Prostate Cancer Awareness Month 2021: Signs, diagnosis, and treatment of prostate cancer

 In this article, Dr Jagdeesh Kulkarni, Surgical Oncologist shares his insight into the issue of prostate cancer.

Prostate cancer is a form of cancer that originates in the gland cells of the prostate. The prostate gland is found in only men. Did you know? The prostate is located below the bladder and in front of the rectum. The function of the prostate gland is to produce the seminal fluid that aids in nourishment and transports sperm. Are you aware? Prostate cancer is a common occurrence in men. Mostly, this type of cancer will grow slowly and steadily and will be limited to the prostate gland, and may not cause any serious problems. But, some types of prostate cancer may spread quickly and may require aggressive treatment. The majority of men are unaware of this type of cancer, and they tend to delay treatment that leads to serious consequences in later life. Therefore men must educate themselves regarding it, and get a proper diagnosis. This will help to reduce morbidity and mortality.

Prostate cancer - Causes and risks

The definite cause of this cancer is not known yet. But, various studies have confirmed that family history, old age, and obesity can lead to this cancer. You will have to consult the doctor once you notice signs like blood during urination, painful urination, blood in the semen, burning sensation while urinating, frequent urination, interrupted urine flow and enlarged prostate.

Moreover, if cancer spreads outside the prostate gland then you may notice tiredness, weight loss, and back, hip, thigh, and shoulder pain.

How to diagnose it?

Once you notice the symptoms and consult the doctor then he/she will recommend you to opt for the prostate-specific antigen (PSA) blood test to screen for prostate cancer. Further, a per rectal (PR) or digital rectal examination (DRE) is mandatory.

Treatment of prostate cancer

The treatment may vary from person to person depending upon the stage of cancer. One may have to undergo surgery or radiotherapy in the early stages and combination treatments like hormone therapy, radiation therapy, immunotherapy, and bisphosphonate therapy in advanced stages.

Dental care cuts prostate symptoms

Treating gum disease can offer relief to men suffering from a prostate condition that makes urination difficult, new research has found.

Gum disease treatment reduces symptoms of prostate inflammation, called prostatitis, a disease that inflames the gland that produces semen, the findings showed.

"This study shows that if we treat the gum disease, it can improve the symptoms of prostatitis and the quality of life for those who have the disease," said corresponding author Nabil Bissada from Case Western Reserve University School of Dental Medicine.

Bissada explained that gum disease not only affects the mouth, but is a system-wide condition that can cause inflammation in various parts of the body.

The researchers studied 27 men, 21 years old and older. Each had had a needle biopsy within the past year that confirmed inflammation of the prostate gland, and a blood test that showed elevated prostate specific antigen (PSA) levels--possible signs of inflammation and cancer.

Researchers found 21 of the 27 participants had no or mild inflammation, but 15 had biopsy-confirmed malignancies. Two had both inflammation and a malignancy.

All the men had moderate to severe gum disease, for which they received treatment. They were tested again for periodontal disease four to eight weeks later and showed significant improvement.

During the periodontal care, the men received no treatment for their prostate conditions. But even without prostate treatment, 21 of the 27 men showed decreased levels of PSA.

Those with the highest levels of inflammation benefited the most from the periodontal treatment. Six participants showed no changes.

Bissada hopes to make periodontal treatment a standard part of treating prostate disease, much like cardiac patients are often encouraged to visit their dentist before undergoing heart procedures and a dental checkup is advised for women who are pregnant or considering pregnancy.

The findings appeared in the journal Dentistry.
