s safe, affordable and easy to administer, and we hope that doctors will use it as early as possible following the onset of severe bleeding after childbirth,” said Haleema Shakur, associate professor of clinical trials at the London School of Hygiene & Tropical Medicine and Project Director on the WOMAN Trial.

The drug, however, did not prevent a new mother’s risk of having her womb surgically removal (hysterectomy). In situations where safe blood supply is limited, a life-saving hysterectomy is sometimes done immediately after severe haemorrhaging starts, which left no time for TXA to have an effect.

High BP during pregnancy may raise stroke risk

High blood pressure during pregnancy could dramatically increase a woman’s stroke risk, according to a new research.
A neurology resident , said that they’ve found that women who had high blood pressure during pregnancy could be at higher risk of stroke, particularly if they had pre-eclampsia, which is a more severe form of high blood pressure. He said that the elevated risk of stroke could be as high as 40 %.

The team conducted nine studies specifically looked at hypertension (high BP) during pregnancy and its relationship to future risk of stroke.

The studies followed women for anywhere from one to 32 years after a pregnancy, and found consistent evidence that those with a history of hypertension in pregnancy are more likely to experience stroke in later life.

The exact cause of hypertension during pregnancy is undetermined, but one theory is that some women are genetically predisposed to high blood pressure and the pregnancy brings it on. Even though it might return to normal post-partum, these women need to monitor their blood pressure and to reduce their risk of having a stroke later on.

Poor sleep in pregnancy could lead to complications

Scientists have shown that inadequate sleep during pregnancy can lead to complications and hinder normal immune processes, says a study.

Women with depression are more likely than non-depressed women to suffer from disturbed sleep and adverse pregnancy outcomes.

"Our results highlight the importance of identifying sleep problems in early pregnancy, especially in women experiencing depression, since sleep is a modifiable behaviour," said the lead author of the report. "The earlier that sleep problems are identified, the sooner physicians can work with pregnant women to implement solutions."

There is a dynamic relationship between sleep and immunity, and this study is the first to examine this relationship during pregnancy as opposed to post-partum," added Dr.
