The drug, Palbociclib targets the rapid division of tumour cells by inhibiting the activity of the enzymes CDK4 and CDK6.

 
A new oral drug whose efficacy in combating breast cancer has been demonstrated alone and in combination with endocrine therapy also has potential to fight other types of cancer, new research has found.

 Palbociclib targets the rapid division of tumour cells by inhibiting the activity of the enzymes CDK4 and CDK6, which propel cell division and increase in number in most cancers.

It is the first CDK4/6 inhibitor to be approved for the treatment of breast cancer, researchers said.

"All living cells undergo cell division and palbociclib's unique capacity to halt the cell division process (also known as the 'cell cycle') therefore has potentially broad applicability," said the study's lead author Amy S Clark, an assistant professor at the University of Pennsylvania.

"Pairing palbociclib with other anti-cancer therapies such as endocrine therapy, chemotherapy, and targeted therapy can create a powerful combinatorial effect with real promise for addressing a variety of cancers," said Clark.

For example, amplification of CDK4 is reported in a high percentage of melanomas and esophageal cancers.

Targeted therapy uses medication and other interventions to more accurately identify and attack cancer cells, usually while doing no or little damage to normal cells.

"This drug has minor effects on normal cells other than neutrophils (white blood cells)," said the study's senior author, Peter J O'Dwyer, a professor at Penn.

"In tumours, it can cause shrinkage, or more commonly, arrest of growth. As we discover new functions for the CDK4/6 target of this medicine, we are likely to use it in combinations to make other anti-cancer agents work better," said O'Dwyer, also director of the Developmental Therapeutics Programme at the Abramson Cancer Centre (ACC) in US.

In addition to inhibiting the cell cycle, palbociclib has been shown, for example to alter several recently described non-cell cycle functions of CDK4/6, a finding expected to expand its therapeutic role, O'Dwyer added.

Assessing 130 relevant publications in the literature, as well as interpreting their own continuing studies, the team found that in addition to its safety and effectiveness in fighting certain types of breast cancer, early trials of palbociclib have shown promise of effectiveness in cases of lymphoma, sarcoma, and teratoma, tumours that while rare, often afflict younger patients.

A phase 2 trial showed that, among 17 patients with previously treated mantle-cell lymphoma, palbociclib resulted in one complete response and two partial responses.

Although, median progression-free survival was four months, five patients had progression-free survival greater than one year.

Another phase 2 trial with 29 sarcoma patients treated with palbociclib showed a progression-free survival of 66% at 12 weeks, researchers said.

The study was published in the journal JAMA Oncology.

Scientists have developed a capsule made from seaweed extract or preserving insulin-producing pancreatic cells, offering hope to diabetics to lead a needle-free life.

Patients with type 1 diabetes require daily insulin injections. Pancreatic islet transplantation is an effective treatment that can dramatically reduce daily jabs or even eliminate dependence on external insulin, researchers said.

So far only human islets can be transplanted and their supply is low.Cryopreservation is the method commonly used for the islet preservation and transportation. However, the freezing process poses a challenge. As the cells are cooled, water in and around them freezes. Ice crystals have sharp edges that can pierce membranes and compromise cell viability .

Researchers led by Amy Shen in Japan developed a cryopreservation method that helps to protect pancreatic islets from ice damage.

The technique employs a droplet microfluidic device to encapsulate pancreatic islets in hydrogel made of alginate, a natural polymer extracted from seaweed. These capsules have a unique microstructure -a porous network and considerable amount of non-freezable water, which does not form ice due to the strong association between water molecules and the hydrogel networks.

Islet encapsulation reduces the risk of rejection of transplanted cells by the recipient. The hydrogel capsule allows small molecules, like nutrients and islet secretions, to pass through the membrane, but prevents direct contact between implanted islets and host cells.

Encapsulation also may prevent an attack on transplants by the autoimmune response that destroyed the patient's own islets in the first place.

Factors like age, illness, stress, diet, exercise, alcohol and smoking can influence how well you produces antibodies, which are essential for combating infectious diseases.

 Researchers have now found out that lower levels of antibodies in the saliva are associated with a high risk of mortality and could also be an early indicator of risk. The study carried out by the team of University of Birmingham have found associations between secretory immunoglobulin A (IgA)--the common antibody found in saliva, and mortality rates in the general population.

The team examined Immunoglobulins, or antibodies, are proteins secreted by white blood cells and are essential for combating infectious disease.

Further analysis revealed that the all-cause association was due to an underlying association with cancer mortality and, in particular with non-lung cancers. There are a number of factors that can influence how well we produce antibodies and maintain their levels.

"There are some that we have no control over, such as age, inherited or illness, but our general state of health can also affect their levels; stress, diet, exercise, alcohol and smoking can all influence those levels," explained Anna Phillips from University of Birmingham.

For the study, 639 adults, from the eldest cohort of the West of Scotland Twenty-07 Study took part in the study.

Participants, aged 63-years old at the time of saliva sampling in 1995, had their IgA secretion rate measured and the mortality rate was tracked over the following 19 years.

"Quite how saliva samples could be used in check-ups remains to be seen as we need to better understand what secretion rate would be considered cause for concern--what we call the protective level," Phillips added.

We could certainly say that, if found to be extremely low, it would be a useful early indicator of risk, the authors added. The study was published in the journal PLOS ONE.

Codeine, a widely prescribed painkiller, can also be purchased over the counter in preparations of cold/cough remedies but over consumption of such medicines can lead to a "confususional" state and loss of the ability to create new memories, says a new study.
There is little evidence showing benefits of codeine in cough remedies, and the risks associated with codeine use in over the counter cough suppressants may be "particularly unnecessary" because of the lack of evidence, the experts said.

An article in the journal BMJ Case Reports described the first published case of confusional state in a healthy 14-year-old girl attributed to excessive consumption of over the counter cough medicine that contained codeine.

Codeine intoxication symptoms often include central nervous system depression, respiratory depression, severe itching of the skin, and flushing. Confusion is a relatively unusual complaint.

However, in this particular case, the teenage girl experienced confusion and the loss of the ability to create new memories.

She falsely reported to have completed tasks, for example, by claiming to have showered when it was obvious to her mother that she had not.

She slept up to 20 hours a day, had a decreased attention span and suffered from intermittent headaches.

Before these symptoms, the patient experienced flu-like symptoms over a 15 day period, during which she was absent from school. She had been taking 2-3 spoonfuls daily of codeine cough suppressant, oral codeine phosphate, over this time.

The patient had not exceeded the recommended daily dosage of three-six spoonfuls, but she had exceeded the maximum recommended duration of usage of three days.

Each spoonful is equivalent to 15 mg of codeine, and the patient consumed a total of 450-675 mg over 15 days, instead of the recommended maximum dosage of 270 mg during any given course of treatment.

A urine test reported positive for codeine, and no other drugs were present.

The doctors warned that there have been many reported child and adolescent deaths following codeine use, and they concluded that "the combination of lack of efficacy, risk of acute intoxication and dependence, suggests that the use of OTC (over the counter) codeine preparations may be unwarranted."
