Two get prosthetic heart valves without surgery
In perhaps a first for the city, an 87-year-old man and a 78-year-old woman got their damaged heart valves replaced with prosthetic valves, without undergoing surgery, at a Hospital and Research Centre last month.
In both cases, using only a small puncture in the groin, an artificial heart valve was guided all the way to the heart through a femoral artery as is done in a cardiac stent procedure.
The entire procedure was carried out under local anaesthesia. The patients were conscious but slightly sedated throughout the procedure, fully obeying the doctor’s commands in between.
This is claimed to be the first time that a Pune-based cardiologist has carried out the procedure — Transcatheter Aortic Valve Replacement Procedure (TAVR) — at a city hospital. A cardiologist from abroad had demonstrated the game-changing procedure at a city hospital 12 months ago.
“Both the patients are doing well. Breathlessness, palpitation and chest pain, with which they were living for the last five to six years, have now resolved. Barring a small dose of blood thinner, they are off most symptomatic medications,” hospital’s cardiologist, who carried out the advanced procedure on the patients, said.
The 87-year-old man, a businessman from Baramati, underwent the procedure on March 22. “He had undergone open heart surgery in the past. A repeat open heart surgery to fix the aortic valve was a risky proposition in his case. Moreover, his bones have become brittle and fragile. He also has kidney problem,” the Dr. added.
The man had been suffering from critical aortic stenosis for the past five years. “His heart’s blood pumping rate had reduced significantly — the ejection fraction was 35%,” the cardiologist added.
“He was pulling off with the help of blood thinning drugs, diuretics and blood pressure medications,” said one of the patient’s family members. After the procedure, the man was kept under observation in the hospital’s intensive care unit (ICU) for 24 hours. He was discharged on March 25.
The man’s relative said, “Poor heart functioning had restricted my grandfather’s physical activity. He had also become very negative. Now, within a week of undergoing the procedure, he has started feeling more energetic and positive. He has already resumed most of his physical activities.”
The other recipient of the prosthetic valve was a 78-year-old woman from Khadki. “She has been suffering from asthma for the past six years. Both her lungs are badly affected,” Hardas said. She was also diagnosed with critical aortic stenosis six years ago and was pulling off with the help of drugs. The woman successfully underwent the TAVR procedure on March 22 and was discharged on March 26.
The procedure is unaffordable for many patients. “Cost is the biggest deterrent. The cost of the procedure with an imported valve can be between Rs 25 lakh and Rs 30 lakh. It may come down by lakhs of rupees if an India-made valve is used,” the Dr. said. “Most importantly, a paradigm shift is taking place towards offering less invasive, less mutilating and safer cardiac procedures which are scar-free and well tolerated even in old patients,” the Dr. added.
