Health Expert Reveals How Lifestyle Changes Helped Reverse Diabetes

A Type 2 diabetes diagnosis often leaves people wondering if they will have to take medicines for the rest of their lives. Health and wellness expert Luke Coutinho says that, in some cases, better daily habits can help improve blood sugar control and may even help reverse the condition. He shares the examples of two 52-year-old men, both living with Type 2 diabetes and receiving treatment. One followed a very low carb diet, lost weight, exercised hard and tracked his blood sugar. The other controlled his food portions, added protein, vegetables and fibre, walked after meals, worked on losing belly fat and improved his sleep. He also stopped alcohol, managed stress and did strength training.

Both men saw major improvements in their blood sugar levels and, over time, their doctors stopped their medicines.

Targeted lifestyle changes—such as losing 5% to 10% of body weight, eating fewer processed carbs, and exercising regularly—can help reverse or achieve remission of Type 2 diabetes. 
Health experts, including insights featured by NDTV, note that improving daily habits targets the root cause of insulin resistance rather than just masking symptoms. 
Key Lifestyle Changes for Reversal
  • Weight Management: Losing a modest 5% to 7% of body weight lowers fat deposits in the liver and pancreas, helping restore normal insulin production. 
  • Nutritional Adjustments: Cut back on added sugars, processed foods, and simple carbohydrates while increasing your daily dietary fiber. 
  • Post-Meal Movement: Walking for just 10 to 15 minutes right after eating significantly helps clear glucose from the bloodstream. 
  • Consistent Exercise: Aim for at least 30 minutes of moderate physical activity five days a week to make your muscles more sensitive to insulin.
  • Stress and Sleep Control: Managing daily stress keeps cortisol levels low, preventing stress-induced spikes in blood sugar. 

What if Type 2 diabetes wasn’t a lifelong condition but something that could be reversed? For years, managing diabetes meant relying on medication, but recent research has challenged this belief. Targeted lifestyle changes, weight loss, and innovative medical treatments can push Type 2 diabetes into remission. We spoke to Dr Vrinda Agrawal, Consultant Endocrinology, CARE Hospitals, Banjara Hills, Hyderabad, who explained the latest advancements in diabetes reversal and what they mean for the future of healthcare.

"Diabetes reversal means getting your blood sugar levels back to normal without needing medication, although continuous lifestyle changes are pivotal to maintaining remission. Unlike a cure, which means the disease is completely gone, remission means the disease is still there. However, with remission, the disease is under control and blood sugar levels are normal without needing medication," explained Dr Agrawal. Substantial weight loss, dietary changes, and medical treatments can lead to diabetes remission, especially in patients diagnosed within the last five years.

Role of Lifestyle Changes and Weight Loss

Caloric Restriction and Dietary Interventions

One of the most promising strategies in diabetes reversal lies in caloric restriction, as demonstrated extensively in the Diabetes Remission Clinical Trial (DiRECT) conducted in the United Kingdom. This landmark study revealed that people with Type 2 diabetes who adhered to a very low-calorie diet (800 kcal/day) for 3-5 months experienced substantial weight loss, resulting in remission for nearly 46% of participants after one year and 36% of the two-year mark.

"The mechanism behind such reversal involves reducing fat deposits accumulated in the liver and pancreas, restoring normal insulin production levels. Restriction of calories can downregulate genes associated with inflammation and reverse epigenetic changes linked to obesity," said Dr Agrawal. While demanding, such restrictions provide hope that complete recovery from diabetes is possible through sustained lifestyle changes focused on nutrition and weight management.

Intermittent Fasting

"Time-restricted eating plans and low-carbohydrate diets have shown encouraging outcomes. These eating methods aid in controlling insulin sensitivity, decreasing excess fat storage, and upholding balanced blood sugar levels," added Dr Agrawal.

According to a 2022 trial, fasting and carbohydrate constraints can lower HbA1c levels, diminish insulin resistance, and advocate sustainable weight reduction, factors in diabetes remission.

Physical Activity

It helps improve insulin sensitivity and glucose metabolism. Both aerobic exercises, for instance, brisk walking, cycling and swimming, and strength training, such as dumbbell lifts and bodyweight moves, add to more advantageous blood sugar oversight. According to a 2019 study, 23% of participants achieved partial or complete remission after a 12-month exercise-based lifestyle intervention.

Emerging Medical Treatments for Diabetes Remission

Bariatric Surgery

"Procedures, such as gastric bypass and sleeve gastrectomy have emerged as among the most impactful interventions for diabetes remission, especially for those with a BMI over 30. Surgeries not only facilitate significant fat loss but also enhance insulin secretion and glucose processing," said Dr Agrawal. Clinical trials demonstrate upwards of 80% of patients experiencing diabetes going into remission post-operation, with benefits maintained over the long haul, according to the Indian Journal of Endocrinology and Metabolism.

New Medication

New medicines, such as semaglutide and empagliflozin, have shown promise in helping people lose weight and control their blood sugar levels. "These medicines not only lower blood sugar levels, but also help control hunger, burn fat, and improve heart health. While they are not a cure for diabetes, researchers think they might be able to help people achieve long-term remission from the disease," explained Dr Agrawal.

Personalised Medicine and the Gut Microbiota

The gut microbiota has emerged as a key player in metabolic well-being. Modifications in gut microorganisms influence insulin resistance and swelling. Future remedies may involve microbiome-targeted interventions, such as probiotics, prebiotics, and faecal microbiota transplants to restore metabolic balance and aid diabetes remission.

Challenges and Future Directions

"While there have been advancements in diabetes reversal methods, several obstacles still arise. Maintaining significant weight reduction and lifestyle modifications over extended periods is no simple feat, requiring behavioural support, guidance from coaches, and involvement in community initiatives to solidify remission," highlighted Dr Agrawal.

Not every person with diabetes sees remission through identical interventions; personalised treatment blueprints are critical for maximising outcomes. Furthermore, many patients and healthcare providers remain oblivious to the possibility that Type 2 diabetes can be reversed. Broader education, campaigns to spread awareness, and policy shifts are still needed to ensure these remission strategies become widely accessible.

Natalie Dautovic, environmental scholar for the National Sleep Foundation says “A cool sleeping environment is an important ingredient for obtaining a good night’s sleep. Some people find that sleeping with a hand or foot outside of the bed covers can help with body cooling, as heat dissipates quicker through the body extremities.” 

 2. Hug a Pillow 

Sometimes, the only thing you need in bed to help you feel cozier is an extra pillow. The University of Rochester Medical Center says it can keep your body better aligned, preventing back pain. Furthermore, aside from the body benefits, it is also great for anxiety. Experts also say that hugging a pillow is soothing, giving you a sense of comfort, safety, and security.

3. Wear bamboo pajamas

There's more to your sheets and bedding that affects the level of comfort you experience when in bed. What you're wearing also plays a role. According to the National Sleep Foundation, there is a formula for choosing the right sleep attire. Experts recommend breathable and lightweight cotton, silk which keeps your body at the right temperature, and bamboo for its moisture-wicking abilities. 

4. Or, Just Sleep in the Nude 

If you can't find the right fabric to keep you cozy in bed, you can just ditch it altogether and sleep naked instead. Experts say that sleeping naked has been shown to keep you cooler in bed enabling you to get more restful sleep. 

5. Grab the Ear Plugs

6. Turn Down the Thermostat 

While what you wear to bed plays a big role in keeping you comfortable while you sleep, the temperature of the room plays a big role in helping you to drift off to sleep too. The National Sleep Foundation says the best temperature to keep your room at is 65 degrees. This is believed to be the most optimal temperature for sleep, ensuring that it's not too hot or too cold.

In addition, the scent is backed by science in its ability to put you in a more relaxed state. This will help you get into a deeper sleep, making you more calm and comfortable over all.

 Is your mattress pad making you feel too hot? You might need an upgrade. The ChiliPad is a great recommendation which has a cooling and heating temperature control system that can keep your body at the perfect temperature all night.

To feel comfortable in bed, the National Sleep Foundation recommends that you wash your sheets and pillowcases at least once a week. You should also wash your mattress cover and comforter on a regular basis. You can even stick your pillows in the washing machine.

 Not just any pillow is going to meet your needs. There are different types of pillows designed specifically to meet the needs of stomach-sleepers, side-sleepers, and back-sleepers which all have different levels of firmness and materials, keeping your head and neck comfortable. 

11. Keep Screens Out of Your Space
While you can scroll through your phone on the couch all you want, be sure to leave your phones out of the bedroom if you want to stay calm and comfortable. Exposing yourself to light-emitting screens right before bedtime is going to make it harder or you to fall asleep.
